Job Description POSITION: Home Study/Post-Release Services Case Manager REPORTS TO: Case Management Supervisor, Regional Supervisor CLASSIFICATION: Full Time STATUS: Exempt SALARY: 55,000-57,000 Position Summary: The “Home Study/Post Release Services” program (“HSPRS”), operated by Lutheran Immigration and Refugee Service (“LIRS”), provides supportive services to unaccompanied immigrant children after release from ORR custody. Reporting to LIRS’s Regional Supervisor and LSSNY’s Case Management/Personnel Supervisor, the HS/PRS Case Manager will conduct home studies, case management and supportive services to unaccompanied immigrant children once they are released to their sponsors in the New York metro area. This position is primarily clinical and consultative in nature and provides services to all supported housing programs (both single- and scatter site) throughout the cluster. Conducts training on clinical subjects, charting, fire safety plan, fire drills, emergency plans and procedures, building security, volunteers and interns, and site-specific exposure control plan., • Manages employee accident reporting and initial submission of workers' compensation claims., • Monitors employee morale and the climate of the work environment to stop or eliminate sexual harassment or any form of employee harassment or employee discrimination. Responsible for taking complaints of sexual harassment, informing the Human Resources Department of the complaint, and investigating the complaint (as directed)., • Monitors employee accrual totals, approves or recommends approval of employee requests for time off, and notifies the Department of Human Resources when an employee should be placed on or taken off Family and Medical Leave (FMLA). Monitors the quality of the rehabilitative services provided to consumers/residents., • Responsible for the development of a program mission statement and program goals that supports the division and agency mission statements and goals., • Coordinates with the Department of Quality Management and the departments in the Administrative Division for staff assistance in the preparation for an audit., • Reviews plans of corrective action, incident reports, and program evaluation reports and addresses areas that need improvement., • Exercises control over budgets through planning, prioritizing of spending, and monitoring of spending patterns., • Approves/disapproves all purchase orders and financial requests from staff and submits approved purchase orders and financial requests to supervisor for approval., • Manages resident/consumer funds program. Develops internal money management procedures., • Educates staff in the benefits of consumer and family involvement, levels of consumer and family involvement, types of consumer and family involvement, nature of consumer and family involvement, and issues of working with consumers and families., • Develops a program strategy for working with consumers and families and develops activities that foster program-consumer and family cooperation and coordination., • Promotes Mental Health and Managed Care programs in local communities; maintains liaison between local elected officials and community boards, educates citizens in the community on mental illness., • Educates staff and residents/consumers on the importance of a constructive relationship with local elected officials, community boards, and community citizens. Job Description Summary This position is primarily responsible for oversight and management of focused outreach, administering regulations, policies, procedures and practices; planning, organizing, developing, implementing and coordinating program activities for the Center of Innovation (LEAP) Scope of Work. The Program Manager reports to the Director of Outpatient Services and supervises the LEAP program team. Essential duties and Responsibilities: Core duties and responsibilities include the following. Other duties may be assigned. • Coordinates and integrates service delivery within the program., • Ensures program compliance with OASAS regulations and program policies directly related to program operations and record management., • Coordinates with Quality Assurance/Data Specialist to ensure accurate data collection and reporting, • Provides community education, coordination of community-based initiatives, and implementing initiatives to promote health and safety of individuals affected by drug and alcohol use., • Develop program services that meet OASAS standards for medically assisted treatment provided through telehealth, in community settings, and existing substance use treatment providers., • Direct planning, coordination, and follow-up of outreach to the community., • Coordinate with the nurse practitioner and medical doctor to ensure MATS treatment compliance., • Develop and manage peer support services., • Develop program protocols to improve service delivery and participant outcomes., • Works collaboratively with the Director of Outpatient Services to ensure effective program operations., • Ensure staff maintains accurate and confidential clinical records that comply with OASAS standards and internal policies., • Manage participant data, statistics and tracking for quality improvement and monthly reporting., • Responsible for the implementation of interventions to address the ongoing expansion of overdose prevention, • Provides crisis interventions and consultation as needed, • Manages and responds to crisis in the field, • Monitors participants’ services, assuring that staff are responsive to participant needs and take appropriate action to make adaptations as necessary., • Evaluates staff performance; identifies documents and develops corrective action plans for personnel performance problems.
