Using independent judgement, the Speech Pathologist assumes the responsibility of providing a comprehensive evaluation of communication skills and/or feeding skills, and provides goal-oriented treatment to facilitate maximum development of communication and/or feeding skills. This position does provide direct patient care children ages 3-21, including comprehensive assessment, treatment planning and provision of therapy in the school setting. This position serves students with various physical, behavior and developmental challenges. Age range from 3-21 years old. Age specific experience and/or special training and/or expertise is required to serve this population Implements a daily program through a consistent daily/weekly schedule that can be shared with the school and/or supervisors.Delivers specialized individual and group instruction.Consults with other staff members that work directly with each of the students in order to better serve the students and their educational/functional needs.Assesses and documents each student's progress through formal and informal tests and measurements.Writes and prepares annual review reports/goals for each student in accordance with their IEP dates in order to remain in compliance with state guidelines.Assists with any school-based speech and language input when writing an FBA/BIP.Provides parent support and education.Attends all school related meetings as scheduled, such as, but not limited to, Triennials, Annual Reviews, Domain, Referrals and pre-IEP meetings for each individual student.Plans and prepares the learning environment by setting up and preparing needs materials and supplies.Follows the guidelines and needs of each of the individual students in the classroom.Maintains professional attitudes and respect to the school.Communicates and follows up with school and families in order to request appropriate MD signatures/orders for school-based occupational therapy referrals in order to initiate and continue with services.Treats all students with dignity, compassion, empathy and respect.Maintains confidentiality about students, their families, and other employees outside the school.Supervises COTA as indicated through state practice act guidelines.Assists the students in activities of daily living such as toileting, feeding, personal hygiene, dressing and diapering as needed.Follows the safety guidelines and directives for the student in regards to functional mobility and orientation (i.e. transfers, wheelchair or walker mobility, etc.) and exercises provided by other related service providers (i.e. physical therapist, vision itinerant, hearing itinerant, occupational therapist, orientation and mobility instructor, etc.).Assists with any devices and assistive technology as outlined by the other related service team members.Collects data and completes reports regarding the student goals and progress.Follows any fire drill or safety guidelines outlined for each individual student.Completes of clear therapy lesson plans in a timely manner in case of any emergencies or substitutes that may have to follow the treatment schedule.Provides screenings and evaluations to determine requirements for skilled school-based speech language pathology intervention relative to client's level of function. This includes, but is not limited to the following areas: providing a comprehensive evaluation of communication skills and/or feeding skills, and provides goal-oriented treatment to facilitate maximum development of communication and/or feeding skills assessments, discharge planning, development of maintenance programs and other related client care issues.Develops and implements treatment plans that focus on functional and measurable outcomes and adheres to professional standards of practice. Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming a CORE Peer Specialist. Community Oriented Recovery and Empowerment (CORE) services is a program serving individuals living with severe and persistent mental illness. This program serves Medicaid HARP enrollees and works to assist individuals in reaching their recovery goals. Services provided include Psychosocial Rehabilitation, which incorporates education and employment goals, and Peer Support. There is an extensive and broad range of services that include but are not limited to the improvement of daily living skills, managing finances, obtaining or improving safe and affordable housing, furthering one’s education, obtaining employment, achieving personal recovery and/or resilience, improving family and interpersonal relationships, preventing social isolation, connecting with effective and comprehensive behavioral health and primary care, and full community integration. This position is primarily clinical and consultative in nature and provides services to all supported housing programs (both single- and scatter site) throughout the cluster. While the Clinical Specialist does not carry formal supervisory or operational responsibilities, the VP may deputize him/her to delegate and monitor certain tasks and responsibilities to staff at the Program Director level and below in relevant Supported Housing (SH) programs. ESSENTIAL JOB FUNCTIONS: • Assists Program Directors and case management staff to provide quality clinical and crisis management services across the supported housing system, primarily through consultation and training., • Provides consultation to staff on challenging clinical cases, such as those involving high-risk consumers, (i.e., those with co-morbid medical conditions, those identified as at risk psychiatrically) or individuals who present compliance issues or added complexity due to factors such as AOT/forensic status. Job Description JOB SUMMARY: Under the general supervision of the Program Director/designee, functions as part of a team assigned to support consumers living in independent apartments in the community. Assists assigned individuals in developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psychosocial, educational, financial, vocational and other services. Participates in the provision of crisis intervention services to participants. Travels to/visits consumers' residences or apartments to provide counsel and assistance and to help the consumers, within their capabilities and interests, in maintaining the greatest degree of independent living.
