Job Description Job Summary This position provides administrative support in organizing and implementing case management, groups, clinical, and/or reproductive health support services for all women referred to the Strong Mothers program. The coordinator will support leadership in maintaining, quantitative and qualitative data for reporting to grantors and the Director of Quality and Performance Management. In addition, the coordinator will support front line program operations conducting individual/group interventions, as determined by program leadership. Major Responsibilities/Activities • Serves as a liaison between the Program, agency and community partners., • Provides information to prospective participants on the service programs, registration requirements, policies, and procedures., • Timely response to telephone, e-mail, and other inquiries requesting information on the program services., • Arrange intake interviews, maintain and computes intake data and generates enrollment reports., • Collect and prepare Continuous Quality Improvement (CQI) monthly reports for data in ie. Triage, referrals, grants, assessments, registration and cohorts., • Schedule at a minimum of 10-15 virtual intake appointments per week., • Coordinate/Schedule quarterly group cohorts, as designated by leadership., • Facilitate educational workshops for participants in Strong Mothers i.e. Parenting, Family Planning., • Link clients with resources in the community, including health, educational and vocational resources., • Provides support for event planning, production of graduate certificates, letters to providers, and presentations., • Maintains accurate and complete files on programs and participants., • Timely communicates to participants and staff of any operational changes such as classes postponed, canceled, changed, instructor absences, etc., • Fluency in Spanish a must., • Bachelor's degree in Social Work or related field required., • At least 2 years of experience in administrative assistance and service delivery, preferably with families and children., • Experience with grant data management and reporting., • Organized, efficient, manage time well, capable of multitasking.
