ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Floating Direct Support Professional will include, but are not limited to the following: • Provide support as a Direct Support Professional in various designated programs, based on need., • Provides care and assists with the personal development and independence of the people we support., • Ensures that all measures for safety and well-being are maintained at the highest level., • Teaches the people we support to improve their activities of daily living and in the development of self-help and social skills., • Attends weekly staff meetings as well as any other assigned meetings and in-service training., • Prepares concise written reports including Incident Reports, Progress Notes and other reports as assigned., • Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action., • Ensures actions are conducted in a manner that prevents abuse and reports observations of co-workers who may be abusive in accordance with established procedures., • Administers appropriate first aid based on visible symptoms and medical history in accordance with established guidelines and accepted first aid practices., • Summons police, fire, ambulance or other emergency personnel if situation warrants it., • If an emergency has caused a person we support to exhibit behavior that is threatening or dangerous, may be required to provide strategies for crisis intervention prevention in accordance with OPWDD approved Behavior Management guidelines., • Travels with the people we support in agency vehicles to and from work, shopping, programs, restaurants, movies, doctor’s appointments, etc., • Assists with meal planning and preparation as well as with purchasing food and supplies., • Demonstrates an understanding of and implements behavioral programs., • Participates fully as a member of interdisciplinary plan., • Maintains an ongoing informal exchange of information. Maintains written logs for recording daily activities., • Performs housekeeping, cooking, laundry, light maintenance and light preventative maintenance duties as needed to maintain a safe and homelike environment if assigned to an Agency residential program., • Supervises any other activities as may be necessary to maintain quality of the program., • Dispenses prescribed medications to the people we support after successful completion of medication administration course and in line with Agency protocol. AMAP’s are supervised by program nurse(s) in all areas related to medication administration., • Completes all activities related to skill building as outlined in individual program plans., • Attends to personal hygiene needs of the people we support, i.e. toileting, bathing and changing as situation warrants., • Handles all documents for the people we support in a secure and confidential manner., • Performs related duties as requested. collects and enters data as part of daily operations using an electronic health record (e.g. Digital Agency (DA), Electronic Medication Administration Records (eMAR). * Reviews and completes all other required paperwork and documentation to ensure compliance and/or billing completely, accurately and within appropriate timeframes (e.g., notifications, logging, incident reports, update files, maintenance requests, phone calls, and tracking of incidents). * Uses foundational teaching skills (Explain, Model, Observe and Evaluate [EMOE], task analysis, essential skills, supports and modifications, etc.) and progressive teaching. * Understands behavior as communication and employs person-centered thinking to support positive interactions and prevent further escalation. * Complies with all Federal, State, Local and other relevant regulatory agency requirements, including the Health Insurance Portability and Accountability Act (HIPAA). * Reports maintenance, upkeep and safety concerns observed at YAI facilities to Program Supervisor or appropriate designee. * Consults with program leadership, clinical and other direct support staff to ensure streamlined and top-quality care for individuals and attends and actively participates in team meetings. * Utilizes/demonstrates approved YAI and OPWDD training techniques (i.e. verbal, environmental, and physical interventions, including take downs) to respond timely and appropriately to challenging and/or self-injurious behavior, ensuring the health and safety of the people we support, self and others. * Provides coverage for other direct support staff as requested, which may include assisting at another YAI or off-site location (e.g. hospital) based on needs of program and/or individuals supported, including remaining on shift to ensure maintenance of minimum coverage levels for program. * May participate in the development of treatment plans. * For certain assignments, may be required to operate YAI motor vehicle to transport individuals to and/or from programs, residences, community or other locations on a daily (or other regular) basis with appropriate credentials. * Performs all other duties, as assigned. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Residence Program Specialist will include but are not limited to the following: • Provides care and assists with personal development and independence of the people we support., • Ensures that all measures for safety and well-being are maintained at highest level., • Provides training for the people we support in activities of daily living and in development of self-help and social skills., • Maintains a personalized, homelike atmosphere in the residential facility., • Attends weekly staff meetings as well as any other assigned meetings and in-service trainings., • Prepares concise written reports including Incident Reports, Progress Notes and other reports as assigned., • Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action., • Ensures actions are conducted in a manner that prevents abuse, and reports observations of co-workers who may be abusive in accordance with established procedures., • Administers appropriate first aid based on visible symptoms and medical history of the people we support in accordance with established guidelines and accepted first aid practices., • Summons police, fire, ambulance or other emergency personnel if situation warrants it., • If an emergency has caused a person we support to exhibit behavior that is threatening or dangerous, may have to utilize strategies for crisis intervention in accordance with OPWDD approved Behavior Management guidelines., • Provides transportation for the people we support in agency vehicles to and from work, shopping, programs, restaurants, movies, doctor’s appointments, etc., • Assists with meal planning and preparation as well as with purchasing of food and supplies., • Demonstrates an understanding of and implements behavioral programs., • Participates fully as a member of interdisciplinary plan., • Maintains an ongoing informal exchange of information. Maintains written logs for recording daily activities., • Performs housekeeping, cooking, laundry, light maintenance and light preventative maintenance duties as needed to maintain a safe and homelike environment., • Supervises any other activities as may be necessary to maintain quality of the program., • Dispenses prescribed medications to the people we support after successful completion of the medication administration course, and in line with Agency protocol. AMAP’s are supervised by residential nurse in all areas related to medication administration., • Completes all activities related to skill buildings as outlined in individual program plans, • Provides support around personal hygiene, i.e. toileting, bathing and changing as needed., • Handles all personal documents of the people we support in a secure and confidential manner., • Performs related duties as requested. About the Role Accountable for the development, direction, and coordination of supports for individuals receiving services from the I/DD Services department. Actively supports the organization’s mission and core values, endorsing a culture of people-centered choice and their achievement of the highest quality of life. What You’ll Do • Program Development, • Participates in the application for funding to provide residential services to adults with Intellectual/Developmental Disabilities based on people served needs and desires., • Creates, implements, and enforces policies and practices that reflect the mission and philosophy of the organization and funding sources., • Coordinates with other departments such as Clinical, Finance, General Services, QI, Development, and Human Resources, • Coordinates the management of Reportable Incidents through to their conclusion for residential programs., • Monitors Human Rights issues in regard to behavior management and the use of psychotropic medications., • Works closely with the Director Entitlements and Community Affairs to ensure full census and benefits for those individuals deemed eligible for Residential Services Delivery of Habilitative Services, • Develops, facilitates, and models appropriate treatment models and techniques used in residential settings for adults with Intellectual/Developmental Disabilities., • Provides I/DD Staff with the resources necessary to meet criteria of Residential Services and Community Inclusion, • Monitors adherence to regulations and standards for the field., • Completes observations at all residential settings at least monthly., • Reviews the staff action plans to ensure meets all regulatory regulations., • Oversees delivery of services to all individuals served in assigned programs and ensures that services are properly documented., • Recommends changes in the delivery of treatment programs based on service outcomes. Planning and Administration, • Develops and implements outcome monitoring system for staff action/habilitation plans in coordination with all services provided to the person served., • Ensures adherence to standards for OPWDD funded programs., • Creates and manages budget through subordinate supervisors., • Represents unit, department, or division in multi-disciplinary meetings., • Represents organization with governmental agencies, community groups, community based organizations, and families of residents., • Participates in the development and monitoring of each programs P.E.I.s Management and Supervision, • Responsible for: conducting one unannounced visit per month to I/DD IRAs and completing the unannounced visit checklist during each visit and reporting results to Vice P resident and Director QI within one week of visit; and following up on action items at next unannounced visit., • Actively review all evaluation and disciplinary action., • Ensures that staff meet mandatory training requirements., • Provides coaching and mentoring to subordinates., • Provides on-call emergency follow up and guidance as needed., • Must be culturally sensitive to the population being served Supervisory Responsibilities, • Manages subordinates who supervise direct care and clinical staff in the Intellectual/Developmental Disabilities Department’s, I/ DD Residential Programs., • Responsible for the overall direction, coordination, and evaluation of each program in conjunction with the Vice president of I/DD Support Services, Coordinator of Intake and Entitlements and the Senior Vice President for I/DD Services., • Gives direct supervision to non-supervisory employees who work within the I/DD offices., • Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws., • Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems., • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
