Job Description JOB SUMMARY: Background: The Office of Mental Health (OMH) and the Department of Health (DOH) are co-defendants in the Stipulation and Order of Settlement, United States v State of New Your, Civil Action No. 13-0CV-4165 (NGG) O’Toole et al v. This settlement requires the state to assist residents of impacted adult homes, who have been identified as “class members” to move out of adult homes into supported apartments if they choose to and are eligible. They will have the assistance of a care manager to ensure any needed wrap around services are in place prior to the move. An assessment determines the care planning needs that the care manager implements as part of the preparation for transition out of the Adult Home. The Registered Nurse Assessor, in collaboration with the Social Work Assessor and assigned Care Coordinators, will be responsible for working with other team members to streamline the assessment and placement into supported housing, of identified residents of impacted Adult Homes. About the Role The Training and Data Coordinator plays a vital role in supporting the success of the Runaway and Homeless Youth (RHY) and Supervised Independent Living Programs (SILP) by developing and coordinating staff training programs, managing program data, and ensuring compliance with reporting DYCD, ACS and OCFS requirements. Job Description PRIMARY FUNCTION/PURPOSE: Provide direct case management services including assessment, counseling, crisis intervention, referral, and advocacy to ensure an effective social service support plan is in place. This role is responsible for working at our Union Hall location in Jamaica, Union Hall Street to enable the successful relocation of families to permanent housing.
