PURPOSE: The Jewish Board's Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff uses a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities. Our children's community residences are single site residence's that provide a group living for eight children and/or adolescents. The program provides a supervised, trauma informed therapeutic environment which seeks to develop the resident's skills and capacity to live in the community and attend school/work as appropriate. POSITION OVERVIEW: The Case Manager works with children and adolescents in the community residence and is responsible for the supervision, care and skill building related to the youth's individual service and treatment plans. The Case Manager serves as a positive role model while providing therapeutic support, trauma informed care and supervision. All responsibilities are to be carried out in accordance with the standards of regulatory bodies and of the Jewish Board of Family & Children's Services. KEY ESSENTIAL FUNCTIONS: • In collaboration with the director, interview and assess youth and their families in the matching process that are referred to the community residence., • Will perform the necessary assessments and documentation associated with the intake process to complete the admission of youth to the program., • Assist in orienting families/resources of the youth to the services provided by and in collaboration with the program., • Engage with family and other support systems on a weekly and as needed basis., • Develop culturally affirming, trauma informed, individualized service plan for each youth in the program in collaboration with them, their families/resources, school, outpatient mental health providers and other community providers., • Complete assessments to identify client strengths, behavioral health needs, recommend services and interventions to meet the client needs., • Provide the clinical coordination of services and after care as required., • Provide crisis intervention., • Work collaboratively as a member of an interdisciplinary therapeutic team., • Liaise with families, schools, healthcare providers, hospitals, criminal justice, and other community agencies to coordinate care, • Perform these services in the community residence, family/resource's home, community and via telehealth (when applicable)., • Participate in clinical treatment team meetings, staff meetings, trainings, and supervision., • Maintain all documentation according to the standards and time frames established by the Jewish Board, regulatory agencies and/or funding sources, • Maintain professional behaviors and ethical standards as established by licensing board, relevant professional association and the Jewish Board policies and procedures., • The ability to travel and navigate the community in order to provide services where the youth and family will benefit most from the services delivered, • Responsible for the day-to-day care of the youth in the program including but not limited to assisting youth maintain their personal hygiene, social guidance, use of leisure time, food planning and preparation., • Develop, supervise and implement therapeutic recreational activities., • Provide feedback to the supervisor and treatment team, observations of the youth and response to interventions must be provided verbally and through documentation to the supervisor and members of the treatment team. ***Unusual and crisis oriented behaviors must be reported live to a supervisor immediately., • Assist in maintaining a safe and clean environment., • Work collaboratively as a member of an interdisciplinary therapeutic team by implementing culturally affirming and trauma informed interventions and skill building as outlined in treatment and service planning., • Liaise with families, schools, healthcare providers, hospitals, criminal justice, and other community agencies in order to provide appropriate care and supervision of youth., • Perform these services in the community residence, family/resource's home, community and via telehealth (when applicable)., • Maintain professional behaviors and ethical standards as established by licensing board, relevant professional association and the Jewish Board policies and procedures., • The ability to travel and navigate the community in order to provide services where the youth and family will benefit most from the services delivered, • Additional duties as assigned. ADDITIONAL FUNCTIONS MAY INCLUDE: • Must be willing to work evening and weekend shifts as needed as of the enhanced model to provide additional support and supervision during peak hours., • Must attend, participate and successfully complete the Therapeutic Crisis Intervention (TCI) certification and recertifications., • Must attend and participate in the ongoing training and consultation of the evidenced based model utilized within the program., • Provide supervision of youth and oversight of the program. Care Coordinators help members figure out and take the actions needed to get and stay healthy-making it to appointments, sticking to a medication schedule, and access benefits. POSITION OVERVIEW: Care Coordinators link adults and children with chronic behavioral health and medical conditions to the services they need to stay as healthy as possible and inspire the people they serve (members) to use those services to optimize their health outcomes. Working in a team setting and primarily in the field, Care Coordinators assess risk and needs, develop person centered care plans, provide care management services, track and arrange appointments, educate members and coordinate other aspects of members' health and community services. As this is an evolving program, additional responsibilities will be added. RESPONSIBILITIES include but are not limited to: • Integration of medical, specialized, and behavioral health services in addition to social support and/or educational support services, • Periodic assessment of a member's medical and behavioral health needs as well as compliance with recommended treatments, • Collaborative development of an Individualized Care Plan (ICP) with the member, the member's family and/or caregivers in addition to other service providers, • Providing required care management services, • Tracking all specialty medical, behavioral and support service referrals made for patient using Health Information Technology (HIT) provided., • Assuring that member has access to, engages in and retains needed services as defined in the member's ICP. The Center for Family Representation Inc. The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. The Medspan program provides patient education and care coordination around HIV, hepatitis C, and associated conditions such as hepatitis B and sexually transmitted infections (STIs). Medspan team members support patients through medication adherence support, discharge planning, and connection to care services. The Medical Director works across the Correctional Health Service Medicine service, with MedSpan staff and with other interdisciplinary Correctional Health Services (CHS) staff in all facilities to provide, improve, and evaluate patient centered infectious disease care. Responsibilities include: (Detailed Task) Clinical Responsibilities: • Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools., • Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes., • Deliver clinical consultation and guidance related to infectious diseases to site medical directors (SMD), infection control teams, and clinical staff. Administrative Responsibilities: • Offer direct administrative and clinical supervision for the infectious disease service, encompassing ID clinicians and the Medspan team., • Oversee staffing requirements for the infectious disease service within the facility., • Direct the formulation and execution of workflows and protocols pertinent to the infectious disease service., • Organize and lead regularly scheduled rounds and meetings for the ID team., • Facilitate conferences and seminars with other clinical services and both CHS-based and external re-entry programs., • Provide supervisory clinical guidance to the SMD and staff of the communicable diseases unit (CDU)., • Oversee admission and discharge criteria for patients with confirmed or suspected communicable diseases within the CDU., • Lead patient tracking initiatives within the CDU, monitoring clinical conditions, medication regimens, and laboratory results., • Collaborate with and provide clinical consultation to the infection control team., • Engage in initiatives focused on system-wide education, quality improvement, and population health., • Direct projects, analytical efforts, patient tracking, and quality improvement initiatives for the infectious disease service., • Provide supervisory direction and guidance for the Medspan Program., • Oversee the creation and implementation of staffing, workflows, and protocols for the Medspan team., • Direct regularly scheduled rounds and meetings for the Medspan team., • Lead research initiatives, patient tracking, and quality improvement efforts for the Medspan team. Minimum Qualifications Applicants must have all of the following: 1. Graduation from a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association. 2. Completion of an acceptable internship in an approved hospital., 3. A license to practice medicine in the State of New York., 4. Certification by American Board in field of specialization or a faculty appointment of Associate Professor or higher in a medical school recognized by the American Medical Association., 5. Eight (8) years of satisfactory full-time paid experience in field of specialization including at least three (3) years in a supervisory or administrative capacity.(Training and experience required for certification will be credited toward required specialized experience.) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient’s clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management. The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. Under general supervision of the Medical Director, the Senior Psychiatrist is the psychiatric leader of the facility or program and works in close collaboration with other discipline leadership to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong clinical support and oversight, leadership and mentorship for existing staff. Responsibilities • Develops workflows, policies and quality metrics for their dedicated area in close coordination with the Medical Director and Chief of Mental Health, • Recruits, hires and trains staff to provide care in their dedicated area, • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with other leadership: design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both in vivo reviews of clinical skills as well as chart review., • Close collaboration, including regularly scheduled meetings, with other relevant disciplines within the mental health, medical, nursing and operations departments, as well as the Department of Correction, • Ensure that medication renewals are completed in a timely manner, • Ensure that the standard of care is being met or exceeded in all patient care, • Educate clinical staff about new developments regarding best practices in all aspects of psychiatric care, with specific attention to the challenges and issues facing a jail setting (e.g., influence of legal case, dual loyalty, medication diversion), • Ensure compliance with all regulatory standards, • In collaboration with other leadership, oversee the orientation of new staff to the facility or program, • Conduct regular multi-disciplinary case conferences and/or other educational programming, • Report on an as-needed basis to the CHS Morbidity and Mortality review committee, • Work in collaboration with other mental health leadership to manage clinical schedules, vacation requests, CME activities and other relevant activities as they pertain to psychiatric staffing and education, • Participate in policy revision and creation, and practice guideline creations, related to psychiatric care, including input into the psychiatric medication formulary, • Lead interdisciplinary mental health staff meetings in coordination with local MH leadership., • Personnel management of clinical staff, including setting quality standards, establishing effective recruitment and retention strategies, and managing workforce complaints, • Administrative tasks, including coordinating scheduling and assignment of patient care appointments, scheduling of staff, and management of referrals to mental health service, • Direct clinical care, amount determined by facility needs, which may include direct supervision of psychiatric trainees., • On-call responsibilities for 2 weeks at a time, one to two times/year, for off-hours issues that require high level clinical intervention, • Other duties at the discretion of the Medical Director of Mental Health or the Chief of Service., • Personnel management of the clinical staff, with assistance from the Clinical and/or Medical Directors as needed, including recruiting, mentorship, supervision, and disciplinary procedures, • Meet regularly with assigned supervisor to provide meaningful input into program design and policy formation Minimum Qualifications Applicants must have all of the following: 1. Graduation from a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association. 2. Completion of an acceptable internship in an approved hospital., 3.
