Ensure best practices are utilized regarding handoffs for all transitions., • Monitor site financial performance and identify and create new areas for growth and revenue., • Develop an expertise and understanding of the yearly budget, financial performance measures and monitoring systems, and billing and reimbursement issues / systems., • Improve patient census and billing practice statistics to optimize reimbursement for the practice., • Maintain awareness and interactions with payers such as significant IPAs, Medical Groups, Foundations, and ACOs associated with the hospital / health system., • Coordinate efforts with key leaders in central operations. PURPOSE: Care Management is a service that helps children with chronic physical or emotional issues obtain and utilize the medical, social and community services they need to get and stay healthy. Care Coordinators learn all they can about the child and family's unique needs. They work with other care providers to develop a plan of care and help members determine and take the actions needed to make and keep appointments, adhere to medications, obtain educational and health advocacy, and access benefits. POSITION OVERVIEW: Care Coordinators link children who have chronic medical or behavioral health conditions to the services they need to stay as healthy as possible and avoid unnecessary trips to the emergency room. Care coordinators encourage the clients (members) they serve to use those services to optimize their health outcomes. Our children's community residences are single site residence's that provide a group living for eight children and/or adolescents. The program provides a supervised, trauma informed therapeutic environment which seeks to develop the resident's skills and capacity to live in the community and attend school/work as appropriate. POSITION OVERVIEW: The Case Manager works with children and adolescents in the community residence and is responsible for the supervision, care and skill building related to the youth's individual service and treatment plans.
