Full-time: 2550 Main Street, Buffalo, NY JOB DESCRIPTION: To provide housing case management services and support to adult individuals who are homeless, or at risk of being homeless, with mental health, chronic health, substance use & co-occurring issues with the goal of obtaining and maintaining safe, affordable housing and residential stability in the community. Some participants may be being releasing from Correctional Facilities and/or from New York State Psychiatric Centers into the community. ESSENTIAL FUNCTIONS: • Knowledge and demonstration of agency core values in day-to-day activities, • Work with individuals, in locating safe and affordable housing in an area of their choice, • Promotes and supports individual movement through program phases, including assisting customer to develop goals, and target activities leading to sustainable housing and successful graduation., • Purchase individual household furnishings for participant residence, • Assist with obtaining benefits if needed, collect income, diagnostic and complete a rental stipend to determine voucher per month, • Maintain open communication with all treatment and service providers, families, landlords and any other supports or networks the individual is working within the community., • Assist in outreach and location of the individuals referred, • Responsible for recordkeeping (progress notes) in the Millennium Database and other required paperwork such as service planning, roster maintenance, rental stipends, income and homeless verifications, and any other duties asked by supervisor of supportive housing & meets documentation timeliness standards, as required by the agency., • Must have own transportation., • There will be an expectation to see individuals in person twice each month, preferably in their home(s). Visit frequency is dependent on client need above that expectation., • Attend team and community-based team meetings in the community, • Proficient in the use of computer software such as MSWord, Excel, Power Point, Desktop Publishing, Outlook, etc., • Associate Degree with 1 years’ qualifying experience • “Qualifying Experience” means-verifiable full or part time experience working with the following populations: mental health, substance use, chronic illness and/or co-occurring disorders Must possess a valid drivers license with a satisfactory driving record, and possess a personal vehicle for job requirement COMPENSATION: $19.00-$22.26

POSITION OVERVIEW Under supervision of the Clinical Supervisor the CASAC Counselor will provide counseling services in integrated outpatient program. The CASAC Counselor provides comprehensive assessment services to clients who may have substance use and mental health barriers to employment by applying clinical judgment to determine appropriate treatment needs and level of care. This includes performing SUD diagnostic screening, assessment, evaluation, orientation, treatment planning, case management, counseling, intervention, crisis intervention, patient education, reporting and record keeping in both individual and group counseling settings. The CASAC Counselor is also responsible for referrals and coordination of care in consultation with other professionals. KEY ESSENTIAL FUNCTIONS • Intervention and employing a range of treatment modalities to include person centered evidence-based treatment approaches to include but not limited to motivational interviewing, stages of change, trauma informed care, anger management, medication assisted treatment, dialectical behavioral therapy, cognitive behavioral therapy and contingency management., • Ability to perform SUD and mental health screenings and intakes, orientation, SUD assessment, SUD diagnostic Impression, level of care as determined by LOCADTR, treatment planning, individual/group counseling, family interventions, psychoeducation, case management, crisis intervention, client education, referral, reporting and recordkeeping, care coordination and administrative supervision with regular supervision., • Participates in supervision regularly., • Develops and conducts group and individual counseling on a regular scheduled basis to caseload clients within the integrated outpatient program., • Obtain random observed urine samples according to established protocol., • Orients clients about program goals, objectives, rules and regulations, client’s obligations, right’s and program operations, • Knowledge of the obligation to engage in prevention as well as treatment techniques., • Maintains and updates referral services and resources that may be useful to our client population; makes referral when necessary and appropriate., • Ability to demonstrate proficiency in English including the ability to speak, write, comprehend aurally, and read at a level necessary to perform as a CASAC Counselor., • Participates in outreach functions to reengage client lost to contact and maintain census capacity., • Ability to complete all client progress update reports in a professional and timely manner to ensure coordination of care with client linkages to include the criminal justice system and entities., • Participates in program meetings and required trainings., • High School Diploma or GED., • Associate Degree preferred., • Valid unrestricted OASAS certification as a CASAC., • Minimum of three (3) years working in a Substance Use and Human Service setting., • Must obtain Mandated reporter (2 hours) training/certificate prior to hire date., • Must obtain training/certification within thirty (30) days of hire., • Domestic Violence, • Infection Control BASIC (non-medical/nursing staff), • HIV, • Child Abuse, • Tobacco Cessation, • Must obtain training/certification during in-house orientation., • Supporting Recovery with Medications Addictions Treatment (MAT), • Must obtain SUD training/certificate within thirty (30) days of hire (if applicable), • Screening, Brief Intervention and Referral to Treatment, • CASAC Canon of Ethics (6 hours), • Confidentiality related to 42CFR (3 Hours), • Valid unrestricted OASAS certification as a CASAC., • Excellent organizational, communication and time management skills, • Ability to work on a strong team of professionals in a culturally diverse environment.
