Job Description Nurse Community Health Care Managers collaborate directly with patients, families, local providers, and community organizations to deliver clinical support, evaluate treatment plans, and help prevent gaps in care and duplication of services for Medicaid members. Nurse Community Health Care Managers work to improve member/patient outcomes through goal setting and comprehensive care planning. Additionally, this role plays a vital part in lowering costs linked to high healthcare utilization by promoting a patient-centered approach, supporting continuity of care, and empowering members/patients to actively engage in self-management of their health. Nurse Community Health Care Managers report to Care Management Supervisors with clinical oversight provided by the CMO. Knowledge / Skills / Abilities • Strong clinical skills, • Case Management Experience preferred., • Skills to communicate and interact appropriately and respectfully across multi-cultural differences., • Critical Thinking and strong decision-making skills., • Strong verbal communication skills (both over the phone and in-person)., • Basic Business writing skills., • Strong customer service skills., • Ability to multi-task, prioritize, and ability to handle multiple priorities while meeting deadlines., • Experience working with EMRs preferred., • Experience partnering with local and regional health and human service resources preferred (not required)., • Experience working with Medicaid and Medicare preferred (not required)., • Strict Confidentiality required., • Bilingual preferred (not required)., • Reliable transportation and a valid driver license and insurance Education and Formal Training · Graduate of a Nursing Program · Duly certified and licensed as a Registered Nurse or Licensed Practical Nurse to practice Medicine in the State of Colorado · Current CPR Certification Job Duties: Assists Community Health Care Managers in the clinical component of care coordination for members/patients. • Develop individualized interventions for members/patients., • Enter all documentation, contacts, and assessments into multiple databases with a high level of accuracy., • Participate in health promotion and education activities for members and staff., • Attend member appointments with providers and community resources as needed., • Understand, communicate, and facilitate member’s complaints, grievances, and the appeal process., • Regular case reviews with supervisor., • Optimize member and family independence through education, community resources, and support.
