Evaluates patient responses to therapy/interventions. Establishes a treatment plan based on clinical findings and determines when further evaluation by the collaborating physician, specialist, or emergency care is warranted., • Collaborates with patients, families, primary care physicians and other team members to provide assessment and care planning. Assesses, plans, and provides intensive and continuous care management across client settings., • Manages and provides clinical services in compliance with standards of Patient-Centered Medical Home standards, meaningful use of medical record data, HEDIS and QARR quality of care measurements., • Manages inter-professional team efforts regarding the medical, nursing, therapy and ancillary care provided to patients to ensure quality outcomes are achieved., • Participates in on call coverage schedule to ensure 24/7 access to practice clinicians., • Assesses, educates, and improves client and caregiver knowledge of chronic disease, self-care management, and identification of changes in health status, including appropriate responses and actions through individualized education and inter-professional interventions., • Observes and analyzes team performance patterns related to population under care and assists in developing interventions to improve team performance. Provides remedial support and guidance to interdisciplinary team members to address implementation/evaluation of plans that maintain/increase customer experience with care, cost-efficiency, and quality care compliance with regulatory standards., • Communicates with internal and external care partners regarding the needs of the patient or population to ensure interventions occur in a timely and appropriate manner. Recruiting Certified Peer Specialist for Manhattan Psychiatric Center Manhattan Psychiatric Center (MPC), an Office of Mental Health (OMH) Joint Commission-accredited facility, is recruiting a Certified Peer Specialist to join their Transition to Home Unit (THU) team. The THU team provides a multi-disciplinary approach to care for individuals who have a history of homelessness. The goals of the THU team are to develop personalized treatment plans for active rehabilitation and reintegration to the community. The schedule is Monday through Friday from 9:00 AM to 5:00 PM with pass days of Saturday/Sunday. As a Certified Peer Specialist, you will support the recovery and wellness of individuals in the THU. The successful candidate will be jointly mentored by Drs. Tuuli Lappalainen and Neville Sanjana at the intersection of statistical genetics and functional genomics analyses and tech development and have the opportunity to also be affiliated with New York University. About the labs The Sanjana lab works at the intersection of genomics and bioengineering. We are curious about many things and are passionate about what we do. We primarily develop new technologies for hacking and understanding biological systems. We try to learn from nature and to re-purpose what we learn to gain deeper insights into how genomes and neural systems function. In the lab, we modify genomes, perform high-throughput screens, differentiate human neurons from stem cells, watch synapses rewire, find vulnerabilities in cancer cells and have a lot of fun in the process. We aim to have a tangible impact on improving human health by creating new biological tools and fundamental biological discovery. The Lappalainen lab studies functional genetic variation in the human populations by analyzing how genetic variants affect molecular and cellular traits, and how these cellular changes contribute to genetic risk for both common and rare diseases and traits. We analyze these questions both by computational analysis of large genomic, transcriptomic and multi-omic data sets, and by experimental approaches. Job Description About the Role Hamilton-Madison House (HMH) is the leading provider of behavioral health services for New York City’s Asian community and the second largest provider in the country. HMH serves nearly 1,000 community members through a comprehensive program model that understands and incorporates the strengths of individual cultures and addresses the stigma related to mental illness. Services range from psychological services; personalized recovery-oriented services for individuals with mental illness; recovery programming for addiction in six languages and seven dialects; supportive housing for people with mental health issues; and training and internship opportunities for graduate students in social work, mental health counseling, and psychology. Job Description Summary: The Peer-to-Peer Mentor is an essential member of the Fatherhood Program team. This role supports fathers by providing lived-experience mentorship, assisting with group facilitation, helping participants navigate program requirements, and building community across all four program tiers. The Mentor strengthens father engagement, supports retention efforts, and contributes to the program’s father-friendly environment. This role is designed for someone with personal experience as a father or caregiver who can connect authentically with participants, model positive fathering behaviors, and support fathers as they work toward reunification, improved relationships, and personal development. Job Description Summary Streetwork Project is an innovative outreach-counseling program for homeless youth in Harlem, which utilizes a harm reduction approach providing comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). The Youth Services Navigator will promote access to harm reduction supplies and services at Streetwork’s Drop-In Center, with a focus on LGBTQ youth and youth of color at risk of SUDs Youth Services Navigators (YSN) will facilitate and conduct one-on-one harm reduction goal setting intervention for drug using clients. Youth Services Navigators will provide overdose prevention and response trainings for drug users. The YSN will provide youth seeking harm reduction services with information, counseling and supplies (safer syringe services, sniff kits, safer sex supplies, Naloxone kits and sharps disposal kits), The Youth Services Navigators will also be needed to provide general program services such client monitoring in the milieu, intake and assessment, supportive counseling, referrals, syringe exchange, escorts and the provision of concrete services (meal preparation client, laundry, provision of supplies), and groups.
