Updates plan at specified intervals, and as needed based on changes in client/member condition or circumstances * Performs and maintains effective care management for assigned caseload of clients/members. Leads the care coordination for complex psychiatric clinical cases. Tracks and monitors progress; maintains detailed, accurate and timely progress notes and other documentation * Provides supportive counseling and/or supportive therapy as well as ongoing mental health services * Collaborates and refers to appropriate agencies as required. Addresses any client/member concerns to ensure satisfaction with overall services provided and uses motivational interviewing techniques to foster behavioral changes * Develops inventory of resources that meet the clients/members needs as identified in the assessment * Provides linkage, coordination with, referral to and follow-up with appropriate service providers and managed care plans. Facilitates periodic case record reviews and case conferences with all providers serving the clients/members * Provides information and assistance through advocacy and education to clients/members and family on availability and eligibility of entitlements and community services. Arranges transportation and accompanies clients/members to appointments as necessary * Assists clients/members and/or families in the development of a sustainable network of community-based supports, utilizing identified strengths and tools designed to prevent future participant crises and/or reduce the negative impact if a crisis does occur * Participates in initial and ongoing trainings as necessary to maintain and enhance clinical and professional skills * Maintains updated case records in program EMR. Overview Provides a broad array of counseling/support services to individuals living with HIV/AIDS, and/or HIV negative, homeless and transgender persons, including persons with serious mental illness (SMI). Shares personal, practical experience, knowledge, and first hand insight to benefit program enrollees. Works under general supervision. What You Will Do • Acts as ‘Health Navigator’ and works with Members to develop and implement an individualized action plan. Speech Language Pathologist (SLP), school based, full or part time RCM Health Care is looking for a Speech Language Pathologists to provide services to school students, in and around Bayonne, NJ. Services must take place in person at the school. Provides a full range of services in the identification and diagnosis of speech, language, communication, and hearing disorders and in the recommendation and delivery of appropriate educational services and programs to eliminate, reduce, or accommodate disabilities that interfere with the student’s meeting and exceeding the New Jersey Student Learning Standards. QUALIFICATIONS: The Speech Language Specialist shall: • Hold a valid New Jersey educational services certificate in accordance with the requirements of N.J.S.A. 18A:27.1 et seq., and N.J.A.C. This role involves maintaining the program vehicle, adhering to schedules, and supporting residents' successful reintegration into the community in addition to ensuring the security of the residence and residents while following establish protocols and procedures approved by NJAC and the NJ Department of Corrections. ESSENTIAL DUTIES AND RESPONSIBILITIES include but are not limited to the following: • Responsible for providing transportation and escorting clients to and from medical appointments, interviews, employment sites and other approved destinations as necessary, • Implement appropriate security procedures in the facility and vehicles. Supportive services include: counseling, support groups, case management, childrens services, referrals for educational/vocational facilities, job preparation and health promotion for women and children. The Program Manager is responsible for the overall daily operations of the program and the safe dwellings. In addition to acting as VIPs primary coordinator to assure that all efforts are fully met and exceed contractual obligations (programmatic and administrative) and direct the day-to-day activities of risk management, safety/security, maintenance and client move-in/turn-over. The SWF program will serve a total of 65 families residing in Brooklyn. Job Description Astor’s Prevention Services/Family Connections is a time-limited, evidence-based model that utilizes a home-based approach to address both the concrete and clinical needs of the family. Prevention services utilize comprehensive family assessment tools to identify behaviors related to risk and to develop Core Outcomes that address the identified child welfare concerns within a family. The Case Planner provides in-home counseling services to families in need of support in developing skill and competencies that will enable them to keep their children in the home and out of placement. The Case Planner will maintain a caseload of 12-14 families and provide change focused interventions with families to improve parental functioning, address life stressors, and improve overall familial functioning. This is happen over a course of 6 to 12 month length of service and a minimum of 2 face to face contacts with each family per month. Qualifications • CASAC-T; CASAC, or higher preferred., • 1 year of experience working with clients with substance abuse disorders., • 1 year of experience working with adolescents with co-occurring and mental health problems., • 1 year of experience working in in an OASAS funded facility a plus, • Commitment to providing quality services and outcome driven performance measures., • Excellent organizational management skills with the ability to coach staff, manage, and develop high-performance clinicians, set and achieve program objectives., • Strong networking and public relations skills with the ability to engage a wide range of stakeholders, populations, and cultures., • Action-oriented, adaptable, and innovative approach to program planning., • Ability to work effectively in collaboration with diverse groups of people. Position Status This is a full-time non-exempt position. Monday/Thursday/Friday-3pm-11pm Saturday/Sunday-4pm-12am Tuesday/Wednesday- Off Work Environment This job operates in a professional office environment and outside in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be able to travel among assigned sites and locations as needed.
