Job DescriptionTravel Physical Therapist (PT) – Home Health (PACE Program) Bordentown, New Jersey Facility: Capital Health LIFE Program Contract Duration: 13 Weeks (March 9, 2026 – June 6, 2026) Estimated Weekly Pay: $3,300.12 Schedule: Monday – Friday | 8:00 AM – 4:30 PM | 40 Hours Per Week Coverage Area: Mercer County & North Burlington County, New Jersey Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for the Capital Health LIFE (Living Independence for the Elderly) Program, a PACE program designed to support elderly patients living independently in the community. In this role, the Physical Therapist will provide in-home rehabilitation services for elderly participants, focusing on improving mobility, functional independence, and quality of life. The therapist will work closely with an interdisciplinary care team including physicians, nurses, and case managers. This position offers a consistent weekday schedule with no on-call duties or weekend work, making it an excellent opportunity for therapists interested in geriatric care and home-based therapy. Program Overview The LIFE (Living Independence for the Elderly) program helps seniors maintain independence by providing comprehensive healthcare services in the home and community settings. Patients served are typically elderly individuals with chronic medical conditions or mobility limitations who benefit from rehabilitation and supportive care. Key Responsibilities Conduct comprehensive patient evaluations in the home setting Develop individualized physical therapy treatment plans Provide therapeutic exercises and mobility training to improve function Educate patients and caregivers on home exercise programs and safety practices Monitor patient progress and adjust treatment plans accordingly Collaborate with physicians, nurses, and case managers to coordinate care Maintain accurate and timely documentation in accordance with clinical standards Patient Population Adult and geriatric patients Individuals with mobility limitations or chronic medical conditions Elderly patients requiring home-based rehabilitation services Required Qualifications Graduate of an accredited Physical Therapy program Active New Jersey Physical Therapist License BLS Certification (AHA) Minimum 2 years of experience with adult or geriatric patients Home health or senior care experience required Reliable transportation for home visits Preferred Qualifications Experience in pelvic floor therapy Training in Herman & Wallace Level 1A or equivalent certification Strong communication and patient education skills Physical Requirements Frequent standing, walking, and driving between patient homes Ability to lift up to 50 lbs occasionally Bending, reaching, and assisting with patient mobility when needed Why Consider This Assignment Competitive weekly compensation Work within a respected PACE program for senior care Consistent weekday schedule with no weekend or on-call requirements Opportunity to make a meaningful impact helping elderly patients maintain independence Collaborative interdisciplinary healthcare team environment
