Facilitate patient education on disease management, lifestyle modification, and medication adherence, and ensure smooth transitions between inpatient and outpatient settings., • Educate patients and their families on disease processes, treatment options, risk factor modification (diet, exercise, smoking cessation), and self-management strategies to promote optimal health outcomes and prevent disease progression.
