Job Description Founded in 1974, Community Access continues to uphold and operate as a pioneer of supportive housing and social services in NYC for people with mental health concerns. Position Overview The Maintenance Worker is responsible for daily cleaning and maintenance of the building to ensure that a safe and adequate living environment is provided to tenants, staff and visitors at all times. Position Overview The Peer Trainer will develop and offer a series of workshops designed to help peer specialists learn about different specialties in peer work and practice the skills that different roles would require. This would help better prepare those who are interested in working with people who are experiencing mental health crises in a variety of settings such as: mobile crisis response teams; in hospitals; in crisis respite residences; street outreach; or on Intensive Mobile Treatment or Assertive Community Treatment Teams (IMT/ACT), and Intensive and Sustained Engagement Team (INSET) which often work with people with higher needs who may frequently be in crisis. This role involves conducting assessments for medical patients seeking behavioral health services, providing crisis intervention, and referring patients within Callen-Lorde or externally as appropriate. Program/Department Description Intensive Mobile Treatment (IMT) is an innovative, creative, trauma-informed, harm reduction oriented mobile practice model designed to provide treatment, rehabilitation and support services to individuals whose needs have not been met by traditional services. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. Community Access’ IMT Teams serve individuals who are street homeless or living between institutional settings and housing. Team members work collaboratively with settings in which the participant is living and use assertive engagement strategies to proactively engage individuals in services. IMT team members share responsibility for the people they serve, helping individuals attain housing and other personal and safety goals that may be of a health, social, work or education nature. The Vocational Specialist will be responsible for providing ongoing supportive counseling and service coordination for homeless adults living in Bronx mental health shelters, as well as individuals who have recently transitioned to supportive or independent housing. The Vocational Specialist is also responsible for integrating person-centered goals and services with the tasks of all ACT team members and for providing psycho-education individually and in groups. The Vocational Specialist works as part of a multi-disciplinary team providing the range of ACT services to assist participants in achieving their treatment, rehabilitation and recovery goals. They specialize in assisting people with honing vocational goals and finding schools, training, jobs or employment programs that will help them reach them. This includes policy and procedure development and implementation, staff recruitment, scheduling, training, supervision and evaluation, management of physical environment, quality assurance and quality improvement activities, internal and external reporting. The Program Director is responsible for meeting all contractual and licensing standards and obligations. The Program Director is on-call 24/7 for emergencies.
