    Position Summary The Care Manager provides services within the Care Management programs, including Health Home Care Comprehensive Care Management, HCBS Basic Plan Support, and State Paid Care Management services. The Care Manager works with the member, their family and/or representative, and providers to develop, implement, and monitor an integrated and person-centered driven Life Plan, following the completion of a comprehensive assessment process. The Life Plan is the foundation upon which service delivery is built. The Life Plan identifies services that meet medical and behavioral health needs, community, social supports, and other necessary services to support them to live their healthiest and most meaningful life. A key function of this role is being a strong advocate in supporting the member to access needed services to reach their identified goals and live a meaningful and quality life. ACA/NY is a 501(c)(3) not for profit organization that has been designated as a Care Coordination Organization/Health Home (CCO/HH) by New York State. Duties & Responsibilities • Deliver person-centered care management services in compliance with regulatory standards and in alignment with the agency’s quality management plan, policies, and standard operating procedures., • Responsible for the completion of a comprehensive assessment/reassessment process., • Identify gaps in service provision and make referrals when appropriate. Provides regular communication, monitoring, and action oriented follow up on critical and acute healthcare needs., • Identifies, coordinates, and provides access to preventative and health promotion services as needed., • Coordinates transitional care inclusive of appropriate follow up from inpatient to other settings, discharge planning, facilitating transfers within the healthcare system, residential settings and aging out of childhood services to adult services., • Use health information technology in the delivery of care management services, included but not limited to the use of the electronic health records and programs to facilitate telehealth services for members.     Program Overview: The Support and Connection Center provides 24/7 short stay, engagement, stabilization, and treatment services for adults with mental health and/or substance use issues referred directly by NYPD as an alternative to the traditional police responses of arrest, summons, or transport to an emergency room. Guests are referred to the Center when their infractions are non-violent and/or low-level offenses such as loitering, trespass, public inebriation, and disorderly conduct. The goal of the Center is to help guests begin to identify and modify the behaviors that result in repeated police contact. An OASAS licensed outpatient clinic, located on site, provides services to guests who are eligible. The maximum length of stay is 5 days. Overall Responsibilities: Under the general direction of the Assistant Program Director, the Clinical Team Lead conducts rapid assessments, engages guests in wellness, discharge, and aftercare planning, and facilitates linkages to community-based services and residential settings. The team lead also oversees a team of CASAC's and Peers and is responsible for the delegation of tasks to the team.     CEDC CANNABIS STAFF ATTORNEY The Community & Economic Development Clinic (CEDC) at CUNY School of Law seeks applicants for the position of Staff Attorney. The CEDC Cannabis Staff Attorney will work to advance the CEDC’s mission of advocating for racial, economic, and social justice by providing legal advice, educational services, and other assistance to social & economic equity & CAURD cannabis licensees and applicants in a variety of areas, including incorporation, governance, application support, compliance (with corporate, employment, tax, cannabis and other laws), and real estate, zoning/land use, and lease issues. The Staff Attorney will also provide educational opportunities via workshops, trainings, and materials development. The Staff Attorney will also participate in relevant coalition work, including policy development, and support relevant grant-related responsibilities. The position offers a unique opportunity to provide pro bono legal services to the emerging cannabis industry in New York State. Development Associate Responsibilities and Duties: • In collaboration with the Deputy Director, support core fundraising activities including: membership, individual giving, special events, sponsorship, and grant writing, • Manage donor research, tracking, acknowledgments, and correspondence through Salesforce, • Accurately maintain organizational systems and fundraising data to support accounting processes like financial transactions for the monthly close, • Draft and design content for donor materials, including appeal letters and corporate pitches, • Support logistics and production for donor events, including cultivation and community events, the Open City Benefit in the spring, and the OHNY Weekend Launch Party in the fall, • Additional administrative and operational duties as needed Development Associate Qualifications and Skills: • Track record of successful donor engagement, from prospect research to solicitation to relationship stewardship, • Event production experience, • Aptitude for problem-solving and the ability to manage multiple projects at once, and work independently, • Must be adaptable and highly organized with exceptional attention to detail, • Strong written and verbal communication skills, • Ability to initiate and maintain relationships with a wide variety of stakeholders, including Board members, donors, members, and program partners, • Fluency with Microsoft Office, CRM, and CMS platforms (databases like Salesforce preferred), and project management platforms (Asana, Slack, etc.), • Curiosity for New York City and the built environment (architecture, planning, real estate, infrastructure), • Commitment to the mission and values of Open House New York, • 3+ years of experience in fundraising required, Bachelor’s or commensurate experience Education Bachelor’s Degree or commensurate experience. Location and Hours We currently work on a hybrid schedule. The Development Associate will work three days per week (typically Tuesday-Thursday) in our office at 150 Varick Street, though this schedule may shift and evolve, particularly in the run-up to major events. Ability to attend events in the evening and some weekends is required. Note: OHNY Weekend is October 16-18, 2026. Salary and Benefits Salary is commensurate with experience, in the range of $64,000—$70,000. Full benefits provided, including health insurance, retirement contribution, and pre-tax flexible spending accounts.
