Job Description The Association for Mental Health and Wellness is seeking a passionate Fee for Service Children’s Care Manager (CCM) to make a difference in lives of children and their families. This position helps families of children living with complex behavioral and/or mental health conditions to navigate complex systems of care; building supportive relationships and developing individualized care plans and will also be responsible for completing the enrollment process, developing a care plan and maintaining detailed accurate case notes, reports and documentation. The fee for service CCM total compensation per service unit includes travel time, required administrative tasks, and the anticipated time to complete the client services. Service delivery is community based throughout Suffolk County. Service Unit Compensation: $40.00 per service unit, or $42.00 per service unit for Bilingual Care Managers Training Compensation: $30.00 per hour for all required training Key responsibilities will include, but are not limited to: • Meeting with child and family for initial consent and enrollment process, confirming eligibility etc., • Contact providers to obtain necessary eligibility documentation, • Maintaining detailed, accurate and timely case notes and documentation in the EHR, • Carrying a mixed caseload of clients in outreach and enrolled cases., • Providing comprehensive Care Management, assessments, linkages and referrals. Children’s Care Managers assist in navigating the complex healthcare/behavioral care system, Department of Social Services, community resources, housing systems and other systems of care. Children’s Care Managers assess socioeconomic and environmental factors and they understand the social determinants of health. Children’s Care Managers are responsible for providing comprehensive and coordinated care as well as linkages and referrals to an array of services. They also serve as advocates, ensuring that their clients receive appropriate care and the attention they require. Children’s Care Managers provide emotional support and encouragement to those served. They implement a strengths-based, trauma-informed approach; offering coordinated care upon assessing their clients’ needs and collaborating in the development of a person-centered care plan. Care Managers reevaluate clients’ eligibility, ongoing needs and appropriateness for services; they support their clients in achieving the established goals until they are ready for graduation from the program. Job Description The VP of Clinical Quality and Excellence will be responsible for the oversight of all Association for Mental Health and Wellness (MHAW) clinically oriented services, residing under current and future programs. This is a high-level management position that requires extensive clinical experience, experience leading quality improvement a belief in and commitment to Person Centered Psychiatric Rehabilitation and Recovery initiatives, which is incorporated into clinical service delivery. The position will require an affinity for strategic thinking and problem solving in the course of day-to-day clinical operations. Responsibilities typical of the position include strategy focused on: best practices, clinical outcomes, quality of services, management of clinical personnel, and, census and revenue growth, regulatory compliance. The VP of Clinical Quality and Excellence reports directly to the CEO and works in collaboration with Programmatic Supervisors and Directors, along with clinicians, Psychiatrist/Nurse Practitioners, and fiscal/billing staff to ensure that all resources in the programs are being used to best serve clients’ needs. The VP of Clinical Quality and Excellence individual will lead and manage a comprehensive continuous quality improvement plan to ensure the successful implementation of all agency programs.
