Job Description Team Supervisor- Adult Care Coordination Program For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. Catholic Charities provides individuals with serious mental illness, complex medical needs and substance abuse issues with comprehensive care coordination and case management services. RESPONSIBILITIES: The Health Home Team Supervisor oversees a team of direct care staff charged with coordinating all aspects of care for assigned clients. In addition to overseeing client-centered care, the Team Supervisor has additional goals of reducing avoidable health care costs, specifically preventable hospital admissions/readmissions and avoidable emergency room visits, and improving client’s health outcomes by addressing primary medical, specialist and behavioral health care through contractual arrangements with appropriate service providers. The Team Supervisor is a driving force behind comprehensive, integrated services, ensuring direct care staff conduct a comprehensive assessment of clients and facilitate access to the full range of medical, behavioral health, substance use, social and psychosocial services in the community, in a timely, efficient, and effective manner. Clients are provided care coordination services in their home and in the community. The program applies a person-centered and recovery-oriented approach to care, empowering the individual to be active participants in their healthcare journey. The Team Supervisor also maintains health home networks with primary medical and specialty practitioners and mental health providers, substance abuse service providers, community-based organizations, managed care plans, emergency rooms, hospitals, and residential/rehabilitation settings to ensure coordinated, and safe transitions of in care for clients who require transfer to/from various levels of care. The Team Supervisor monitors daily activities of staff, ensuring the scope, frequency and duration of client encounters are adequate to meet client needs, and substantiate monthly billing of core services. This includes oversight of progress notes documentation, assessments, Care Plans, and billing tools through regular chart reviews and staff supervisions. Job Description JOB DEFINITION: Provide administrative supervision to a dynamic team of residential staff providing housing and services to people with psychiatric disabilities. Responsible for the overall functioning of the programs and ensuring compliance with agency policies and procedures as well as applicable local, state and federal regulations. EDUCATIONAL REQUIREMENTS: Minimum of Bachelor's degree in human services, Master's Degree preferred. EXPERIENCE REQUIRED: Two years supervisory experience in a mental health related field. DUTIES AND RESPONSIBILITIES: • Oversee and supervise all aspects of assigned residential programs and assure that appropriate and necessary services are delivered., • Ensure all program guidelines, protocols, policies, procedures and regulations are followed., • Participate in the recruitment, hiring, training and ongoing evaluation of residential staff under the supervision of the Asst. Program Managers are to review the medication logs/EMAR on at least a monthly basis to ensure all procedures are being followed appropriately., • Monitor occupancy rates and ensure maximum occupancy in cooperation with the management team., • Conduct intakes for potential residents in cooperation with the admissions committee., • Ensure that appropriate discharge planning activities occur for all residents., • Participate in the development, implementation and following of all program policies and procedures in conjunction with the Assistant Director and Director., • Responsible for routine (at least bi-monthly) monitoring of all client records to ensure that records are up to date and include all required documentation in compliance with applicable individual program regulations and policy manuals., • Assist housing supervisors in maintaining consistent and professional collateral contacts to ensure that all program participants are receiving coordinated and adequate services. This includes participation in case conferences when necessary., • Oversight of the sites to ensure a clean, safe and homelike environment. Our Queens ACT team provides an integrated set of evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multidisciplinary mental health treatment team. ACT supports client recovery through a highly individualized approach that provides clients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. The Substance Use Specialist is part of this multidisciplinary team and is responsible for delivering intensive in-home and community-based services to individuals living with severe mental illness, whom have a treatment history characterized by frequent use of psychiatric hospitalization and emergency rooms, involvement with the criminal justice system, alcohol/substance abuse, and lack of engagement in traditional outpatient services. RESPONSIBILITIES: The Substance Use Specialist will utilize a person-centered planning process to provide rehabilitation, case management and support services to individuals served by the ACT Team. The Substance Use Specialist will lead the team’s integrated dual diagnosis treatment (IDDT) with the tasks of other team members. Job Description JOB SUMMARY: Under the general supervision of the Team Leader/Program Director and the Behavioral Health Specialist I, the Behavioral Health Specialist II functions as part of a multi-disciplinary treatment team.
