We're hiring! We are hiring a full-time Community Health Worker to join us onsite in our clinic in Upper East Side, NYC. This is an opportunity to join an interdisciplinary team committed to providing personalized, compassionate and high-quality SMI care. The Opportunity: We are seeking a Community Health Worker to join our interdisciplinary team at our Upper East Side Clinic and support patients throughout their care journey. Reporting to the Practice Operations Manager and working in collaboration with Psychiatrists, Primary Care Providers, Therapists, Dietitians, and Clinic Care Coordinators, this role will involve working directly with patients to address concerns, answer questions, and provide guidance and information. The goal is to connect patients with appropriate resources and community support. This position plays a key role in bridging gaps in care by meeting patients where they are, whether in the hospital, in the community, or occasionally in their homes—to ensure they receive the services they need. What You Will Work On: Patient Support & Advocacy: • Represent Amae Health in the ER and inpatient hospital setting with our academic medical center partners to coordinate patient transitions to Amae, ensuring continuity of care as patients begin their care journey with Amae Health, • Support the Amae patient journey through conducting thorough patient screenings either in the hospital, the community or in the home, • Function as a patient advocate, supporting access to necessary health and social services A true patient advocate, you excel in using your knowledge of New York City's health systems and services to connect patients to the most appropriate resources in order to support their care journeys. Strong communication skills and an inherent ability to build trust are your hallmarks, and you are passionate about initiating, establishing and maintaining relationships with patients and families that are founded on trust and rapport. You are adaptable, flexible and organized, people centered, and excited about the opportunity to be part of a team that is changing how SMI care is delivered in our community.
