Adult Social Worker - Locality Team, South London (Hybrid) UK Only Social Work England-registered Social Workers with the Right to Work in the UK will be considered. Location: South London Contract: Permanent, Full Time (36 hours per week) Salary: £42,060-£50,085 per annum (up to £52,194 unconsolidated maximum) Bonus: Welcome Bonus of £1,000 (paid in month one) or an annual Retention Bonus of £1,000 (both subject to payscale) About the role The Adult Social Care Locality Team help adults live as independently as possible. You'll work within integrated Locality teams aligned to Primary Care Networks, collaborating closely with NHS colleagues and wider partners to deliver holistic, person‑centred support. What you'll do Undertake Care Act 2014 assessments, ensuring personalised, outcomes‑focused planning. Complete Care Act reviews to ensure care and support plans remain proportionate, effective and strengths‑based. Lead safeguarding enquiries, addressing risk and promoting safety and wellbeing. Carry out Mental Capacity assessments and contribute to best‑interest decision‑making. Manage duty on a rota basis and contribute to DSTS and CHC activity. Prepare high‑quality reports and undertake court‑related work where required. Work collaboratively with professionals, families/carers and external agencies to co‑produce outstanding care plans.About you Registered with Social Work England and qualified in Social Work (or equivalent). At least 2 years' post‑ASYE UK experience within adult services. Confident working in multidisciplinary settings with complex mental and physical health needs. Skilled at communicating complex information to residents and professionals. Strong organisational skills, able to coordinate social care plans and support safe, timely discharges. Compassionate, resilient and motivated to deliver positive outcomes under pressure. Willing to travel outside the Borough to visit residents when required.Why consider this job? Lucy has cerebral palsy, developmental delay and requires adult support for all her care needs. She is a wheelchair user; the house is fully adapted to meet her needs, including hoists. Lucy is fed via a PEG, with a fully blended diet. Her epilepsy is well managed with medication, but she can experience seizures of varying strength/lengths. Lucy can experience panic attacks when she needs reassurance and comfort. Lucy can let people know her choices, likes and dislikes, through eye-pointing, facial expressions and vocalising/babbling. The Role at a Glance The role involves supporting Lucy with all aspects of her healthcare and daily living, including personal care. During term time, you will assist with the school run, either driving the adapted vehicle or accompanying her in the back of the car. Outside school hours—after school and during school holidays—you will help implement therapies, support Lucy to enjoy a range of activities, and ensure time spent together is engaging and fun. Support is provided on a 1:1 or 2:1 basis, working alongside another team member or a parent. There may be occasions where you supervise Lucy’s brothers, aged 11 and 6, if her parents need to focus on Lucy. The family would also like the support worker to accompany them on short family holidays in the UK or abroad. Days & Hours: Monday to Friday. Term-time: 7:00am to 10:00am (unpaid break) then 2:30pm to 7:00pm. School Holidays: 10:00am to 5:00/6:00pm. Additional weekend shifts are sometimes available if required and run from 8:00am to 6:00pm. This Job Would Suit This role will suit a reliable, nurturing and sociable *female support worker who can integrate comfortably into family life. You’ll enjoy spending quality time with Lucy, including taking part in fun activities and regular pamper sessions. Previous disability experience is preferred; however, with high-quality, client-focused training provided, the role is equally suited to someone with a childcare background who is enthusiastic about supporting a child with a disability. You will appreciate the importance of confidentiality and maintaining privacy within the family home. To support Lucy’s communication, clear spoken English is essential.
