Job Description About the Role The Trauma Recovery Center (TRC) of Rising Ground understands the importance of removing barriers to care and providing trauma-informed services. Through this dedication, the TRC aims to transform services for survivors of violent crime. Rising Ground’s TRC is designed to help victims of violent crimes overcome barriers to accessing mental health treatment, health care, and legal resources in the acute aftermath of trauma. The TRC supports recent victims of violent crime, including survivors of physical assault, sexual assault, gunshot wounds, stabbings, domestic violence and human trafficking. The Trauma Recovery Center facilitates healing for survivors of trauma, violence, and loss through an innovative model that blends comprehensive care, advocacy and outreach. The Clinical Social Worker/Therapist of the Trauma Recovery Center (TRC) will provide clinical intakes, assessments, planning and implementing individual and group psychotherapy, assertive outreach, and clinical case management services, using evidence-based practices that are effective for the service population. The ideal candidate will bring a passion for social justice and anti-oppressive practice, ample experience providing clinical services to survivors of trauma, and a desire to work collaboratively with a team of creative and committed advocates. All candidates must have a strong understanding of trauma-based work and have experience providing crisis intervention and emotional support to trauma survivors. The ideal candidate recognizes that “the most harmed are the least helped,” understands the importance of removing barriers to care, and has a passion to increase accessibility of services to survivors of violent crimes. What You’ll Do Clinical Intervention, Consultation, Psychotherapy, Liaison, Outreach and Case Management: • Conducts clinical intakes of new clients using a trauma-informed approach and identifies service needs when appropriate. Assessment includes identifying presenting problems, psychosocial history, mental status exam, and DSM-5 diagnosis., • Clinical interventions include crisis intervention and emotional support to clients., • Clinical services also include short-term, trauma-informed individual and group psychotherapy as well as comprehensive clinical case management, liaison with other health care providers, and referral and accompaniment to community services when appropriate., • May visit patient at home and/or in the community for the purpose of outreach and follow-up if the patient is unable to keep regularly scheduled appointments., • Caseload consists of complex clients, all of whom have histories of trauma, including acute and chronic physical and sexual abuse, and are victims of interpersonal violence such as sexual assault, physical assault, domestic violence, gunshot wounds, political torture, and immigration trauma. Position Summary: PHS is hiring 2 full-time Community Health Workers – Harlem Health Advocates (CHW-HHA) to conduct health coaching, wellness activities, outreach, engagement, and advocacy in East and Central Harlem for the Harlem Health Advocacy Partners (HHAP) program. Funded by the New York City Department of Health and Mental Hygiene (NYC DOHMH), the Harlem Health Advocacy Partners (HHAP) program aims to improve the health of NYC residents by supporting access to health and social services to which they are entitled, providing health coaching to individuals to increase self-efficacy for healthy behavioral changes and disease management, building community capacity to seek and/or create healthy conditions and acceptable services through advocacy to government and other service providers, and providing Group Wellness Activities and high-quality educational workshops. The CHW-HHA works independently with clients to do outreach, engagement, case management, and follow up, as well as closely with the HHAP team and supervisor to share experiences, identify unresolved issues, share best practices, and participate in problem-solving. They will work under the supervision of a CHW-HHA Supervisor, and work closely with other members of the HHAP team, with interaction with other teams within PHS’ Neighborhood Health Division as needed. The SWF program will serve a total of 65 families residing in Brooklyn. POSITION SUMMARY: The Therapeutic Case Manager is responsible for screenings and assessment, engagement, safety planning and coordination of services and treatment for members of the family system experiencing IPV. Approach to service is family-centered, trauma-informed, strengths-based and emphasizes both short-term and long-term goals to restore and maintain Safety for all family members, Protect children from future exposure to IPV, and promote Healing and Improved Family Relationships.
