Clinician will also participate in CSE meetings and maintain contact with SCCC CSE chairperson., • Acts as liaison with child care staff and case management staff through participation on the treatment team, and at treatment reviews, service plan reviews, medication reviews, and staff meetings., • Documents treatment processes as required by SCCC and by its regulating agencies including:, • Completion of thorough and up-to-date progress notes in both the AWARDS and NYS Connections systems, • Preparation of clinical/diagnostic assessments, court reports, correspondence with other agencies, psychosocial summaries;, • Completion of quarterly Treatment Reviews and facilitation of treatment meetings at houses;, • Preparation or collection of all materials necessary to bring records into compliance with internal and external agency regulations, e.g., school information, requests for clinical involvement, etc., • Summary reports requested by referring and other collaborative agencies., • In conjunction with the House Management Team (Child Care Supervisor, Case Manager and Nurse), develops and maintains a therapeutic environment at the site., • As a member of the clinical staff, provides 24-hour on-call crisis services (this includes but is not limited to assessment and counseling) on a rotational basis., • Attends all court hearings pertaining to the placement, permanency, and treatment of the child and family., • Meets weekly with the Supervisor of 29i Clinical Services to review progress toward goals, and design and implement specific treatment strategy and goals for children and families., • As the primary clinician for child/ family, flexes hours at least two days a week to include evening hours outside of the traditional work day., • Functions as a member of the Direct Care Planning Team, meeting with HMT members, unit child care supervisors, consultants, nurses and case managers to evaluate and plan for the therapeutic milieu., • Additional position-related duties and responsibilities as assigned by the supervisor. Essential Duties & Responsibilities include: • Accept all referrals from Albany County Department for Children, Youth and Families Prevention intake department for the prevention program; Attend intakes within 24 hours of receiving referral., • Attend home visits with the families in order to assess the needs of the children for essential community linkages., • Make the linkages for youth to the community activities that align with their goals and interests., • Ensure that all children on caseload have a primary care doctor, dentist; school aged children are enrolled in, and attending school or an educational alternative., • Coordinate services by attending service plan reviews and other meetings, and via phone contacts., • Coordinate with the families and the Primary Clinician/Case Planners around the child(ren)’s needs and progress., • Transporting families or children to and from appointments (e.g., counseling sessions, Parent Skills Training, supervised visits)., • Administer the NYS Child Adolescent Needs and Strengths Assessment to help identify targeted areas of need., • Complete progress notes and contributions to the Family Assessment and Service Plans within the NYS OCFS Connections system, in a timely and efficient manner., • Complete additional paperwork as needed for the program., • Advocate for the program and our clients with other agencies in the community., • Work as a team with the Primary Clinician/Case Planner/Family Advocate, and the Program Supervisor, who will team lead for all prevention cases. LifeVest® is worn by patients at risk for sudden cardiac arrest (SCA), providing protection during their changing condition. It is lightweight and easy to wear, allowing patients to return to their common activities of daily living, while having the peace of mind that they are protected from SCA. This is the perfect opportunity for the health professional with patient care and teaching experience to supplement his or her income. A typical LifeVest® patient is in the hospital awaiting discharge, and the Patient Service Representative sets up the equipment and trains the patient and caregivers on the use and care of the device. They also follow up with patients as needed to assure patient understanding and satisfaction. PSRs retain the flexibility to accept or decline assignments as their schedules dictate. Most assignments can be scheduled during free time – before/after work, and on days off.
