The individuals we support have diverse needs, so our carers are expected to provide personalized care tailored to each persons preferences. The care provided should be of the highest quality, adhere to best practices, ethical standards, and align with Fairbrook Healthcare’s policies and procedures. Carers should encourage service users independence whenever possible while supporting their personal interests and hobbies. Additionally, carers are expected to continuously enhance their skills and knowledge by participating in ongoing training and development courses focused on safeguarding practices and adult social care. Key responsibilities:  Providing personal care and support with activities of daily living such as washing, dressing, using the toilet, eating and drinking.  Assisting individuals with mobility, movement and transfers  Medication administration  Assisting with meal preparation and dietary requirements  Supporting with domestic tasks such as cleaning and  Adhering to health and safety standards and ensuring that service users live in a clean and safe environment  Promoting and enhancing individuals’ independence and supporting individuals to access the community, social and leisure activities  Documenting the care delivered and maintaining up to date records of completed activities, medication administration and any incidents  Following all relevant safeguarding policies and procedures to ensure the safety of vulnerable adults  Reporting any safeguarding concerns to relevant professionals in a timely and appropriate manner  Working collaboratively with service users, their family members, other team members and other professionals to ensure high standards of care.  Carers must be able to communicate effectively with service users  Carers must have the communication skills to inform colleagues, management and professionals about the needs of service users  Carers to have good problem-solving skills and be able to adapt and address situations quickly Person Specification:  Experience in care and willingness to undertake further training. Experience:  Experience of working in adult social care settings such and domiciliary services, hospital and care homes  Previous experience in a similar role, such as personal care assistant or support worker (desirable)  Excellent verbal and written communication skills  Building rapport with service users  Experience of working with individuals with dementia and behaviour management skills  Ability to build strong rapport with candidates and other team members  Strong ability to demonstrate empathy and demonstrate, respectful and dignified care.  Ability to work autonomously in fast paced environment  Knowledge of safeguarding and health and safety protocols.  Ability to communicate clearly and effectively with individuals, families, and professionals. The Role We are looking for a dedicated support worker to provide holistic, ongoing support to young people facing a range of challenges including mental health issues, homelessness, poverty, addiction, or ACEs (Adverse Childhood Experiences). You will build professional, trusting relationships to assess needs, arrange appropriate support, monitor progress, and empower each individual toward independent living. Duties: To remain alert throughout the day to ensure the safety of those we support. To promote the rights of our service users and ensure that they are treated with dignity and respect. To record and update information within a support plan about the service users you support, ensuring all interactions and support provided are accurately documented to assist in the writing of monthly reports. To empower our young people to achieve the outcomes they identify as desirable for independent living. Build empathetic, professional relationships with young people to develop trust and offer emotional and practical support - especially during times of crisis. To support with tasks such as medication and domestic duties, including cleaning of the house, as well as conducting and participating in regular health and safety checks. Maintain and review personalised risk assessments, support plans, ensuring the active involvement of the young person to promote a person-centred approach. Work closely with external agencies (e.g., social workers, healthcare professionals, police or ambulance service) and supporting young people to various appointments including health visits, college interviews, home office appointments or social worker meetings. Identify development and training needs to promote employability, including help with education and college applications, whilst accurately recording and documenting this with feedback to the keyworker as required. Promote inclusion by encouraging participation in local community activities and events. Candidate Attributes and Desirable Skills: Good verbal and written English. Minimum 1 year experience in the children and young persons’ care industry. Have an enhanced Child and Adult workforce DBS, or willing to obtain one at the company’s expense. Proof of right to work in the UK. Understanding of the importance of safeguarding and experience of implementing safeguarding procedures. Familiarity with CQC and Ofsted standards is desirable. Strong communication and interpersonal skills, with the ability to build positive working relationships. Ability to work independently and as part of a team, with a positive and proactive approach. Ability to manage own diary and time. Understanding of the principles of GDPR and confidentiality. Ability to work independently and as part of a team, with a positive and proactive approach.
