This role involves evaluating patients' medical histories, coordinating care with the interdisciplinary team, and facilitating a smooth admission process. The nurse will also provide education and support to patients and their families regarding hospice services. Primary Responsibilities: • Conduct comprehensive assessments of patients referred for hospice, addressing physical, emotional, and spiritual needs., • Collaborate with physicians and the interdisciplinary team to develop and adjust individualized care plans., • Educate and support patients and families during the hospice admission process, ensuring understanding and compassion., • Complete required documentation accurately and promptly., • Ensure all medical and administrative admission requirements are met., • Participate in team meetings, case conferences, and ongoing training., • Perform regular hospice visits, including death visits, and participate in the on-call rotation., • Stay current with hospice best practices, policies, and regulatory standards., • Perform other related duties as assigned. Qualifications: • Education & License: Active RN license required; hospice/palliative care certification (e.g., CHPN) a plus., • Experience in hospice or palliative care settings is preferred, along with a strong understanding of end-of-life care principles and best practices., • Strong clinical assessment, communication, and interpersonal skills with a compassionate, patient-centered approach., • Certifications: BLS required; ACLS preferred (based on care setting)., • Technical Proficiency: Proficient with EHR systems and healthcare technologies., • Other Requirements: Ability to work independently, respond to crises, and adapt to diverse social, cultural, and religious backgrounds. Must have reliable transportation, a valid drivers license, and auto liability insurance.
