As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do: • Provide in house physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury, • Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals, • Observe movements, conduct assessments, document progress and modify care plan as needed, • Assist patient with exercises and hands-on therapy to manage pain and increase mobility, • Educate patients on their plan of care, established goals, exercises and self-care, • Graduate of an accredited Physical Therapist Program and active PT license in NJ., • Knowledge of state and federal home health regulations Founded in 1966, Interim HealthCare is the nation’s first home care company and a leading employer of Home Health Physical Therapists (PTs).
