Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Position Overview Under supervision of the Site Medical Director, the Physician Assistant will provide comprehensive, compassionate, and thoughtful care to our patients in the New York City jail system. The Physician Assistant will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services Responsibilities include: • Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated., • Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable., • Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits., • Implement plans for patientcare utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care., • Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate., • Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented., • Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed., • Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags)., • Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care., • Ensure that all progress notes and orders are signed before the end of the shift., • Respond to emergencies in a timely and professional manner., • Notify the staff MD and Urgicare about 3 hour runs and EMS activation., • Complete special housing rounds when assigned., • Be familiar with quality of care and population health indicators. Minimum Qualifications 1. Valid license and current registration to practice as a Physician Assistant issued by the New York State Education Department (NYSED); and, 2. Current certification issued by the National Commission on Certification of Physician Assistants (NCCPA); and, 3. One (1) year of full-time, satisfactory clinical experience as a Physician Assistant; and, 4. Possession of a valid Life Support certification (e.g., Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS)) is required in the appropriate practice area through the American Heart Association (AHA) within sixty (60) calendar days of appointment to the title. The Consultant position is a mid-level consulting staff position. This position requires a highly motivated problem solver with strong analytical ability and a desire to advance within the organization. An individual with an entrepreneurial spirit and an ability to apply creative solutions is a natural fit for this position. The Consultant is an integral part of the CDI team and works closely with the client’s CDI team, supporting classroom education and mentoring. They are responsible for ensuring the successful transfer of CDI best practices from the consulting team to the client team. The Consultant also facilitates accurate documentation for severity of illness (SOI) and quality in the medical record, which involves extensive record review and interaction with physicians, health information management professionals, coding professionals, and nursing staff. Correctional Health Services The New York City jail system is the second largest in the nation. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Summary of position The Senior Psychiatrist is the psychiatric leader of the facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Supervising Psychiatrists (if applicable) to ensure that safe, holistic and evidence-based therapeutic interventions are provided for the individuals incarcerated in the New York City jail system. North Infirmary Command (NIC), West Facility (WF), and the Contagious Disease Unit (CDU) are the jail facilities that house a significant number of incarcerated individuals with advanced age, complex medical problems, disabilities, and/or enhanced nursing needs. The Senior CL Psychiatrist working in this facility will lead the team in diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for individuals housed in this facility with comorbid mental illness, including direct patient care as well as supervision of other psychiatric providers. The Senior CL Psychiatrist will also consult on the care of medically complex patients in other jail facilities. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for elderly and medically complex patients with comorbid mental illness., • Directly treat and oversee the psychiatric care of additional patients housed in NIC/WF/CDU, • Direct patients to the appropriate level of mental health care, within the facility or in the hospital as needed., • Close communication with psychiatric and medical providers at Bellevue Hospital to ensure continuity of care as patients transition between the two facilities., • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief and/or Senior Clinical Supervisor, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both in vivo reviews of clinical skills as well as chart review., • Close collaboration, including regularly scheduled meetings, with other relevant disciplines within the mental health, medical, nursing and operations departments, as well as the Department of Correction, • Ensure that medication renewals are completed in a timely manner, • Educate clinical staff about new developments regarding best practices in all aspects of psychiatric care, with specific attention to the challenges and issues facing a jail setting (e.g., influence of legal case, dual loyalty, medication diversion), • Ensure compliance with all regulatory standards, • In collaboration with the Mental Health Unit Chief, oversee the orientation of new staff to the facility or program, • Conduct regular and multi-disciplinary case conferences, • Report on an as-needed basis to the CHS Morbidity and Mortality review committee., • Work in collaboration with the Mental Health Unit Chief to manage clinical schedules, vacation requests, CME activities and other relevant activities as they pertain to psychiatric staffing and education, • Participate in policy revision and creation, and practice guideline creations, related to psychiatric care, including input into the psychiatric medication formulary, • Lead at least bi-weekly interdisciplinary mental health staff meetings in coordination with the Mental Health Unit Chief and Clinical Supervisor., • Personnel management of clinical staff, including setting quality standards, establishing effective recruitment and retention strategies, and managing workforce complaints, • At least 50% of time devoted to direct clinical care, which may include direct supervision of psychiatric trainees., • On-call responsibilities for 2 weeks at a time, one to times/year, for off-hours issues that require high level clinical intervention, • Other duties at the discretion of the Medical Director of Mental Health or the Chief of Service. Department Preferences: • Excellent diagnostic and treatment skills with particular attention to issues of psychopharmacology, safe administration of medication in a variety of settings, medical complications of mental illness and therapeutic resistance, • Excellent interpersonal communication skills and ability to work collaboratively with other disciplines, including DOC staff, • Strong leadership skills with ability to identify staff needs and provide mentorship and education to suit those needs, • Advanced knowledge of psychopharmacology in elderly and medically complex patients, • Experience working with medically ill psychiatric patients, • Buprenorphine waiver training highly encouraged but not required Qualification Requirements: Education • M.D. or D.O. degree with active NY State license in good standing, • Completion of a 4-year psychiatric residency training program, • At least two years of post-residency experience working with individuals with mental illness in a criminal justice setting (jail, prison, probation and/or parole) or public health community setting, • At least one year of post-residency experience in a supervisory role in psychiatry, • At least one year of post-residency experience working with individuals with serious mental illness, • An equivalent and satisfactory combination of education and experience may be considered.

Ensure that all monthly reports are typed, submitted and routed on time to the appropriate person., • Provides reception services: receives phone calls and visitors with courtesy and professionalism, takes accurate phone messages, directs incoming and outgoing calls and faxes in a timely and accurate manner., • Cleans and maintains equipment and resolves routine problems with equipment and systems., • Develops and revises work methods as needed., • Maintains inventory and requisitions office supplies., • May be required to take minutes at meetings and produce a final typed copy for distribution to appropriate individuals either by hard-copy or electronically., • Designs, sets up and maintains electronic and hard-copy files of correspondence/documents, codes and files material, searches for requested material and periodically purges obsolete material for storage in records center or for appropriate disposal., • Types and distributes meeting notes, correspondence and reports. Organizes the flow of paper in the office., • Greets visitors, recipients, answers telephones, responds to standard inquiries concerning program activities and procedures, and refers more technical questions to appropriate staff., • Develops, maintains and updates program manuals and information., • Transmits instructions from supervisor to appropriate staff orally, in writing or electronically and follows-up to see that instructions are followed and deadlines are met., • Review and update Medicaid information in CORE for monthly billing., • Reviews and confirms that appropriate billing documentation has been obtained., • Secures medical documentation required by program for billing ex. Physician authorization., • Runs assigned daily billing reports to determine account needs and updates files with current billing information., • Maintains knowledge of Medicaid and Medicare guidelines and applies that knowledge to billing as required by individual programs., • Understands Medicaid/Medicare and Insurance billing including coordination of benefits., • Works collaboratively with clinical staff and other staff to ensure that correct diagnosis/procedures are reported to Medicaid/Medicare and third party insurance carriers., • Monitors electronic transmission of claims to avoid delay in processing. Check and balance all claims/services were loaded and file was received., • Identifies monitors and corrects errors made on electronic and paper claims and follows up until service is properly documented and resubmit claims., • Works with Supervisor to maintain accuracy of reference materials and implementation of new billing codes and fee schedules., • Multi-tasks in various computer applications to gather billing and collection information., • Works unbilled revenue reports to verify pending claims and bring them closure., • Ability to become proficient with EHR software., • Knowledge of Medicare, Medicaid and Managed Care guidelines., • Ability to establish effective working relationships with a variety of people at all levels of the organization., • Knowledge of office and secretarial practices and procedures., • Knowledge of proper grammatical usage, punctuation and spelling., • Knowledge of agency policies and program policies, procedures and functions., • Ability to use office equipment (e.g. computer, fax, copy machine, scanner, etc.)., • Ability to organize office operations., • Ability to produce letters, tables, documents, etc., using an alphanumeric keyboard., • Ability to design and maintain filing systems., • Ability to resign forms for the collection of information., • Ability to determine priorities., • Ability to maintain program accounts., • Ability to interact with consumers in a respectful and professional manner., • Knowledge of agency procedures, formats and guidelines for the receipt, processing and distribution of correspondence, forms, memoranda, documents and other material., • Ability to use and maintain office equipment and systems., • Ability to manipulate an alphanumeric keyboard (computer and typewriter) to produce documents, such as letters, memos, reports, charts and forms, with a high degree of accuracy and within required time frames., • Ability to format, use and prepare forms, charts, tabular listings and other documents., • Ability to organize materials to establish and maintain electronic and hard-copy files., • Ability to understand and use basic office reference material, such as equipment manuals, procedure manuals and dictionaries., • Ability to proofread and correct errors., • Ability to communicate basic information clearly and politely by telephone or in person., • Ability to report for work as scheduled on a consistent basis., • Ability to understand and adhere to internal and external laws, rules and policies. High School Diploma or GED plus at least two years of clerical or secretarial experience, or graduate of secretarial school. Preferably education should include coursework in bookkeeping/business. AA degree preferred but not required. Post-high school courses in Medicare/Medicaid billing, data processing, medical terminology or Accounting highly desirable. At least one year of Medical Billing or other insurance billing preferred. Must be able to satisfactorily use computer work processing equipment and programs (Microsoft Word) and type letters, reports, forms and other documents.
