Compensation: $53 - $56 hourly Responsibilities: • Coordinate and manage comprehensive care plans for hospice patients, ensuring their unique needs are met with compassion and precision., • Conduct regular assessments to monitor patient progress and adjust care plans as necessary, fostering a responsive and adaptive care environment., • Collaborate with interdisciplinary teams to deliver holistic care, ensuring seamless communication and coordination among healthcare providers., • Educate patients and their families about hospice care processes, empowering them with knowledge and support during challenging times., • Provide emotional and psychological support to patients and families, helping them navigate the complexities of end-of-life care with empathy and understanding., • Maintain accurate and timely documentation of patient care activities, ensuring compliance with healthcare regulations and standards., • Advocate for patients' rights and preferences, ensuring their voices are heard and respected in all aspects of their care journey.
