Job Description Job Description ESSENTIAL DUTIES AND RESPONSIBILITIES Home Health Aide (HHA) will provide quality personal care and designated health related services to the client in the client’s home as specified by the plan of care and in compliance with standard precautions and according to the established policies, procedures and objectives of the organization. Direction from the Case Coordinator or Supervising Nurse must be sought when or if asked to share any client’s information with outside parties. * Assist clients with personal care activities in accordance with the Plan of Care including: * Feeding – Serve meals and assist in feeding. * Ambulation and Transfer – Assist the client in and out of bed to chair or wheelchair; help with going outdoors and doctor’s appointments; other assistance with ambulation; provide safety supervision to prevent falls. * Bathing – Assist client with bathing and washing in bed, tubs or shower. * Toileting – Assist client with use of toilet, commode, bedpan or urinal. * Dressing – Assist client with clothing. * Personal Grooming – Assist client with care of hair, shaving, and ordinary care of nails (nail cutting is not permitted) and brushing teeth. * Summoning Emergency Assistance – In an emergency situation call 911 immediately or as soon as possible. * Other duties may be assigned as needed in accordance to the Plan of Care. Reporting: The following must be reported to Case Coordination immediately or as soon as possible: * Any change in a client’s health status or mental condition. * Any client hospitalization or other absence from their home that prevents service from being provided there. * Inability to gain entry to a client’s home. * All client falls or accidents whether or not the client is injured (when the office is closed, notify our “On-Call Emergency Service” and inform the Case Coordinator the next business day). * Absence or lateness from work (when the office is closed, notify our “On-Call Emergency Service”). * HHA should notify us of all the above occurrences even if not present when they occur. * HHA’s are required to notify Case Coordinator as well as the client’s family members or contacts. * When arriving to or from an assignment, HHA must check on the client to best insure they are not in distressed or in a condition that does not appear to be normal for them. It is not acceptable for a client or their family to tell a HHA not to disturb a client when arriving or leaving. If this presents a problem, HHA must a call their Case Coordinator so they can review this responsibility with them. * Under all circumstances, HHA must call 911 without hesitation if a client is in distress or in need of immediate help. Meal Planning and Preparation: * Perform meal planning and preparation as per the specific needs of the client. Marketing and Errands: * Perform marketing and errands as per the specific needs of the client. General Housekeeping: * Participate in case conferences with the health care team as appropriate when directed by RBHC. * maintain the home’s clean and orderly appearance for the client’s health and comfort. * Kitchen: Sweep and wash floors, wax if time permits; defrost and clean refrigerator, maintain proper storage cabinet and arrange contents for accessibility. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. In all programs, we provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. About the Role The Case Manager work directly on the reunification process of the child along other stakeholders as per ORR Policy and Regulations. Job Summary: The Case Planning Supervisor oversees the day-to-day case management activities for the assigned NSP and LSP programs, including Aftercare. Job Summary: The goals of the Preventive case planner, within the Family Connections model, are to keep families together, while continually assessing for risk and safety issues. Through the Family Connections model, they are to help strengthen families, stabilize and maintain family functioning. Under the direction of the social work supervisor, the case planner shall serve as the primary service provider. The model requires great time flexibility, as we meet with the entire family system most of the time. Therefore, workers must make themselves available when the families are. Job Responsibilities: The Case Planner will have a caseload of 12 families. Each family will be seen for one hour of change focused intervention, primarily in the home, each week. Therefore 2-4 contacts are expected per month. Based on the Family Connections model, once a case is assigned, face-to-face contact must be made with the family to present the model and explain the expectations with 24 hours. Concurrently the JHV is set up with ACS. Once that is completed the intake is conducted, within a short timeframe, and all documents must be signed. The intake referral form must be completed and placed in CONNX within 7 days. Case Planners are to carry out ongoing risk and safety assessments of the family and clearly document this within their progress notes. Any concerns must be brought to the immediate attention of their supervisor. Case Planners must complete all Family Connections paperwork, within the required timeframes. These include the CASI instrument; Comprehensive Family Assessments; Service Plans; Evaluations for Change; and Case Closure Form. In addition, all FASP’s must be completed and submitted to their supervisor for review two weeks prior to submission to ACS. Progress notes must be written within 72 hours of the contact and inputted into CONNX. These notes must follow the template that was developed, which encompasses Family Connections, as well as ACS guidelines. Notes are to be printed out and submitted to their supervisors on a weekly basis for review. All casework contacts must be entered into PROMIS on a weekly basis. To facilitate family team conferences for your caseload and enter into PROMIS by end of week. Family team conferences will be held at the beginning of a case, or at the point of case closure. Weekly contact grids and calendars are to be submitted to supervisor at the end of each week. To participate weekly in supervision; and bi-weekly in group supervision. To serve on committees as assigned and perform other job-related tasks and duties as assigned. Qualifications BSW degree plus experience with families and children in a human service agency setting. Proven clinical skills pertaining to children and families are preferred. Good writing skills are necessary. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. About the Role The Case Manager Supervisor oversees the case managers to ensure the timely and safe reunification of the unaccompanied children in the program. Through this dedication, the TRC aims to transform services for survivors of violent crime. Rising Ground’s TRC is designed to help victims of violent crimes overcome barriers to accessing mental health treatment, health care, and legal resources in the acute aftermath of trauma. The TRC supports recent victims of violent crime, including survivors of physical assault, sexual assault, gunshot wounds, stabbings, domestic violence and human trafficking. The Trauma Recovery Center facilitates healing for survivors of trauma, violence, and loss through an innovative model that blends comprehensive care, advocacy and outreach. The Clinical Social Worker/Therapist of the Trauma Recovery Center (TRC) will provide clinical intakes, assessments, planning and implementing individual and group psychotherapy, assertive outreach, and clinical case management services, using evidence-based practices that are effective for the service population. The ideal candidate will bring a passion for social justice and anti-oppressive practice, ample experience providing clinical services to survivors of trauma, and a desire to work collaboratively with a team of creative and committed advocates. Maintain timely and ongoing communication with ORR DHUC for complex cases and clinical recommendations. · Participate in treatment team meetings and other meetings pertaining to the needs of the children in care. · Complete internal audits to ensure that the clinical records meet the requirements of standards set by OCFS and ORR. · Assist in facilitating training and shadowing for new clinicians. · Maintain confidentiality of sensitive and protected information according to HIPAA standards. · Integrate a collaborative approach with all different departments in the program to ensure clear communication, quality service provision and teamwork to meet the needs of the children in care. · Demonstrate commitment to departmental and organizational goals. · During times of influx, the lead clinician could be temporarily assigned a small caseload if needed to support the program. · Assumption of other duties as assigned. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. In all programs, we provide therapeutic, medical and educational services, as well as other supports that youth need to achieve success through an array of strength-based and targeted interventions as we link them with family members and/or other sponsors in the United States. About the Role The Case Manager work directly on the reunification process of the child along other stakeholders as per ORR Policy and Regulations. The Case Manager ensure all the child needs (medical, legal, educational, and emotional,) are meet while in care. The caseload assigned as per ORR regulations is up to 8 cases per case manager. What You’ll Do · Case management responsibilities include all necessary tasks as required by the Federal Office of Refugee Resettlement. · Works closely with Unification Specialist (TPG) to ensure timely and safe reunification process for all minors in care abiding by ORR Field Guidance 24. · Develops and maintains contact with youth and their service providers for medical, social, educational, and other related service needs. · Follow up with legal service providers regarding the timely submission of any documentation or correspondence needed for the legal relief process. · Maintains professional and timely communication with stakeholders including , GDIT, Legal Service Provider, The Young Center, ICE regarding the child case. · Performs discharge and release functions to ensure that each youth has a safe, appropriate and prompt release plan. · Documents weekly client contacts and maintain case files up to date and audit ready in accordance with regulations.
