Overview Provides advanced nursing clinical care to patients in the VNS Hospice Care program consistent with VNS Health philosophy and standards of nursing practice in relation to VNS Health goals and objectives and improves hospice utilization for hospice eligible patients. Works under general supervision. • Provides advanced nursing care and treatment to hospice patients in both the home and via telephonic visits, which includes assessment, diagnosis, prescribing medication, ordering and interpreting appropriate tests, and ordering treatments and DME., • Performs in-person pre-admission F2F visits when necessary, for certification of terminal illness by the Hospice Team Physicians and complete hospice admission to the Hospice Program when deemed hospice eligible. Perform F2F visits for re-certification at specified intervals for continued service in the Hospice Program., • Completes certification of death., • Provides primary medical and nursing care which includes prescribing medication to select patients, in accordance with collaborative practice agreement and within scope of practice of a nurse practitioner., • Documents patient care services in accordance with VNS Health standards., • Collaborates with patients, families, primary care physician and other team members to provide assessment and care planning. Consults with the primary care physicians and specialists in the community, when necessary, in relation to management and disposition plans., • Works with multidisciplinary team (e.g., nurses, social workers, physicians, etc.) on a consistent basis in order to provide collaborative care and make recommendations on additional services (home care, community services, etc.) as necessary. Provides medical and nursing expertise to patient care staff on issues and challenges involving evaluation of terminal decline, pain and symptom management, patient/caregiver education and proactive care planning., • Participates in the on-call NP coverage rotation after hours and weekends for in-person pre-admission F2F visits with completion of admission and completes admissions without required pre-admission F2F visits. Clinical Expectation: • 60% Clinical effort., • Clinical coverage will be assigned according to the expertise of the candidate and may include a combination of OBGYN clinical areas such as outpatient practice, inpatient, labor and delivery, and/or GYN surgery., • Responsibilities will include precepting/mentoring of attendings, teaching of residents and medical students, collaboration with midwives, physician assistants and other mid-level providers., • Call coverage will include in house call requirement monthly., • Call coverage may include back up call according to clinical expertise. Academic Expectation: • Promote scholarly activities by faculty and trainees., • Responsibility for the substance, quality, review and evaluation of all research programs within the Department., • Develop and oversee continuing medical education programs for staff at all levels within the Department. Administrative Expectation: • Ensure a culture of open dialogue with all staff, promote healthy team norms, and develop pathways to support staff and promote wellness., • Ensure a culture of diversity and inclusion within the Department. Identify and address areas of inequity by promoting activities that build awareness and acceptance of all individuals., • Ensure faculty meet performance expectations of their services assignment and customer service standards., • Create, implement and maintain defined objectives for OBGYN services for clinical improvement, growth, and the expansion of services. Qualifications: • Board Certification in General Obstetrics and Gynecology or OBGYN Sub-Specialty, • Minimum 8 years post-residency clinical and administrative experience, • Proven track record of clinical/academic productivity, • Experience working in an academic position with combined clinical, teaching and research experience, • Familiarity with Accreditation Council of Graduate Medical Education (ACGME) requirements for residency education, • Ability/Willingness to cover In-house OB/GYN service call, • Actively involved in faculty recruitment, selection and mentorship, • Demonstrated competence in leadership, management and team building, • Experience leading quality improvement and process improvement work in hospital/healthcare settings, • Must possess strong analytical, interpersonal, and communications skills Wages and Benefits include: Annual Base Salary: $358,955* based on 40-hour work week.
