Job Description Location Bronx 10466 Schedule Full-time PM | Sunday-Thursday 3pm-11:30pm | 40 hours Essential Job Functions with a disability to perform the essential duties unless this causes undue hardship to the agency.) DSP provide observations, evaluations, and reports to other team members to ensure and/or record an individual’s progress toward treatment/habilitation goals. In accordance with habilitative plans, they guide and teach individuals to develop skills required for a more independent lifestyle. Incumbents may take a lead role in one or more of the following duties within a work site. Duties may be performed in several or all of the following areas: Personal Care • Teaches and reinforces accepted infection control and personal grooming techniques; assists in bathing, dental care, hair and nail care, personal hygiene, toileting, incontinence care, menstrual care, and grooming routines as necessary., • Teaches and assists individuals in dressing, clothing selection, storage, laundering and repair of clothing and accessories; helps them to shop for clothing., • Teaches and assists in proper use and care of shavers and razors, eyeglasses, hearing aids, prostheses as prescribed by appropriate medical practitioner., • Administers and teaches individuals first aid treatments; administers cardiopulmonary resuscitation techniques and performs other health-related techniques as required., • Takes, records, reports temperatures, blood pressures, pulse rate, weight, and other significant symptoms/health-related occurrences; aids developmentally disabled individuals in understanding, recording, and reporting such factors., • Maintains confidentiality of information., • Reports unusual incidents and physical or behavioral symptoms promptly and accurately to the supervisor., • Applies valid and accepted limits for inappropriate behavior and helps control and restrain individuals with challenging behaviors in their care., • Administers prescribed medications and treatments; teaches, observes, reports problems and progress in self-administration of medications programs; assists individuals in achieving more independence in administration of medications; teaches use of skin care lotions, topical ointments, eye and ear drops and assists in their use as needed., • Assists individuals in eating who cannot feed themselves independently., • Teaches individuals to eat in a family-style setting; models and teaches mealtime socialization skills. Under the direction of the appropriate level supervisor, the DSP performs various tasks and duties to assure that the living environment is properly maintained and kept in a clean, neat and orderly condition. • Performs/teaches developmentally disabled individuals routine housekeeping duties such as dusting, washing floors and walls, cleaning bathrooms, and vacuuming rugs., • Assists developmentally disabled individuals in bed making., • Follows safety procedures in living unit by mopping up spills, reporting unsafe conditions, teaches and reinforces fire safety procedures. Checks to see that fire exits are clear, night-lights and exit signs on, windows locked, etc., • Stores supplies and assures that storage areas are kept in a neat and orderly condition., • Responsible for storage and replacement of linens., • May assist in various household inventories., • Performs/teaches minor and routine maintenance tasks such as changing light bulbs, fuses, replacement of faucet washers; and obtains repair services for appliances., • Performs/teaches routine removal of trash, cleaning of trash containers, ashtrays, etc., • Teaches/performs outdoor maintenance tasks and repairs of residence, yard, garden, walks; reports need for or obtains repair services for automobiles, vans, mowers, etc., • Checks fire equipment and reports any malfunction., • Teaches and reinforces use of a variety of communication skills., • Guides individuals through motor exercises designed to increase physical coordinative functions., • Teaches and reinforces principles of human growth and development, including human sexuality, as determined by each person's developmental level and program plan., • Teaches acceptable work habits, e.g., punctuality, dress and grooming, to enable individuals to develop readiness for work outside the home, or in support of individuals' habilitative goals., • Accompanies developmentally disabled individuals to community activities including worksites, cultural, religious and social events, day programs, and visits to health practitioners; aids them in developing awareness and use of community resources and in interacting with others., • Provides support for maximum community integration., • Transports/arranges for transport of individuals to community events; and aids them in development of their mobility skills., • Maintains an adequate supply of and accounting of personal clothing and possessions., • Teaches money management principles to developmentally disabled individuals; and completes/aids in completion of banking activities., • Provides an accurate accounting of all financial transactions made on behalf of individuals in their care., • Helps developmentally disabled individuals to develop/maintain positive interpersonal relationships with their peers, with others in their families, and with others in the community. Aids in explaining feelings, resolving conflicts, teaching/reinforcing acceptable means of dissipating frustration, and anger., • If pets are in the home, teaches/assists with their feeding, grooming, and care., • Teaches/supervises/participates in a variety of leisure and recreational activities, crafts projects, seasonal and permanent home decoration; assists in planning, supervising and conducting holiday celebrations selected by residents and staff., • May accompany developmentally disabled individuals to dances, camping trips, swimming, horseback riding, basketball, or other sports; assists in selection and teaching of appropriate community inclusion leisure activities., • Observes and reports on behavior and specific responses to programming, medication, interpersonal relationships, etc., • Actively participates at staff meetings by communicating observations concerning progress and reaction to various therapy programs., • At meetings, may suggest alternative programs or procedures based on experience with the individual’s habilitation plan., • Records all significant behavioral responses in prescribed format., • Participates in the development of individual program and habilitation plans., • May be asked to assist in the orientation and training of other staff., • Knowledge of policies and procedures of OPWDD, as well as state and federal regulations as they apply to the care and treatment of the developmentally disabled., • Knowledge of active treatment programs as they relate to the developmentally disabled., • Currently AMAP certified or able to become certified within six months of hire., • Currently SCIP-R certified or able to become certified within six months of hire., • Currently CPR and First Aid certified or able to become certified within six months, • Direct Care experience with people with mental and/or physical disabilities. Supports provision of appropriate, therapeutically documented levels of supervision to ensure and protect the health, safety and well-being of individuals at all times by ensuring updates and changes to levels of supervision for persons we support are regularly communicated to all staff and appropriately documented; may run shift meetings, assigning duties, completing shift sheet and making recommendations to ensure staff are qualified and able to maintain current prescribed levels of supervision. Understands and uses specialized clinical knowledge of person-specific needs and signs or symptoms of ailments/syndromes including mental health/medical conditions to provide effective supports to individuals, identify and escalate concerns to leadership and inform activities, training or specialized education or augmented trainings for staff. Stays up to date with current issues, diagnoses, needs and levels of supervision for the persons supported in assigned program under the supervision of BIS. Works with BIS to identify habilitative, behavioral and psychiatric needs of the persons supported and to ensure all programming and activities relating to clinical interventions are implemented consistently, in accordance with individualized plans. Provides guidance to program staff, setting the appropriate tone and ensuring that the standards of care are followed by modeling excellent counseling skills and interactions with persons supported and through use of person-centered philosophy and tools. Implements Behavior Support Plans, demonstrating understanding of behavior as communication and employing person-centered thinking to support positive interactions and prevent further escalation. Trains direct care staff individually, on the floor and/or in groups using the EMOE training system to support effective implementation of habilitative and behavioral programming by program staff through regular observations, direct feedback, communicating clinical rationales and ensuring appropriate data collection techniques. Serves as lead trainer for weekend staff and supports new staff in developing and demonstrating the OPWDD Core Competencies. Participates in the development of treatment plans including individualized habilitative plans, collaborating with BIS to write services, goals, outcomes and/or supports in accordance with the agency philosophy, as determined by assessments of individual needs and learning capabilities. Assists BIS with gathering and collecting data required to conduct formal and informal behavioral and habilitative assessments (written questionnaires, standardized tests, rating scales, baseline data, etc.) by conducting observations and/or soliciting information from other direct support staff, circles of support or other sources, as applicable. Monitors collection of data used to assess progress toward habilitative and behavioral goals, ensuring that new data sheets are accessible to staff at the beginning of each month and that staff are consistently and accurately collecting and documenting data in accordance with established systems by reviewing documentation, retraining staff and following up as needed; may assist with design of data collection systems for habilitative goals. Completes and maintains all required paperwork and documentation for persons supported in appropriate format(s) and file(s) in accordance with applicable requirements including compiling, totaling and summarizing data, writing monthlies describing overall progress and completing person centered materials; reviews and ensures all psychiatric/therapy anecdotals are complete and appropriate and assists other program staff with documentation or compiling information or data, as needed or directed. Attends and actively participates in team meetings as a member of the interdisciplinary team (IDT), consulting with IDT to ensure streamlined and top-quality care and providing feedback or ideas to improve supports and/or program operations and updates or changes relating to data management, assessments or programming requirements. Regularly participates in life plan and other meetings for persons supported and in regional assistant or other clinical team meetings providing information, contributing to agendas and completing tasks or activities resulting from meetings as directed by leadership team (training, assessments, reviews, etc.). Serves as lead on weekend and evening shifts or when otherwise requested, monitoring day-to-day program operations and providing support to staff in the absence of Program Supervisor, Assistant Supervisor and/or BIS. Maintains an awareness of program operations, staff performance and overall condition/wellness of persons supported through regular interactions and observations and by working together with the leadership team to identify and directly address, resolve, recommend actions or solutions and/or appropriately escalating concerns, signs and symptoms, problems or other occurrences. Completes and maintains all necessary trainings and certifications, including specialized trainings based on program and to ensure compliance with YAI policies or other applicable rules or regulations and clinical trainings as recommended by BIS, effectively applying skills or teaching others. Complies with all Federal, State, Local and other relevant regulatory agency requirements, including the Health Insurance Portability and Accountability Act (HIPAA). For certain assignments, may be required to operate YAI motor vehicle to transport individuals to and/or from programs, residences, community or other locations on a daily (or other regular) basis with appropriate credentials. Performs all other duties, as assigned Experienced Assistant BIS may be selected for assignment reporting into the BIS Department, under day-to-day supervision of Program Supervisor and BIS Supervisor performing the following tasks, in addition to the above: Provides support and coverage by performing ABIS tasks for multiple programs across assigned region, with assignments of varying length, frequency and schedule as determined by caseload needs and as directed by BIS Coordinator/Supervisor. Integrates into assigned program teams, demonstrating flexibility in accommodating the needs of each specific program and provides training on behavioral/clinical system to new staff, as needed. Obtains and effectively communicates behavioral/clinical information to interdisciplinary teams and families as directed by the BIS and ensures tracking and completion of follow-ups needed for the people we support across multiple programs. Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.) Bachelor’s degree in psychology, social work or closely related Human Services discipline and at least six (6) months of experience working with individuals with intellectual and developmental disabilities (I/DD) and/or supporting adults or children with complex social-emotional, mental health and/or behavior support needs in a group or individual setting (home program, day treatment center, school, ABA environment, etc.) or Formal training, including college coursework in psychology, sociology, human behavior, mental health, special education and/or I/DD and at least two (2) years of experience as described above; or At least four (4) years of experience in a direct care role (e.g. DSP) in a YAI program in good standing; and Working knowledge of behavioral/psychiatric conditions, including the interplay between I/DD and other conditions and ability to effectively gather, document and accurately communicate related information to staff, clinical team and/or external providers; and Exceptional written and oral communication skills including the ability to professionally, coherently and objectively document services provided and summarize progress for billing and tracking purposes and to effectively convey information regarding individuals supported across various levels and with external providers or circles of support; and Ability and willingness to lead, direct and teach others technical skills and/or knowledge, both on-the-job and through formal training in group or individual settings; and Must hold foundational certifications for current service setting: Strategies for Crisis Intervention and Prevention – Revised (SCIP-R), CPR (issued by American Red Cross or American Heart Association) and YAI-issued Approved Medication Administration Personnel (AMAP) certification for residential assignments or Day Services Model Mentorship Training for day assignments. Current YAI employees changing settings must obtain AMAP (for new residential assignment) or Day Services Model Mentorship Training (for new day assignment) and any additional certifications as required by program assignment and persons supported within three (3) months of appointment Newly hired employees must obtain (and maintain) required YAI certifications based on setting and program within six (6) months of appointment; and Must maintain foundational certifications (as described above) and any required program-specific certifications; or Satisfactory equivalent combination of education, experience and/or training; however, all newly hired employees must be at least 18 years old. Job DescriptionJOB SUMMARY The Department Case Manager supports individuals receiving OPWDD and other Regulatory Agencies to ensure the foundation necessary to achieve their highest level of independence as they transition into adulthood. Through person-centered planning, service coordination, and ongoing case management, the Case Manager facilitates access to social, vocational, and community-based opportunities in collaboration with the agency and external providers. The role includes intakes, enrollment for, afterschool and weekend respite services, day habilitation, camp, employment services, and community engagement. ABOUT YOU The Case Manager is responsible for the development, implementation, and ongoing monitoring of individualized service and habilitation plans in accordance with OPWDD/and other Regulatory Agencies. This includes ensuring services promote individualization, independence, inclusion, and productivity, as well as coordinating supports among internal agency departments, families, and external service providers to ensure continuity and quality of care. The Case Manager will work with the Department Senior Director to ensure all enrollment, reporting and compliance documentation is completed in a timely manner for all current and future programs. WHAT YOU’LL DO • Oversees client care by assessing treatment needs during the intake and enrollment process; developing, monitoring, and evaluating individualized treatment plans; facilitating interdisciplinary collaboration; and monitoring staff performance., • Manages the admission of new clients by reviewing records and applications and conducting comprehensive orientations., • Determines client needs through intake interviews; coordinating therapeutic, medical, psychosocial, and psychiatric evaluations; and reviewing clinician evaluations, treatment objectives, and care plans., • Establishes and coordinates treatment programs by developing schedules and routines; overseeing service delivery; and arranging necessary resources, including transportation and escorts., • Monitors ongoing cases by tracking attendance; evaluating treatment effectiveness and client responses; advocating for services and entitlements; securing additional resources; and offering ongoing client support., • Maintains accurate and compliant client records by reviewing case documentation and systematically recording significant events and progress updates., • Communicates client progress through weekly interdisciplinary meetings and formal evaluations; disseminates outcomes, barriers, and contributing treatment factors to clinical teams and families., • Facilitates client transition by reviewing and strengthening transition plans; coordinating post-transition services; educating and training family members; and providing appropriate community resources., • Improves treatment outcomes by analyzing, evaluating, and redesigning programmatic operational processes; implementing evidence-based improvements; and updating policies and procedures accordingly., • Prepares comprehensive reports by collecting, analyzing, and summarizing treatment outcomes, performance metrics, and trends; compiling statistical data; and completing grant and subsidy applications., • Maintains professional expertise by engaging in continuing education, reviewing professional literature, networking with peers, and participating in relevant professional organizations., • Must have a valid NY State Driver’s License, • Experience within a human service-related environment., • 2 years’ experience working with the Special Needs Population, • Preferred BA/BS degree in Special Education, Social Work, Psychology- or any other Human Service-related fieldSALARY, • Full-time ABOUT US The Jewish Community Center of Staten Island was founded in 1929 with the goal of creating a home for the Jewish community by offering social, recreational, and educational activities.
