About the Clinic: Rincon Physical Medicine and Rehabilitation is a medical clinic specializing in diagnosing and treating traumatic brain injuries (TBI’s) and musculoskeletal injuries. Our comprehensive team of practitioners work closely with each and every patient to create and implement a detailed, humanistic treatment plan that takes into account all aspects of a patient’s condition, both seen and unseen injuries, to reach one goal – improving that patient’s quality of life by helping them achieve functional independence. About the Role: The Nurse Practitioner (NP) is a licensed provider who practices independently and collaboratively with physicians and the care team to assess, diagnose, treat, and manage patients with TBIs, musculoskeletal conditions, and other complex medical needs. The NP develops individualized care plans, educates patients and families, monitors outcomes, and ensures continuity of care while maintaining patient safety and quality standards. New graduates are encouraged to apply; no specialty experience required. Responsibilities: Patient Assessment & Care • Conduct comprehensive health histories and physical examinations., • Order, interpret, and act on diagnostic tests and imaging., • Assess patient status, identify health risks, and formulate evidence-based diagnoses., • Prescribe medications, injections, and therapies within scope of practice., • Educate patients and families on treatment, health promotion, and prevention., • Monitor cognitive and behavioral responses to treatments., • Coordinate supportive care and community resources., • Provide telephone triage and follow-up between visits., • Maintain an 80% utilization rate (~12 patients per day). Procedures & Clinical Interventions • Perform procedures such as Obturator Nerve Blocks (ONBs), Botox, Trigger Point Injection (TPIs), and Prolotherapy., • Obtain and document shared informed consent for procedures. Treatment Planning & Implementation • Develop and implement individualized care plans in collaboration with patients, families, and providers., • Adjust treatments based on ongoing evaluations and patient response., • Establish and reassess short- and long-term goals. Documentation & Compliance • Complete SOAP notes and maintain accurate EHR records for all encounters., • Document diagnostics, treatments, medications, and patient instructions., • Participate in chart reviews, peer reviews, and quality assurance initiatives., • Ensure HIPAA compliance and maintain patient confidentiality. Collaboration & Support • Work with physicians, clinical partners, attorneys, and other stakeholders., • Participate in staff education, clinical guideline development, and meetings., • Create and improve documentation templates and processes., • Travel 10–25% to satellite locations as needed., • Perform additional duties as assigned. Qualifications: • Licensed and certified Nurse Practitioner in California (unrestricted)., • Ability to obtain Nevada NP license within 90 days of employment., • Proficient in independent medical assessment, diagnosis, and treatment planning., • Strong clinical judgment and ability to form evidence-based medical opinions., • Excellent documentation, EHR, organizational, and communication skills., • Proficient with technology and clinical applications., • Valid and unrestricted driver’s license., • Ability to travel 1-2 times per week to satellite locations.
