Job Description We are seeking a compassionate & dedicated Peer Counselor to join our team at Crisis Residence in Patchogue, NY. In this role, provide a positive role model of community functioning, informal counseling, peer support and ADL training in a licensed crisis residence providing crisis housing to people with psychiatric diagnoses. The Crisis Residence program provides short-term residential support to persons exhibiting symptoms of mental illness who are experiencing a psychiatric crisis. Crisis residence programs provide short-term interventions to individuals experiencing crisis, to address the cause of the crisis and to avert or delay the need for acute psychiatric inpatient hospitalization or emergency room admission. Job Description The VP of Clinical Quality and Excellence will be responsible for the oversight of all Association for Mental Health and Wellness (MHAW) clinically oriented services, residing under current and future programs. This is a high-level management position that requires extensive clinical experience, experience leading quality improvement a belief in and commitment to Person Centered Psychiatric Rehabilitation and Recovery initiatives, which is incorporated into clinical service delivery. The position will require an affinity for strategic thinking and problem solving in the course of day-to-day clinical operations. Responsibilities typical of the position include strategy focused on: best practices, clinical outcomes, quality of services, management of clinical personnel, and, census and revenue growth, regulatory compliance. The VP of Clinical Quality and Excellence reports directly to the CEO and works in collaboration with Programmatic Supervisors and Directors, along with clinicians, Psychiatrist/Nurse Practitioners, and fiscal/billing staff to ensure that all resources in the programs are being used to best serve clients’ needs. Job Description Children’s Care Managers establish supportive, helping relationships with those who have experienced trauma, significant behavioral/physical health and/or substance abuse challenges in order to assist them in achieving improved overall health and social functioning. Children’s Care Managers assist in navigating the complex healthcare/behavioral care system, Department of Social Services, community resources, housing systems and other systems of care. Children’s Care Managers assess socioeconomic and environmental factors and they understand the social determinants of health. Children’s Care Managers are responsible for providing comprehensive and coordinated care as well as linkages and referrals to an array of services. They also serve as advocates, ensuring that their clients receive appropriate care and the attention they require. Children’s Care Managers provide emotional support and encouragement to those served. They implement a strengths-based, trauma-informed approach; offering coordinated care upon assessing their clients’ needs and collaborating in the development of a person-centered care plan. Care Managers reevaluate clients’ eligibility, ongoing needs and appropriateness for services; they support their clients in achieving the established goals until they are ready for graduation from the program. Provides on-site visits to housing locations. • Performs routine assessments of residents' substance use needs., • Provides addiction education and related individual and group counseling tasks to residents., • Assists residents in accessing and utilizing community resources as well as other support services, i.e., D.S.S., medical clinics, sponsorship, etc., • Where appropriate participates in formulation of individualized assessments, service plans and discharge plans for assigned residents and legibly documents them according to specified timeframes and guidelines., • Documents residents' progress and other information in charts in a timely manner and according to specifications., • When appropriate, involves significant others, collateral contacts (family, probation officers, caseworkers, etc.) in education and supportive services., • Communicates pertinent information regarding residents to leadership staff as well as other coworkers to enhance quality of care to residents., • Provides assistance to coworkers and serves as a resource person to non-certified staff, interns and volunteers as needed., • Attend initial recovery meeting (AA, NA, etc.) with residents when needed to provide a linkage to support services., • Serve as liaison to connect residents to detox, inpatient, residential, and/or sober living facilities when needed., • Attend local county SUD and MAT work groups to enhance relationships with community providers., • Other appropriate tasks as assigned by management. We are looking for a highly skilled individual who can assume the role of Children’s Care Management Team Coordinator. Responsibilities combine duties of a professional direct service provider with administrative and supervisory tasks in order to further our mission. The goal of the Children’s Care Management program is to work with children and their families, who have an array of behavioral health and medical conditions, to assist them in accessing medical, behavioral and social services in their communities so that unnecessary hospitalizations and emergency room visits can be avoided. Key Responsibilities will include: • Maintaining mixed caseload and providing comprehensive care management, • Monitor enrollment eligibility & appropriateness, • Collaborative documentation, • Providing oversight to and monitoring productivity of Care Managers, • Conducting ongoing risk assessment for all children on team, • Running and utilizing reports in EHR and other IT platforms, • Auditing/monitoring records to ensure quality and timely completion of documentation, • Providing group and individual supervision to Care Managers on team, • Monitoring staff hours, timesheets, mileage reimbursement requests, • Training and monitoring new staff, • Participating in staff meetings, Team Coordinator meetings and other administrative meetings/trainings as required, • Performing duties with designated amount of time in field and office setting, • Working collaboratively with Program Leadership and Program Director Supervisory Responsibilities: • Facilitate weekly Team Meetings and monthly individual supervision meetings in order to provide clinical and risk assessment guidance and strategize effective interventions., • Address programmatic and procedural changes and provide ongoing clarification and support related to the changes (i.e. monitoring excluded settings, diligent search efforts, eligibility criteria, etc.)., • Utilize reports as a management tool to ensure all client service documentation deadlines are being met (i.e. progress notes, re/assessments). Ensure team members are current with all program reports and documentation (incident reports, hospital TOC, gaps in care/care opportunities, CANS, disenrollment documentation, HCBS & CFTSS applications)., • Understand, develop and update workflows in accordance with DOH and Health Home guidelines, • Track and monitor productivity of team members and formulate strategies to ensure all clients’ needs are being met based on triaging risk/need using tracking spreadsheet, workflows and audit tools., • Monitor new client assignments (new enrolled and enrolled transfers) including ensuring accurate, timely, completion of all pertinent paperwork and documentation, reviewing and signing off on checklists on a regular basis., • To interview potential team members as requested and to monitor and train new team members acting as the program liaison between the program and Human Resources when necessary., • To foster a feeling of team cooperation and cohesiveness and to act as a supportive supervisor and teacher to all team members. Provide guidance/correspondence to team regularly throughout the workday., • Address performance issues as they arise, complete disciplinary action notices when necessary and conducting employee performance appraisals when required., • To address concerns/grievances brought forth by clients, family members and/or providers in a timely fashion and to monitor and follow up as necessary., • To review/approve requests for time off and timesheets within required time frames; monitoring team members’ daily schedules, activities/hours. Review and sign off on mileage reimbursement forms., • To monitor and report any issues with the team’s agency issued equipment (cars, cell phones, laptops, laptop bags) and their usage., • To accompany team members on visits for assessment/crisis intervention or assist with high-risk/needs clients as needed., • To assume a leadership presence in the office and on designated office days and provide clinical and crisis intervention support to all staff as needed. Utilize designated office days to identify and work with team members, individually, who need additional support or require additional training., • To demonstrate an understanding of the electronic Care Management platforms, exhibit computer proficiency and support team in developing their competencies in this area. Required Skills/Abilities: • Strong leadership and supervisory skills including familiarity with an electronic health record, • Excellent written, verbal communication skills, public speaking, • Strong engagement skills and the ability to help the team improve and refine their own skills around engagement, • Two Years’ experience working with children in the target population utilizing a person-centered, trauma-informed approach, • Basic knowledge of the public health/mental health system, public entitlement and social service programs, • Must be a reliable self-starter with the ability to work both independently and as part of a team, • Accuracy and attention to detail are essential, • Ability to work across diverse populations, • Bi-lingual (English/Spanish) candidates are strongly encouraged to apply.
