Job Description JOB SUMMARY Background: The Office of Mental Health (OMH) and the Department of Health (DOH) are co-defendants in the Stipulation and Order of Settlement, United States v State of New Your, Civil Action No. 13-0CV-4165 (NGG) O’Toole et al. v. Cuomo et al., Civil Action No. 13-CV-4166 (NGG). This settlement requires the state to assist residents of impacted adult homes identified as “class members” to move out of adult homes into supported apartments if they choose to and are eligible. They will have the assistance of a care manager to ensure any needed wrap-around services are in place after the move. An assessment determines the care planning that the care manager must implement as part of the preparation for transition out of the Adult Home. In collaboration with the Nursing Assessor and assigned Care Coordinators, the Social Work Assessor will work with other team members to streamline the assessment and placement into supported housing for identified residents of impacted Adult Homes. They will be responsible for making recommendations about the type of housing and the types of services—physical and behavioral health—as well as non-medical services like Meals on Wheels, etc. The Social Work Assessor will complete assessments for Queens Adult Home and SIBN Adult Home Supported Housing Programs. The Social Work Assessor will complete the housing application's psychiatric evaluation and psychosocial portion. The Nurse Assessor will have primary responsibility for assessing the residents' physical health needs, such as Managed Long-term Care services and certified Home Health Agency services. Each assessor will interface with the respective behavioral or physical health providers of the residents and review records necessary to make determinations about the types of services that will be needed to promote successful placement in supported apartments. ESSENTIAL JOB FUNCTIONS • Accompanies in-reach supervisor during in-reach to schedule assessments on-site at in-reach (3 days per week)., • If the assessor is not on-site for in-reach, the in-reach supervisor will email the consent form to the assessor on the same day as the in-reach meeting., • Upon receipt of consent, the assessor will obtain the psychiatric evaluation from a mental health provider., • Assessments to be scheduled promptly within the 30-day window to reduce delays from in-reach to assessment., • Assessor to be on-site at adult home at minimum four days per week to conduct assessments., • Upon completion of the assessment, the assessor will complete the assessment packet and send any required documentation to identified recipients (OMH, DOH, Housing, Care Coordination)., • Assessor to submit HRA application to HRA., • Once HRA approval is received, the assessor will send a packet to the housing program manager and health home., • For backlog assessments, the assessor will obtain the psychiatric evaluation from a mental health provider., • Assessor to approach class members with in-reach supervisor for introduction and to schedule an assessment., • Once the assessment is completed, the assessor must complete the assessment packet and submit the HRA application., • Once HRA approval is received, the assessor will send a packet to the housing program manager and health homes, • Responsible for making recommendations about the type of housing and the types of services (physical and behavioral health and non-medical services like Meals on Wheels, etc.), • Proposed Adult Home Housing Contract Amendment Language, • Provide clinical consultation to the team, conduct trainings, conduct clinical review, and chart review NYS license and current registration as a Licensed Clinical Social Worker. Job Description:\n\nJOB SUMMARY: The Director of Wellness uses their specialized knowledge to insure the Resident’s care and management needs are being adequately and correctly met and assists the Executive Director, and the Director of Case Management as an additional liaison between Residents, the Residents Legal Representatives, their families, administrative staff, and community-based service providers. Responsibilities include the direct supervision of the Wellness and Reflections Unit; motivation, supervision and in-service training/education of the Resident Service Aides. The Director of Wellness will assure compliance with company policy and the State Health Department’s regulatory requirements for the admission and discharge procedure as well as appropriate documentation requirements. Additional responsibilities include: in-service education on health/wellness related topics and on the spot training to address identified performance concerns and/or improvements. As a member of a multi-disciplinary team the Director of Wellness facilitates development of the Resident’s individual plan of care under the direct supervision through collaboration with the Resident, the Resident’s family or Representative, the Resident’s primary care physician, the property Management Team, and any appropriate third-party providers. Assures the accuracy of Resident records and documentation in adherence with company policies as well as regulations of the State Department of Health f. Assures the medication management system adheres to State and local Department of Health laws and regulations and company policies, as well as Memory Care requirements: i. Assures thorough reviews of Medication Administration Records to include “chart to cart” audits, review of pre-MARs and final MARs. ii. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs., • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care., • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner., • Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients., • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication., • Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed., • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics., • Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist 1. Graduation from an approved medical school., 2. Completion of approved residency or fellowship in the specialty or sub-specialty and Board eligible or certified or Subboard eligible or certified., 3. Five years experience in field of specialty or subspecialty acceptable to the Medical Board of the Hospital., 4. Licensed to practice medicine in the State of New York.
