Physical Therapist, PT Responsibilities: • Assessment and Evaluation: Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait., • Individualized Care Plans: Develop individualized treatment plans based on assessments, with clear goals for improving residents' mobility and reducing pain., • Section GG Data Collection: Accurately and consistently collect Section GG data as required for Annual, Quarterly, Falls, and Significant Change assessments., • Treatment: Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function., • Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care., • Documentation: Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC)., • State licensure as a Physical Therapist., • EMR and MS office applications., • CPR/BLS certification., • Knowledge of Section GG data collection and documentation requirements., • Strong clinical and interpersonal skills., • Excellent written and verbal communication skills., • Ability to supervise assistants, technicians/aids and student/interns.
