Job Description POSITION SUMMARY The Client Coordinator is responsible for ensuring that overall operations and services are delivered according to state and federal regulations, agency policies and procedures, and the agency’s mission. The Client Coordinator provides leadership, direction, and training to support the overall development of a specific program. The Client Coordinator also supervises the habilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, the Client Coordinator develops implementation strategies that outline services and supports for individuals as they work toward the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. They are responsible for ensuring service provision by all disciplines represented on the treatment team, and meet routinely with other clinicians for coordination and integration of services. The Client Coordinator coordinates with Program Coordinator(s) and staff to provide individual and day program services, and to maintain a therapeutic milieu conducive to participants’ health and well-being. They coordinate and oversee the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department to complete required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. They hold primary responsibility for developing program goals and methodologies and supporting DSPs to correctly implement and document such activities. The Client Coordinator ensures integrated services by fostering team awareness of service integration, coordinating service delivery with day program services, monitoring service provision by agency clinical staff, consultants, and community vendors, and ensuring services align with IDT treatment recommendations. They ensure a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to maintain adequate coverage levels and ensure an atmosphere conducive to development and well-being. They document progress and update Plans of Care / ISPs according to agency procedures. The Client Coordinator reports regularly to the Supervisor regarding program compliance, noncompliance, and remedial actions. They identify needs and request support as necessary. They participate in clinical reviews, incident reviews, human rights committee meetings, and other meetings as directed. As schedule allows, the Client Coordinator serves as primary chair for Life Plan meetings. They review documentation and provide technical assistance to program management and staff. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES The employee must be able to interact with individuals and staff professionally and ethically, understand and implement service plans, and complete required documentation clearly, legibly, and on time. Previous experience working with individuals with IDD/mental illness is required, as well as experience managing teams. Competency in Microsoft Word is required. Experience working in a Day Support Program or Special Education environment is preferred. Salary: $78,242 annually, Full-Time Benefits/PTO/ Vacation Valid Driver's License Required Position Summary Work with the Divisional Director and the Associate Director of Residential Services to manage the daily operation of the program. Responsible for ensuring the program operates within the stated TSINY Policy and Procedures, as well as the applicable New York City, New York State and Federal regulations at all times. Responsible for monitoring the effectiveness of the program in assisting individuals to achieve independent housing and remain responsible in participating in program development to meet this goal. Remain responsible for managing their employees and the program’s resources to achieve full compliance with policies, procedures and regulations. Also responsible for maintaining the program’s alignment with the TSINY mission, vision and values. Serve as the program’s representative to other TSINY programs and the external community, building and maintaining positive relationships to support the functioning of the program. Job Description Title: Case Manager Location: TBD Pay Range: $24.37 - $26.37 per hour (effectively $45,000 - $48,000 per year) FLSA Classification: Full-time (35 hours), Non-exempt Role Summary: We are seeking a compassionate and dedicated Case Manager to join our team. The successful candidate will provide a comprehensive range of on-site, person-centered, and strengths-based services to a caseload of approximately 20-25 clients. Your primary objective will be to empower individuals to lead fulfilling lives within the community. As a Case Manager, you will play a pivotal role in client counseling, intakes, case management, and providing essential aftercare services. Collaborating with colleagues is key to meeting the diverse needs of your caseload and achieving program success while aligning with NAICA's core purpose, values, and vision. Primary Job Responsibilities/Duties: The Case Manager's role encompasses, but is not limited to: • Conducting initial intake assessments and promptly addressing critical needs., • Utilizing assessment tools, such as CARES, to identify support service requirements and make immediate referrals., • Orienting clients to the shelter program, its requirements, and potential consequences., • Developing Independent Living Plans (ILPs) in coordination with other Social Service providers, ensuring the delivery of comprehensive support services., • Assisting non-working clients in accessing Public Assistance., • Completing psycho-social assessments within the first 30 days of intake., • Monitoring and tracking client progress., • Empowering clients to actively participate in their own planning and goal setting., • Conducting regular meetings with clients (weekly/bi-weekly or as needed).Referring clients for job development opportunities if appropriate., • Connecting clients with suitable resources to help them achieve their goals., • Meeting with clients to address challenges, successes, and strengthen their capacity for long-term self-sufficiency., • Maintaining thorough client files, documenting conversations, warnings, and progress reports, while preserving confidentiality., • Reporting critical incidents immediately to the Site Director/Director of Social Services., • Collaborating closely with other Housing Specialists and Employment staff to identify client readiness for employment and housing., • Treating all clients, visitors, and employees with kindness, respect, and dignity., • Serving as an advocate for clients when necessary., • Providing periodic case management and counseling, extending one year beyond the client's departure., • Establishing and nurturing connections with other programs and units within the organization to facilitate inter-agency referrals., • Escorting clients to housing viewings, DHS or HRA appointments as required., • Facilitating suitable housing placements for clients., • Ensuring that case notes are completed within 24 hours after client meetings., • Offering support as a substitute for other staff when needed., • Performing general clerical duties. Physical Requirements: • Prolonged periods of sitting at a desk and working on a computer., • Ability to sit for extended periods and perform repetitive tasks., • Must be able to lift and carry up to 5 pounds., • Must be able to travel to multiple NYC sites as needed., • Must be able to access and navigate each department at the organization’s facilities., • Ability to climb stairs.
