PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings. New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens. This is a grant-funded position ending July 31, 2026. Specifically, the Navigator Supervisor will: • Directly supervise approximately 3-5 Navigators supporting the WholeYouNYC Social Care Network., • Provide limited screening and navigation support (est. 25-30% of time) on behalf of the SCN, as needed, including, but not limited to:, • Conduct outreach to Medicaid populations residing in the SCN’s region (Brooklyn, Manhattan, Queens) utilizing a standardized assessment tool to assess their health-related social needs., • Assess client eligibility for a range of services and refer to appropriate community-based social supports., • Leverage your social services experience and expertise to determine the most suitable resources and service providers for clients based on needs, eligibility and preferences., • Develop and maintain an in-depth knowledge and understanding of the range of services (including eligibility criteria) available in the SCN and existing local social services infrastructure., • Carefully document outreach, screening, and referrals in the SCN data and IT platform, following defined network policies and procedures., • Follow up with clients to confirm that needs have been addressed., • Inform SCN learnings based on client experiences and insight about Medicaid population needs., • Act as a coordinated intake and social navigation subject matter expert, providing leadership, guidance and best practice techniques to CRN staff and network partners., • Ensure high-quality and timely execution of SCN activities by the team supporting the SCN., • Assist with the development and/or revision of SCN service delivery workflows., • Coordinate and facilitate both new hire and ongoing training for Navigators (and other team members, as needed) including, but not limited to: SCN onboarding, screening, workflow, partner relationship management, outreach strategies, and effective use of the SCN data and IT platform., • Ensure effective use of the SCN data and IT platform by Navigators and network partners. Provide insights about platform usage, issues and opportunities to inform training, technical assistance needs and system enhancement requests., • In collaboration with the Manager, Coordination Center Operations, coordinate Navigator team schedules and define team productivity targets and work priorities., • Monitor team productivity. Identify training gaps and performance issues. Develop and implement action plans to enhance team performance., • Proactively review key productivity and performance metrics, data and reporting. Collaborate with Quality Improvement and Evaluation to develop and implement improvement initiatives., • Provide both formal and informal performance review and feedback to supervised Navigators to foster their career growth and development., • Coordinate and lead team meetings and 1:1s with supervised Navigators., • Lead and participate in network engagement meetings, staff / team meetings, mentoring meetings, planning meetings and others, as needed., • Other duties as assigned and prioritized by WholeYouNYC SCN management. Qualification and Experience: • Bachelor’s degree with coursework in community health and/or equivalent., • 3-5 years’ experience working in a care navigation / coordination / intake capacity, specifically within the human services sector., • 2 years’ experience in a supervisory role., • Bilingual or multilingual preferred., • Computer skills and ability to navigate analytic technology solutions for documentation, tracking and review of data., • Strong leadership and management skills including the ability to set team priorities, identify and resolve issues constructively, assess staff performance, provide career mentoring and coaching., • High degree of self-organization and ability to work independently., • Attention to detail with the ability to analyze data and make informed decisions., • Excellent communication (listening, written, oral) skills and ability to communicate effectively in-person, via email and/or phone with clients and network partners who work across various sectors., • Comfortability presenting to various audiences (e.g., community-based organizations, healthcare providers, community members) and succinctly community key messages., • Knowledge and experience working with vulnerable populations; possess cultural competency skills., • Enthusiasm for assisting New Yorkers of diverse backgrounds., • Knowledge of motivational interviewing and/or other coaching techniques preferred.
