Job Title: Licensed Practical Nurse (LPN) Location: Brooklyn, NY 11213 Schedule: All Shifts Needed, 37.5 hours/week (7.5 hours/day) Position Summary: The Licensed Practical Nurse (LPN) provides direct nursing care to acutely ill adult patients (18 years and older) under the supervision and direction of a Registered Nurse (RN). The LPN collaborates with the healthcare team to meet the identified needs of patients and assists in the delivery of care within the framework of the nursing process. Key Responsibilities: • Collect and evaluate patient data regarding physical status; assist in completing patient admissions., • Identify common patient problems, symptoms, and behavioral changes., • Assist RN with patient discharge, transfer, and orientation of new patients., • Ensure availability and proper functioning of medical equipment; perform testing as per policy., • Observe, measure, record, and report patient health status., • Perform clinical procedures such as urinary catheterization, oral or tracheal suctioning, sterile dressing changes, and peripheral IV insertion (with additional training)., • Reinforce patient education, health teaching, and counseling to patients and families., • Guide and support unlicensed nursing personnel in performing appropriate tasks., • Report changes in patient condition (physical, emotional, behavioral) to RN and/or physician., • Assist in identifying patient goals for care planning., • Administer most medications, immunizations, and blood (with additional training)., • Complete occurrence reports for any deviations from standard care., • Successful completion of an accredited Licensed Practical Nurse program., • Active New York State Practical Nurse License., • Strong verbal and written communication skills; ability to communicate effectively in English., • Basic computer and word processing skills., • Ability to use verbal and nonverbal techniques to prevent crisis situations. Involves standing, walking, sitting, lifting, and talking.
