Job Description JOB SUMMARY The Case Manager will support and administratively/therapeutically engage the residents progress toward independence, housing, coordination and collaborating services. Perform Independent Living Plans (ILPs), psychosocial and progress notes, documenting resident health, wellbeing and care while in BAVR. Case Manager prime responsibility is organizing comprehensive service planning and assessment activity and assuring service plan implementation through direct case management or co-provider services. The Case Manager will also have either primary or shared responsibility, depending upon the specialty/discipline, in providing the residents with specific linkages/advocacy services with co-providers of housing, mental health, substance abuse, and healthcare services etc. as well as with parole boards. (WSFSSH) is a community‐based organization whose mission is to provide safe, affordable housing with supportive services within a residential setting which enhances the independence and dignity of each person. WSFSSH develops and manages housing and provides social services for low‐income older persons, many of whom live with mental illness and/or have experienced homelessness. Our 440 staff members serve more than 2600 residents in 30 buildings located throughout Manhattan and the Bronx. About WSFSSH’s Wellness Initiative WSFSSH’s Wellness Initiative seeks to create a culture of wellness within our residential developments for older adults to facilitate the adoption of healthy habits that will improve their overall physical, emotional, and mental well-being. We aim to identify, develop and implement best practices to improve health outcomes for older adults, including and particularly for those with histories of homelessness, mental illness and/or substance abuse. Compass enrolls approximately 350 students in pre-kindergarten through fifth grade. THE OPPORTUNITY Compass Charter School is seeking reflective physical education instructors who are driven to provide a joyful, rigorous, and justice-oriented educational experience to all children. Compass Physical Education Instructors are highly reflective, collaborative, curious, and bring joy and knowledge of physical education to their work with students and colleagues. Physical Education Instructors lead physical education classes for students in grades K-5 and collaborate with Classroom Co-Teachers to ensure Classroom Co-Teachers are able to implement skills and activities into their practice. As part of our continuum of services we operate an emergency housing program for homeless youth in NYC, Streetwork Overnight. The Overnight is a 24 bed facility for youth ages 16-20. It is open from 7 pm to 10 am 7 days a week. Casual Youth Advocates work overnight hours to support the youth while they are in our crisis shelter. As part of our continuum of services, we also provide overnight outreach in teams of two seven nights a week (7 pm to 3 am), throughout the five boroughs of New York City. Casual Youth Advocates work overnight hours to meet the most disenfranchised youth while they are in crisis. Casual Youth Advocates work part-time and on-call, filling in for full-time youth advocate absences at the Overnight and Outreach program. Operates and monitors systems for the conveyance of medical gases such as medical air, medical vacuum, oxygen, nitrogen, carbon dioxide, etc. Records data such as hours of equipment operation, temperature and pressure, fuel consumption, equipment status and change of status all in accordance with existing regulations and safety procedures. Records same on round sheets, logbooks, computerized entries, work order systems and reports of any type as the need arises. Coordinates work assignments and supervises other plant staff such as maintenance mechanics, plumbers, electricians, maintenance workers, trades helpers, etc. Resolves work priorities in absence of specific instructions from Chief Watch Engineer and inspects all work for quality, quantity and completeness. Establishes and maintains professional relationships with outside service contractors responsible for repairs and maintenance of Plant Systems. Interfaces as needed to ensure that assigned outside work is performed safely and efficiently. Operates plant systems as needed to facilitate the completion of repairs and maintenance work by outside contractors including starting, stopping and testing of operating parameters of Central Plant Systems. Maintains and follows all established safety and security procedures as established by Plant Manager, Facilities Management and Mount Sinai Environmental Health and Safety. Qualifications: Minimum High School Diploma or GED required Ability to operate Refrigeration, HVAC, and Fire Protection equipment as needed. 3 – 5 years progressive experience as a Watch Engineer in a high-capacity Power/Energy Plant.
