We are seeking passionate, enthusiastic Registered Nurses to join the Neighbourhood Nursing Service across the Barnsley area. The role involves delivering comprehensive nursing care to housebound patients, including wound management, chronic disease management, promotion of self‑care, terminal/palliative care, and administration of IV antibiotics for admission avoidance. Working both independently and in partnership with the wider integrated care team, you will support the District Nurse and participate in clinic sessions. The service operates 8 am–8 pm, and staff may need to travel between neighbourhoods. Full and part‑time positions are available. • Undertake duties under the supervision of the District Nurse and support patient care in the community., • Deliver evidence‑based planned care that is safe, effective and compassionate, enabling patients to reach optimum functioning and independence., • Adapt and provide nursing care across home and community settings., • Assess, plan, implement and evaluate patient care, reporting and escalating information to appropriate health and social care professionals., • Work in an integrated partnership model with primary, secondary, social care and voluntary sector to improve health for individuals and families, especially the most vulnerable., • Maintain statutory, mandatory and core training and preparation required for the service., • Participate in the training of learners and staff inductions where appropriate., • Delegate work to other team members., • Establish good working relationships with Primary Health Care Teams., • Registered Adult Nurse with current UK professional registration and evidence of meeting revalidation requirements., • Ability to undertake the duties and demands of the post with a satisfactory sickness record over the past two years., • Able to fulfil Occupational Health requirements for the post., • Responsive attitude and professional approach., • Dress appropriately for the environment., • Current driving licence and access to a car during working hours (reasonable adjustments considered)., • Ability to work effectively as part of a team and as a lone worker., • Good time‑management and organisational skills., • Excellent written and verbal communication skills., • Understanding of government legislation and documentation affecting NHS and primary care., • Health promotion and motivational interviewing skills., • Teaching and health education skills., • Community nursing experience., • Evidence of teaching skills and skills in male catheterisation, venepuncture, syringe driver, ear syringing., • Information technology skills., • V150 nurse prescribing qualification., • Experience of innovative practice., • Band 5, Permanent, Full‑time or part‑time, flexible working, 8 am–8 pm., • Salary £32 073 to £39 043 a year pro‑rate., • Work located at Kendray Hospital, Doncaster Road, Barnsley, S70 3RD.
