Homeless (often mentally-ill and substance-abusing) men and women are provided with comprehensive services to stabilize their condition in order to successfully transition into to permanent and/or supported housing. CAMBA’s Flagstone Family Center provides comprehensive case management services to 160 families who are in a Tier II transitional residence program for homeless families. The facility consists of eight (8) four (4) story walk up apartment buildings on the east and west sides of the street. There will be two main entrances: one for the east side of the street and one for the west. Each building contains 20 apartments of varying sizes, each fully self-contained. The FEF co-leads the Family and Community Engagement (FACE) Center project for Staten Island and works with the team's other FEFs and Resource and Referral Specialists (RRSs). The FEF collaborates with external partners to develop and deliver technical assistance, training, and coaching to various stakeholders in the special education and disability community, with a focus on enhancing family engagement capacity to improve educational outcomes for children with disabilities. Stakeholders can include families, school administrators and educators, community groups, and professionals working with children with disabilities. Children’s age groups may span early childhood and school-age ranges, but the position will primarily focus on school-age. For bilingual FEF roles, it is expected that approximately 20%-25% of family engagement outreach activities will be in a language other than American English. Rapidly after release into the Supervised Release Program (SRP), the Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions required under the supervision plan. In this role, you will help participants comply with their supervision appointments and court appearances during their legal case's pendency. You will also conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals. Job Description Job Title: Clinical Case Coordinator Job Summary: CASES is currently seeking a highly organized and detail-oriented Clinical Case Coordinator for our Pretrial Services Case Management Team. Rapidly after release into the program, the Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan. The Clinical Case Coordinator assists participants to comply with supervision appointments and court appearances during the pendency of their legal case. In this role, you will conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals. You will ensure participants receive services that are strengths-based, gender-specific, and culturally sensitive while adhering to the core principles of risk, need and responsivity to reduce recidivism. The goal is to increase engagement in community treatment services and support participants to achieve their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration. This position is based in our Bronx Community Office and fieldwork in the larger NYC community as needed to assist participants.
