Job Description POSITION SUMMARY: The Network Support Specialist will work within the Social Care Network (SCN) Team in the Southern Tier region of New York and will be responsible for supporting the Network Performance Specialists in the oversight and provision of enhanced social care services. This position reports to the Network Operations Manager. RESPONSIBILITIES: The responsibilities of the Network Support Specialist will include the following: Essential Functions: • Support the Network Performance Specialists (NPS) and SCN Operations Team in network management activities, including managing referral backlogs and monitoring network capacity to preserve continuity of care for Medicaid member services., • Maintain high understanding of and demonstrated proficiency in SCN programmatic requirements, including care delivery workflows and documentation of services in Unite Us to support the seamless delivery of enhanced health related social needs (HRSN) services., • Review, remediate and approve enhanced service referral requests in the domains of housing, nutrition and non-medical transportation to ensure network adherence to programmatic guidance and invoicing standards, supporting specific priority service domains as needed (ex. housing)., • Collaborate with network providers at the user level, including social care navigators and social care service providers to manage referrals for enhanced health related social need (HRSN) services, and provide technical assistance as needed., • Work with internal cross-functional teams to meet partner needs and ensure positive partner experience., • Monitors and reports on progress and results for current initiatives and ensure that they meet expectations and deadlines. Manages and communicates ongoing changes in tasks, goals and performance., • May assume additional responsibilities, as required., • Bachelor’s degree and at least 2 years equivalent experience in navigation or care management role., • Experience working with/within community-based organizations and social care service delivery in the domains of navigation/care management, housing, nutrition and/or transportation., • Existing familiarity with housing social care services in the 6-county region is preferred but not required., • Exceptional ability to build/maintain collaborative working relationships with internal and external stakeholders., • Proficient in planning, time management and ability to prioritize many tasks in a constantly changing environment., • Strong organizational and communication skills (written and verbal), including the ability to work independently with minimal supervision., • Must have strong personal computer program skills including Microsoft Outlook, Microsoft Office (Word, Excel PowerPoint etc.) Care Compass Network utilizes a hybrid work model where employees are able to work remotely from a home office or from the CCN office, a non-clinical professional office setting. Position Summary: The Ophthalmic Technician ‐ Certified OR is responsible for performing advanced ophthalmic work up, procedures, measurements and tests to provide data to provider and assist in patient care. Assists provider throughout patient exam, office lasers and procedures, and performs as first assists in the operating room (minimum of 12 cases per year). Position requires advanced understanding of ophthalmology outpatient day to day office routine. The Ophthalmic Technician will continually demonstrate Patient Centeredness, Teamwork, and Excellence in the daily performance of their duties. This position requires the ability to be self‐motivated, flexible, punctual, detail oriented, have good time management skills and have excellent communication skills. Education, License & Cert: Certified Ophthalmic Technician (COT) preferred or Certified Ophthalmic Assistant (COA) with two years ophthalmic assisting experience; LPN with completion of home study course within first 6 months of employment. (Licensure in PA and NY required). CPR Certification preferred Experience: Surgical Technician Certified with OR experience may be substituted however completion of home study course within 6 months of employment and Certification as Ophthalmic Assistant is required within the first 12 months of employment. Employee will be paid at one grade lower than Ophthalmic Technician until certification is met. Candidates with clinical experience but no ophthalmic experience will be paid at the C5 pay grade until completion of home study course (3‐6 months); they will move to the C6 pay grade until they have successfully achieved a COA certification which is required within the first 12 months of employment. Candidates with ophthalmic assistant experience of 1 year will be paid at the C6 pay grade, they will be expected to complete home study course within 3 months if they have not already completed and they will move to the C7 paygrade once they achieve a COA certification which is required within the first 12 months. Completion of Ophthalmic Scribe Certification (OSC) is required within the first 3 weeks for those without COA, COT, COMT, LPN. Essential Functions: 1. Obtains and documents accurate chief complaint and comprehensive history of present illness. Reviews, verifies and updates patient’s past ocular, medical, family and social history. Reviews medical records for up‐to‐date master problem list including patient reported medication and allergies and diagnoses that are documented by physicians or providers., 2. Performs a portion of ophthalmic exam, ophthalmic diagnostic testing and procedures; documents findings/results on exam and stays current with new technology and procedures as they become available., 3. Performs as scribe for provider (throughout ophthalmic exam capture, summarize, & document pertinent examination data, clinical findings and the assessment and plan)., 4. Provides patient education, assists provider with informed consent, prepares and reviews surgical charts., 5. Assists provider with laser and surgical procedures within the office, including those that require aseptic and sterile technique., 6. Performs as first assistant in operating room., 7. Responsible for cleaning and sterilization of office instruments, performing basic maintenance, quality control, cleaning, disinfection and maintaining inventory of ophthalmic exam room and ancillary equipment according to organization and departmental protocols., 9. Provides orientation and training of new employees and recognizes and utilizes opportunities to teach other staff members., 10. May travel to regional offices as needed., 11. Attends required staff and educational meetings as requested., 12. Cover as needed throughout the department. Pay Range $22.53-$35.69/hr DOE

Summary: Provides secretarial/administrative support to Internal Medicine Residency Program, with oversight from the Manager of GME Residency Programs (MRP), Program Director (PD)and DIO. The Program Coordinator (PC) is responsible for the operational and financial management of the accredited Residency training program as well as strategic planning support for the programs and institutional improvement efforts. The PC will ensure compliance with the residency specialty specific Accreditation Council for Graduate Medical Education (ACGME) requirements. The position requires a comprehensive and detailed understanding of the national accreditation and hospital policies. The PC will continually assess and reassess and direct a wide range of programmatic issues including long range planning, recruiting trainees, developing projects, analyzing administrative workflow, coordinating and supporting the preparation of didactic work sessions, communicating to all required personnel, and maintain residency related databases. PC will be the point person to serve as a liaison between, GME program manager, Program Director, faculty, resident, students, rotation contacts and other internal and external agencies. Experience: Five years administrative assistant (or related secretarial) experience required. Computer skills and proficiency in multiple software programs including; Microsoft Office, Word, Excel, Power Point and willingness to learn other programs/software that becomes relevant and required for program. Must be able to handle diverse workload, demonstrate strong multitasking capability, exercise solid professional, individual judgement within limits of standard practice. Must have the ability to work well under pressure and maintain composure with a pleasant, positive, professional attitude. Must be motivated, energetic and detail oriented. Individuals must have strong interpersonal skills, be able to work independently, and possess time management skills. Familiarity in hospital or medical setting desired. Education: Two-year degree, associate's degree in secretarial sciences preferred: however, related and significant experience at the administrative may be considered in lieu of a degree. Licenses: No licenses required Essential Functions: 1. Act as lead point person for residency program to manage and coordinate the program as directed by the DME/MRP and PD., 2. Possess through knowledge of policy and procedure guidelines determined by ACGME, RRC, LCME/COCA, institution and specialty board to successfully adhere to the requirements and manage the program accreditation process., 3. Coordinate full scope, recruitment, onboarding and orientation process for program, including the use of ERAS and NRMP. For screening, comparing, processing, and ranking applicants., 4. Provide strategic planning support and development, perform, and monitor S.M.A.R.T. goal driven project work for the program., 5. Support the residency by providing and maintaining accurate, relevant data about specific program requirements and compliance standards in all GME related systems (i.e. ACGME, ERAS, NMRP, WebAds, New Innovations, Specialty Board, FRIEDA, etc.), 6. Direct the workflow and supervise the necessary administrative paperwork and records for the program to ensure compliance and meet accreditation and governance standards., 7. Develop, manage and monitor budget in coordination with the MRP, DME, and PD., 8. Oversight, track, and verify procedure and work hour logs to meet program completion requirements; work hours, conference attendance, EMR Compliance, etc., 9. Coordinate resident filing, data management, seasonal program activities and timeliness, including in-training exams, PEC and CCC milestones, 10. Implement, maintain and refine the resident, faculty and programs evaluations systems. Other Duties: 1. Under the direction of MRP and DME, create and/or maintain external (annual review) program advertising/media through websites, brochures, publications and other social media., 2. Participate in resident wellness events as appropriate, including graduation and wellbeing activities as appropriate to support an environment of appreciation and wellness for all team members., 3. Prepare and maintain resident/faculty schedules including rotation schedules, call schedules, lecture schedules., 4. Be able to occasionally travel for professional development. Position Summary: The Ophthalmic Technician is responsible for performing advanced ophthalmic work up, procedures, measurements and tests to provide data to provider and assist in patient care. Assists provider throughout patient exam, office lasers and procedures, and performs as first assists in the operating room. Position requires good understanding of ophthalmology outpatient day to day office routine. The Ophthalmic Technician will continually demonstrate Patient Centeredness, Teamwork, and Excellence in the daily performance of their duties. This position requires the ability to be self‐motivated, flexible, punctual, detail oriented, have good time management skills and have excellent communication skills. Education, License & Cert: Certified Ophthalmic Technician (COT) preferred or Certified Ophthalmic Assistant (COA) with two years ophthalmic assisting experience; LPN with completion of home study course within first 6 months of employment. (Licensure in PA and NY required). Surgical Technician Certified with OR experience may be substituted however completion of home study course within 6 months of employment and Certification as Ophthalmic Assistant is required within the first 12 months of employment. Employee will be paid at one grade lower than Ophthalmic Technician until certification is met. Completion of Ophthalmic Scribe Certification (OSC) is required within the first 3 weeks for those without COA, COT, COMT, LPN. Essential Functions: 1. Obtains and documents accurate chief complaint and comprehensive history of present illness. Reviews, verifies and updates patient’s past ocular, medical, family and social history. Reviews medical records for up‐to‐date master problem list including patient reported medication and allergies and diagnoses that are documented by physicians or providers., 2. Performs a portion of ophthalmic exam, ophthalmic diagnostic testing and procedures; documents findings/results on exam and stays current with new technology and procedures as they become available., 3. Performs as scribe for provider (throughout ophthalmic exam capture, summarize, & document pertinent examination data, clinical findings and the assessment and plan)., 4. Provides patient education, assists provider with informed consent, prepares and reviews surgical charts., 5. Assists provider with laser and surgical procedures within the office, including those that require aseptic and sterile technique., 6. Performs as first assistant in operating room., 7. Responsible for cleaning and sterilization of office instruments, performing basic maintenance, quality control, cleaning, disinfection and maintaining inventory of ophthalmic exam room and ancillary equipment according to organization and departmental protocols., 9. Provides orientation and training of new employees and recognizes and utilizes opportunities to teach other staff members., 10. May travel to regional offices as needed., 11. Attends required staff and educational meetings as requested., 12. Cover as needed throughout the department. Pay Range $20.44 - $32.39/hour, DOE

Responsible for documentation, tracking and reporting surgical co‐management; completion and processing of forms (DMV, disability, legal blindness, insurance, low vision, etc.); review and monitors provider's schedule for efficiency, errors, and opportunity; triages messages and phone calls and responds to patients as directed by provider. Other Duties: 1. Assists in orientation and training of new employees and recognizes and utilizes opportunities to teach other staff members., 2. May travel to regional offices as needed., 3. Attends required staff and educational meetings as requested., 4. Cover as needed throughout the department.
