Overview Provides clinical leadership to promote increased compliance with a range of quality and cost measures and standards of care. Manages service delivery of inter-professional and para-professional team members working on an individual case or population of cases. Assesses, plans, and provides intensive and continuous care management across client settings., • Manages and provides clinical services in compliance with standards of Patient-Centered Medical Home standards, meaningful use of medical record data, HEDIS and QARR quality of care measurements. Qualifications Licenses and Certifications: • License and current registration to practice as a Registered Professional Nurse in New York State required, • Certificate (license) and current registration to practice as a Nurse Practitioner in the State of New York, with a specialty in adult health, family health or gerontology required, • Valid driver's license, as determined by operational/regional needs may be required, • Maintains credentialed status with VNS Health Medical Care at Home and associated managed care plans required, • Maintains NPI, Medicaid and Medicare provider numbers preferred, • Maintains a collaborative practice agreement with a physician in compliance with New York State regulations preferred, • Must be certified by ANCC or another accrediting Nurse Practitioner body – in order to bill Medicare and meet credentialing requirements required, • For Psychiatric Nurse Practitioners only:, • Master's Degree of Science in Nursing, or other graduate degree from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP certification (licensure) required, • Current ANCC or AANP certification as an adult, family or geriatric nurse practitioner required, • For Psychiatric Nurse Practitioners only:, • Master's Degree in psychiatric-mental health Nurse Practitioner required, • Minimum of two years of experience as a nurse practitioner utilizing full scope of practice preferred, • Clinical home care experience or two years managerial experience preferred, • Demonstrated knowledge of Hedis and QARR quality measures, ICD-10 and CPT coding for reimbursement of services required, • Bilingual skills, as determined by operational needs required Pay Range USD $109,900.00 - USD $146,500.00 /Yr. Roles & Responsibilities: The Scientific Computing and Data group at the Icahn School of Medicine at Mount Sinai partners with scientists to accelerate scientific discovery. To achieve these aims, we support a cutting-edge high-performance computing and data ecosystem along with MD/PhD-level support for researchers. The group is composed of a high-performance computing team, the research clinical data warehouse team and a research data services team. The Senior HPC Administrator, High Performance Computational and Data Ecosystem, is responsible for a computational and data science ecosystem for researchers at Mount Sinai. This ecosystem includes high-performance computing (HPC) systems, clinical research databases, and a software development infrastructure for local and national projects. To meet Sinai’s scientific and clinical goals, the Senior Administrator has a good technical understanding for computational, data and software development systems along with a strong focus on customer service for researchers. The HPC Senior Administrator is an expert troubleshooter and productive team member and leads projects to effective and efficient completion independently under little to no supervision. Interested in exploring the role of adipose tissues in mucosal inflammation in a supportive, creative, and inclusive environment? The Grasset lab Our lab’s long-term objective is to uncover novel mechanisms underlying the pathology of Crohn’s disease (CD), a progressive incurable disease characterized by transmural intestinal inflammation leading to complications often requiring bowel resection surgery. The goal is to identify novel therapeutic targets by focusing on two previously disconnected hallmarks of CD: expanded mesenteric adipose tissue wrapping around the intestine – or creeping fat – and anti-microbial antibodies, both correlating with disease complications. Studying these hallmarks will further reveal unexplored relationships between adipocytes, stromal cells, and immune cells driving B cell responses in gut homeostasis and inflammation. Understanding these relationships will impact how we perceive immune responses at other mucosal sites and systemically. Ongoing projects include: the study of the impact of CD-specific microbiota on stromal cell-supported lymphoid structures embedded in mesenteric adipose tissue; specific mechanisms by which stromal cells and adipocytes communicate with B cells; and the subsequent impact on B cell activation and their differentiation to anti-microbial antibody-producing plasma cells. Serves as an advisor for designated Columbia Engineering MS programs, including but not limited to MBAxMS and the MS in AI program. Tracks and provides guidance to students on curriculum requirements, registration issues, and academic planning. Refers students to their respective faculty advisors and Career Placement Officers to address electives, career-related questions, and other issues. Serves as a liaison with Columbia Engineering departments in supporting all aspects of student affairs to ensure student success. Assists with escalated student cases from Columbia Engineering departments on complex academic policy issues, providing high-touch support and case management. Collaborates with other schools, offices, and departments (GSAS, ISSO, Registrar, Health Services, Student Financial Services, and Columbia Residential) on student cases as needed. Maintains a very high level of knowledge of school academic policies and procedures. Attends, assists, and presents at new-student orientation, relevant school-level or program-specific meetings and events, and other events throughout the year. Tracks, assesses, and reports on student data. Identifies students who are not meeting satisfactory academic progress. Recognizes and advises on trends in advising cohorts. Graduate Student Affairs (30%): Assists in monitoring and tracking MS and doctoral students' degree progress, registration status, academic standing, and changes in status. Identifies students who are not meeting satisfactory academic progress. Provides support and coordinates with department student affairs officers on academic plans for students who require monitoring or intervention. Identifies trends on a larger scale and works to address them through school policy and at the program level. Serves on the Engineering Student Affairs graduate student on-call team, providing crisis intervention for graduate students experiencing difficulty. Responsible for engaging with the student/s and then connecting the student to the appropriate resources for further engagement. Assists with responding to urgent situations. Supports key onboarding, orientation, and professional development programs for doctoral students, including Engineer Your PhD (EYP), Three Minute Thesis (3MT), and PhD Connect. Partners with other Engineering Student Affairs teams, including Student Life and Professional Development and Leadership (PDL) teams, to innovate programming to meet evolving student needs. Assists with partnership and dual-degree program student recruitment efforts, such as but not limited to open house events and yield programming. Liaises with admitted students to address admissions, academics, and registration inquiries. Graduate Academic Affairs (15%): Reviews graduation clearances for MS and MPhil degree levels. Maintains advising resources, program guides, and communication templates for both MS and doctoral audiences. Supports departmental outreach by sending reminders about upcoming deadlines, policy changes, and required actions.
