Open Door is seeking a Full-time bilingual English-Spanish speaking Licensed Clinical Social Worker for its Family Medical Center in Sleepy Hollow, NY. The Licensed Clinical Social Worker will report to the Director of Behavioral Health, based in Ossining, NY. The dedicated individual will work closely with the primary health care team to provide integrated behavioral health care to improve the patients’ medical outcomes. The licensed clinician will possess excellent clinical skills, provide short term Behavioral Health care using evidenced-based treatment models with adults, children, and families, and establish and implement clinical treatment goals in collaboration with the integrated health care team. The CMHS will coordinate service delivery to families through a large network of area service providers to help them secure permanent housing. The CMHS is responsible for assisting families/singles who are referred through Coordinated Entry to secure permanent supportive housing units and provide support services to maintain housing, increase income and work toward self-sufficiency. The CMHS performs an assessment of the client's service needs as they relate to the problem of homelessness. In consultation with the family members, the CMHS formalizes and develops a Service Plan. These issues may include areas of finance management, referrals for substance abuse, mental health counseling, parent support assistance, employment assistance, etc. The CMHS is also responsible for completing all documentation required by HUD, including unit inspections. The program director is responsible for oversight of case record reviews, performance improvement, quality improvement and utilization review to include all required monthly reports. KEY ESSENTIAL FUNCTIONS • Responsible for the daily operations and performance of the integrated outpatient programs, • Manages clinic operations to budgeted and planned results, • Interviews, hires, trains, evaluates, manages, counsels and terminates subordinate personnel, • Works closely with staff via weekly supervision to ensure the completion of performance goals. Provides ongoing, regular feedback and performance evaluation to clinicians on the quality and timeliness of their clinical work as well as their caseload. Also solicits feedback from clinicians regarding supervision and whether needs are being met, • Monitor caseloads and ensure the supervisees comply with all agency, clinical, legal and ethical practices., • Oversight and monitoring of billable hours of service required. Provide support, training, and remediation where needed to assist supervisees in meeting program’s billing expectations and caseload requirements, charting, recordkeeping, documentation standards and all other quality assurance standards, • Develops and implements policies and procedures in outpatient programs. POSITION OVERVIEW Under supervision of the Clinical Supervisor the CASAC-T Counselor will provide counseling services in integrated outpatient program. The CASAC-T Counselor provides comprehensive assessment services to clients who may have substance use and mental health barriers to employment by applying clinical judgment to determine appropriate treatment needs and level of care. This includes performing SUD diagnostic screening, assessment, evaluation, orientation, treatment planning, case management, counseling, intervention, crisis intervention, patient education, reporting and record keeping in both individual and group counseling settings. The CASAC-T Counselor is also responsible for referrals and coordination of care in consultation with other professionals. KEY ESSENTIAL FUNCTONS • Intervention and employing a range of treatment modalities to include person centered evidence-based treatment approaches to include but not limited to motivational interviewing, stages of change, trauma informed care, anger management, medication assisted treatment, dialectical behavioral therapy, cognitive behavioral therapy and contingency management., • Ability to perform SUD and mental health screenings and intakes, orientation, SUD assessment, SUD diagnostic Impression, level of care as determined by LOCADTR, treatment planning, individual/group counseling, family interventions, psychoeducation, case management, crisis intervention, client education, referral, reporting and recordkeeping, care coordination and administrative supervision with regular supervision., • Participates in supervision regularly., • Develops and conducts group and individual counseling on a regular scheduled basis to caseload clients within the integrated outpatient program., • Obtain random observed urine samples according to established protocol., • Orients clients about program goals, objectives, rules and regulations, client’s obligations, right’s and program operations, • Knowledge of the obligation to engage in prevention as well as treatment techniques., • Maintains and updates referral services and resources that may be useful to our client population; makes referral when necessary and appropriate., • Ability to demonstrate proficiency in English including the ability to speak, write, comprehend aurally, and read at a level necessary to perform as a CASAC Counselor., • Participates in outreach functions to reengage client lost to contact and maintain census capacity., • Ability to complete all client progress update reports in a professional and timely manner to ensure coordination of care with client linkages to include the criminal justice system and entities., • Participates in program meetings and required trainings., • Ability to complete any reasonable tasks or requests made by a supervisor. REQUIREMENTS • High School Diploma or GED., • Associate Degree preferred., • Valid unrestricted OASAS certification as a CASAC-T., • Minimum of three (3) years working in a Substance Use and Human Service setting., • Must obtain Mandated reporter (2 hours) training/certificate prior to hire date., • Must obtain training/certification within thirty (30) days of hire., • Domestic Violence, • Infection Control BASIC (non-medical/nursing staff), • HIV, • Child Abuse, • Tobacco Cessation, • Must obtain training/certification during in-house orientation., • Supporting Recovery with Medications Addictions Treatment (MAT), • Must obtain SUD training/certificate within thirty (30) days of hire (if applicable), • Screening, Brief Intervention and Referral to Treatment, • CASAC Canon of Ethics (6 hours), • Confidentiality related to 42CFR (3 Hours), • Excellent organizational, communication and time management skills, • Ability to work on a strong team of professionals in a culturally diverse environment., • Ability to multitask and to work and function under pressure.
