    SCOPE OF ROLE: The Case Manager provides and coordinates client services. II. ESSENTIAL DUTIES & RESPONSIBILITIES: • Conduct intake process with new clients within 24 hours of admission; includes but is not limited to explanation of resident rights and responsibilities, and grievance procedures • Conduct psychosocial, vocational/educational, and housing assessments and documentation of same • Provide accurate, complete, and timely documentation of the provision of supportive social services and client progression/regression toward the goals of securing permanent affordable housing • Prepare housing applications and assist clients with the housing application/interview process., • Monitor and assess client progression/regression in behavior and/or appearance, • Facilitate group and individual counseling, workshops, psych education, and recreational activities, • Conduct room/dorm visits/inspections to assess client personal hygiene, room cleanliness, and overall, ADL skills • Provide comprehensive case management services to an assigned caseload, • Develop initial service plans and conduct service plan reviews as needed for an assigned caseload, • Provide referrals for in-house and community-based medical, mental health, and supportive social services to assigned caseload as appropriate • Ensure that clients attend meals and maintain appropriate nutritional intake, • Provide crisis intervention and address all emergency situations as appropriate, • Assist in building evacuation during fire drills and building emergencies, • Monitor client medication compliance and collaborate with nursing staff monitor compliance, • Refer clients to appropriate community resources as needed, • Maintain CPR/First Aid Certification, • Maintain up to date, accurate, complete, and timely client chart documentation in compliance with DHS, OTDA, other Funder and SUS standards • Facilitate discharge/transfer process and documentation of same JOB DESCRIPTION • Attend staff meetings, case conferences, group supervision, training, and escort clients on housing interviews as needed • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes • A work schedule that includes some weekend and evening hours is required, • Other duties as assigned III. REQUIREMENTS REQUIRED EDUCATION AND EXPERIENCE • Bachelor’s Degree is Human Services field preferred, HS diploma or GED required, • Three years’ experience with homeless population, • Ability to work with a culturally diverse population PREFERRED QUALIFICATIONS & SKILLS • Familiarity of NYC public school system preferred, • Experience in residential setting, group work, tenant organization, case management, referral, and/or community outreach strongly preferred

Responsibilities: • Responsible for reading, knowing and complying with all appropriate governing documents to include contracts, agency policies and procedures, State or Federal standards, specific program operations manuals and circulars both internal and external., • Ensures compliance with the agency’s Operations and Personnel Manuals and the Division of Developmental Disabilities Standards Manual., • Completes and submits in a competent, and timely manner all reports, forms, and documentation as required by the agency and Division of Developmental Disabilities., • Understands basic developmental disabilities and learning styles, using motivation techniques to help clients improve in areas with behavioral or medical challenges., • Must complete all CDS trainings and refreshers as directed., • Attend staff meetings, all other relevant meetings and training as directed., • Assist clients with goal outcomes established on the Individual Service Plan (ISP)., • Responsible for reading the ISP and working within the documented expectation for client development., • Provides a consistent means of relaying accurate and complete information to direct supervisors., • Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors according to plans that are in place., • Provides training and supervision to clients in all aspects of community experience living., • Serves as a positive role model for clients and other staff., • Provides safe transportation in agency vehicles for clients as necessary., • Maintains confidentiality and professionalism when communicating with family members or group home staff, Support Coordinators and any outside agency personnel., • Maintains active, positive engagement with clients at all times., • Always ensures the safety and well-being of clients., • Provide direct care and support to individuals with disabilities or special needs., • Proactively accompany and support clients during medical appointments, ensuring relevant information is provided to the healthcare professional. Clearly and accurately communicate details from the appointment to the Residential Coordinator, Medical Compliance Officer, and/or designee., • Follow and demonstrate understanding of medication policy and procedures in the monitoring and administration of the client’s medications. Completes Medication Error Report when needed., • Ensures home, grounds, yard and van are maintained in clean, safe, and sanitary condition., • Ensures meals are prepared and planned based on clients’ individual diet, client input, and the planned menu for the month., • Ensures that the Recreation Calendar is carried out as per client input and as per the planned calendar completed each month., • Responsible for competing all required documentation to include but not limited to, daily logs, communication logs, and behavior reports., • Ensures line of sight for the individual(s) being supported is maintained and completes reports accordingly., • Responsible for adequately and completely cleaning the home during any shift worked., • Ensures that a client’s dignity is not compromised in any way., • Responsible for reporting all emergencies or unusual incidents to direct supervisor., • Cooperates with the licensee and Department staff in any inspection, inquiry or investigation. JOB DESCRIPTION Summary/ Objective East Side House (ESH) provides high-quality, year-round programming in NYCHA community centers and public schools, serving youth and families through education, workforce readiness, recreation, and community engagement. Reporting to the Senior and/or Department Director, the Program Director oversees the development, implementation, and supervision of Early Childhood Education & Elementary Division. This includes daily operations, staff management, compliance, and creating safe, inclusive, and responsive environments that promote positive outcomes. As a member of the Early Childhood Education & Elementary Division, the Program Director is expected to lead dynamic, age-appropriate programming for Elementary age participants and community members. This role includes direct supervision of the Assistant Program Director, Group Leaders, Activity Specialist, and other frontline staff. It requires regular collaboration with Human Resources, Finance, IT, and other East Side House departments. Essential Functions and Responsibilities: Under the supervision of the Senior Director and /or Department Director, the Program Director will: • Provide strategic leadership and day-to-day oversight of youth and adult programs, including evenings, weekends, and summer., • Recruit, hire, schedule, onboard, and supervise full-time and part-time staff., • Monitor performance and ensure compliance with DYCD goals, quality standards, and contractual requirements., • Conduct staff evaluations, provide ongoing coaching, and lead monthly professional development and team meetings., • Oversee the design and delivery of engaging, age-appropriate programming focused on academics, recreation, workforce readiness, and cultural enrichment., • Ensure compliance with DOH School-Aged Child Care and Summer Day regulations, including quarterly inspections., • Train staff on safety protocols and ensure emergency preparedness., • Track attendance and maintain accurate records using DYCD Connect and internal systems., • Manage CACFP Food Program compliance and documentation., • Serve as the main point of contact for NYCHA, DYCD, schools, families, and community partners., • Build relationships with residents, tenant associations, youth councils, and community stakeholders., • Conduct quarterly community assessments and incorporate participant feedback into program planning., • Plan and execute four community events and a series of family workshops annually., • Cultivate at least three new partnerships each year to expand program offerings., • Manage the program budget, process purchase orders, and submit reports on time., • Represent the program in required meetings and maintain regular communication with NYCHA and DYCD., • Collaborate with ESH colleagues to coordinate events, share best practices, and support agency-wide initiatives., • Perform other duties as assigned.
