Our foster care continuum of care includes Enhanced Family Foster Care, Prenatal, Pregnant and Parenting Agency Operated Boarding Homes, Group Homes, Diagnostic HTP Center and campus-based Residential Treatment Services. PROGRAM BACKGROUND: Enhanced Family Foster Care (EFFC) recognizes that all children in care have experienced some level of trauma, that their needs may change over time, and that their strengths must be supported to successfully reach their full potential. As a blended model, EFFC: • Responds to the needs of the children and family – Allows children, parents, and families to receive therapeutic services and support, accessing a full menu of options with less separation into different programs. POSITION SUMMARY: • The Homefinding Department is responsible for the recruitment, training and certification of all foster homes within MercyFirst. With approximately 200 homes either certified/approved or in the process of recertification, Homefinding is the initial contact for prospective foster parents within the agency’s foster care division. MercyFirst approaches its long-standing commitment to provide safe, supportive, and adequate care to any child referred for service by ensuring that the foster families who provide full-time care and supervision are compassionate, well prepared and capable of meeting the needs of the children and families for whom they become partners in service. The Homefinding Supervisor is responsible for the direct supervision, coaching and support of the Homefinding team, focused on marketing, outreach and recruitment, training, assessment, engagement and support of prospective and existing foster families. Approach to service is family-centered, trauma-informed, and strengths-based. The qualified candidate will be responsible for supervising homefinding staff, tracking regulatory compliance, maintaining safe placements for children, adherence to reporting requirements and timeliness, and meeting programmatic outcome measures. THE OPPORTUNITY The Center for Justice Innovation is seeking a Senior Social Worker to join a team of staff—including social workers and case managers—who work with defendants participating in the Manhattan Felony Alternative to Incarceration (ATI) Court. The ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to the Clinical Director, the Senior Social Worker will be part of a team that provides neutral, third-party assessments and case management services to the ATI Court. The Senior Social Worker will conduct independent clinical assessments, provide referrals to community-based services, offer comprehensive clinical case management, and monitor participant compliance. The Senior Social Worker will be primarily responsible for supporting a caseload of high-risk need program participants and supervising a group of case managers and/or social workers. Additionally, in coordination with the Clinical Director, the Senior Social Worker will help coordinate compliance updates on ATI Court calendar days, help identify gaps in services and forge new partnerships with community-based providers to fill those needs, serve as the team’s training liaison to identify internal training needs and coordinate trainings for staff, and coordinate with the Office Manager regarding participant supplies and other needs. Community Access’ Bronx Shelter ACT Team will serve individuals with mental health conditions who are experiencing homelessness in the Bronx, and in other boroughs. ACT team members share responsibility for the people they serve, helping individuals attain housing and other personal goals that may be of a health, social, work or education nature. ACT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, family support, employment, and wellness self-management. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week. Overview The ACT Team Vocational Specialist is responsible for meeting participants in the community and providing holistic recovery services tailored to the individual’s stated needs. The Vocational Specialist will be responsible for providing ongoing supportive counseling and service coordination for homeless adults living in Bronx mental health shelters, as well as individuals who have recently transitioned to supportive or independent housing. The Vocational Specialist is also responsible for integrating person-centered goals and services with the tasks of all ACT team members and for providing psycho-education individually and in groups. The Vocational Specialist works as part of a multi-disciplinary team providing the range of ACT services to assist participants in achieving their treatment, rehabilitation and recovery goals. They specialize in assisting people with honing vocational goals and finding schools, training, jobs or employment programs that will help them reach them.
