About the Role: The Per Diem Administrator on Duty (AOD) is responsible for supporting health, safety, and overall functioning of the residential program during assigned shifts. Your Qualifications • High School Degree required, bachelor’s degree preferred, • At least 5+ years working in a Residential Treatment Center or related service., • A safe driving record as determined by NYS Department of Motor Vehicles, • Excellent communication, interpersonal, and leadership skills, with the ability to work effectively with students, families, school personnel, and external agencies., • Ability to proficiently multitask and successfully balance multiple projects. Bi- lingual in Spanish preferred but not required., • Empathy, flexibility, and a commitment to providing high-quality care and support., • Experience working with children or adolescents with emotional, behavioral, or developmental disabilities in a residential, educational, or therapeutic setting. This position is based out of our Yonkers Residential Treatment Center location for adolescents ages 12-21. Position Type/Expected Hours of Work: This position is in a 24/7 program. The AOD shifts are 7am-3pm/ 3-11pm and 11-7am. The PD may be offered any of these shifts as needed for the program. RTC staff may be required to remain on shift if there is no immediate staff relief available, ensuring continuity of care and maintaining a safe, supportive environment for youth at all times. Flexibility in shift scheduling may be necessary to meet staffing needs. Job Description JOB TITLE: CASE MANAGER DEPARTMENT: SERVICES REPORTS TO: DIRECTOR OF SOCIAL SERVICE FLSA: NON-EXEMPT LOCATION: MANHATTAN, NY SALARY: $40,000-$45,000 SUMMARY: The Case Manager assesses clients’ needs, plans a course of action, links clients to services, and monitors the progress of clients and residents in their program. The Case Manager ensures that clients receive appropriate information and referrals to obtain and maintain housing. DUTIES & RESPONSIBILITIES: • Conduct intake and assessment process to orient client to the program and develop individual service plan reflecting assessment needs and goals., • Meet weekly with the client to assist with the achievement of individual service plan goals., • Coordinate services with Housing Specialist, other staff, and external partners, • Provide advocacy for individual clients around entitlements and services., • Provide crisis intervention, aggressive and supportive counseling to strengthen clients’ ability to make appropriate life decisions., • Document case management activities in accordance with agency and funding source guidelines and procedures., • Complete Housing applications (both written and computer-generated) for clients based on their individual needs and functioning., • Achieve monthly placement quota, as assigned., • Facilitate workshops, groups, and escort clients as applicable., • Attend staff meetings and trainings to enhance skills that are related to the job function., • Must be able to work flexible, alternate schedules, which may include evening and weekend shifts., • Perform other related duties. EXPERIENCE, EDUCATION, AND SPECIAL REQUIREMENTS: Proof of education required. Bachelor's Degree preferred. At least 1 year of case management experience working with the homeless, mentally ill, substance abuse, and/or low-income populations. Bilingual a plus. Strong relationship-building skills required. Ability to work independently, but know when to raise issues with the supervisor. Excellent organizational and time management skills are necessary. Ability to effectively multitask and follow up on a timely basis. Strong PC skills required, including Word and Excel.
