Summary of position: The Quality Assurance (QA) Radiological Technologist position encompasses responsible, administrative and clinical activity of varying degrees of latitude. Clinical level judgement is needed in the daily supervision of radiological functions and patient care, and departmental projects. Responsibilities include: • Review exam orders, consult with radiologist, schedule patients and arrange for transportation as indicated., • Identify patient and document all information pertinent to final report in the RIS and on images (PACS)., • Prepare allpatients (infants, children,adolescents, adults andgeriatric) for radiological procedures requested bythe clinician. Arrange for proper protection of patients, public andstaff from infectious diseases/materials., • Perform allgeneral diagnostic imaging and routine examination in CT and Angiography., • Assists inperforming daily checks of radiographic imaging equipment, immobilization devices and other necessary accessories., • Enters data and maintains documents and logsas required andmandated by the department QAguidelines and various regulatory agencies., • Maintains adequate stocks of supplies andmaterials needed foruse in thedepartment., • Checks equipment for defects prior for useand assures proper use of such equipment through instruction and review., • Report any unexpected patientincidents in accordance with theoccurrence report protocol., • Maintain and exercise all mandated radiation safety regulations. Maintain basic care of all the imaging equipment. All malfunctions are to be reported to management staff whether mechanical, electrical or suspect and service request documentation completed. Maintain a clean and safe work environment for both patients and staff., • Perform other duties as deemed necessary (by supervisory oradministrative staff) inthe event of emergencies ordisasters., • Continue professional growth and development of job-related skills on an ongoing basis. Participate in continuing education activities., • Ensure allexaminations are accessioned, documented and sentto PACS forreading. Monitor PACS and RIS incomplete worklists to ensure all exams arecompleted., • In accordance with HIPAA (the Health Insurance Portability and Accountability Act), staff members will exercise due diligence in ensuring patient privacy and confidentiality., • Identifies activities thatthe network hasimplemented to promote patient safety andtakes all necessary precautions to ensure a safe environment for patients, visitors andstaff. In addition to the Radiology Technologist job functions, the QA Technologists is responsible for the following: • Organize and file maintenance reports for each building in regards to dentistry and radiology x-ray equipment and ensure dentistry and x-ray equipment follow the Department of Health codes, policies and procedures., • Participates in quality assurance activities, including coordination, control and maintenance of technical equipment., • Establish a daily/weekly/monthly quality control routine for monitoring, evaluating, and maintenance of X-ray equipment to ensure optimal performance and stability., • Documenting and maintaining records for the quality control program in accordance with applicable regulations, legal requirements, accrediting agencies and recommendations from equipment manufacturers., • Performs research and participates in special projects involving evaluation of equipment and delivery of service., • Work with our radiologists to obtain updated documentation of primary diagnostic monitors., • Distribute/collect dosimeter badges from each employee/facility that works with x-ray equipment., • Recommends area assignment of personnel to coordinate activities within the department with other activities and services and represents the department in interdepartmental operations and meetings., • May be assigned duties and responsibilities of X-ray Director in regards to radiology department workflow. Department Preferences: • Possession of a current valid license as a General Radiological Technologist issued by the State of New York., • A minimum of two years of experience as a Radiological Technologist., • A current member of the American Registry of Radiologic Technologists (ARRT)., • Valid BLS required., • Must possess a valid driver’s license., • Must be proficient in Microsoft Office Suite. Minimum Qualifications: Assignment Level III 1. Position Overview Under supervision of the Site Medical Director, the Physician Assistant will provide comprehensive, compassionate, and thoughtful care to our patients in the New York City jail system. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Summary of position The Supervising Psychiatrist is a psychiatric leader of a facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Senior Psychiatrist (if applicable) to ensure that safe, holistic, and evidence-based psychiatric care is provided to incarcerated patients with varying levels of mental health needs. A team-based approach to care is critical; one that incorporates all relevant mental health and medical disciplines as well as security staff. A major component of this position will be training, recruiting and retaining high quality staff and providing strong leadership and mentorship for existing staff. Responsibilities Include (depending on location): • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. This supervision must include both in vivo reviews of clinical skills as well as chart review., • Close collaboration, including regularly scheduled meetings, with other relevant disciplines within the mental health, medical, nursing and operations departments, as well as the Department of Correction, • Ensure that medication renewals are completed in a timely manner, • Educate clinical staff about new developments regarding best practices in all aspects of psychiatric care, with specific attention to the challenges and issues facing a jail setting (e.g., influence of legal case, dual loyalty, medication diversion), • Ensure compliance with all regulatory standards, • In collaboration with the Mental Health Unit Chief, oversee the orientation of new staff to the facility or program, • Conduct regular and multi-disciplinary case conferences, • Report on an as-needed basis to the CHS Morbidity and Mortality review committee., • Work in collaboration with the Mental Health Unit Chief to manage clinical schedules, vacation requests, CME activities and other relevant activities as they pertain to psychiatric staffing and education, • Participate in policy revision and creation, and practice guideline creations, related to psychiatric care, including input into the psychiatric medication formulary, • Lead at least monthly interdisciplinary mental health staff meetings in coordination with the Mental Health Unit Chief, Clinical Supervisor and Senior Psychiatrist (if applicable)., • Personnel management of clinical staff, including setting quality standards, establishing effective recruitment and retention strategies, and managing workforce complaints, • At least 75% of time devoted to direct clinical care, which may include direct supervision of psychiatric trainees. For smaller facilities or programs with less supervisory need, more direct patient care will be expected, • On-call responsibilities for two weeks at a time, one to times/year, for off-hours issues that require high level clinical intervention 1.Master's degree from an accredited college or university in Healthcare Administration, Health Care Planning, Business Administration, Public Administration or a related discipline; and seven (7) years of progressively responsible experience in health and medical service administration, public administration, personnel and labor relations, finance or appropriate functional discipline with an emphasis on planning, liaison and inter-organizational relationships, or related administrative or managerial functions; or 2.Bachelor’s degree from an accredited college or university in disciplines, as listed in “1” above; and eight(8)years of progressively responsible experience in areas, as listed in “1”; or 3.Satisfactory equivalent combination of education, training and/or experience. However, all candidates must have a minimum of a Bachelor’s degree in disciplines, as listed in “1” above. North Infirmary Command (NIC), West Facility (WF), and the Contagious Disease Unit (CDU) are the jail facilities that house a significant number of incarcerated individuals with advanced age, complex medical problems, disabilities, and/or enhanced nursing needs. The Senior CL Psychiatrist working in this facility will lead the team in diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for individuals housed in this facility with comorbid mental illness, including direct patient care as well as supervision of other psychiatric providers. The Senior CL Psychiatrist will also consult on the care of medically complex patients in other jail facilities. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for elderly and medically complex patients with comorbid mental illness., • Directly treat and oversee the psychiatric care of additional patients housed in NIC/WF/CDU, • Direct patients to the appropriate level of mental health care, within the facility or in the hospital as needed., • Close communication with psychiatric and medical providers at Bellevue Hospital to ensure continuity of care as patients transition between the two facilities., • Ensure the highest possible quality of mental health care both in the facility and in collaboration with the other jail facilities in the system. Provide ongoing monitoring of the patient in the home care setting and escalates any clinical changes to the physician or non-physician provider as needed. 18. Utilizes input from patient, caregiver, MD and peers to devise a Nursing Care Plan which is feasible within the physical, financial and emotional resources of the patient/caregiver and the Department of Health regulations. 19. Evaluates patient's response to plan of care as necessary, consistent with physician's orders., 20. Reviews and revises plan of care as necessary, consistent with physician's orders., 21. Plans activities necessary for effectuating the timely discharge/transfer of patient, and notifies all appropriate parties including patient's physician. 22. Collaborates with case managers and other disciplines regarding the patient’s progress and discharge plans., 23. Demonstrates a professional attitude and ethical conduct in compliance with departmental standards., 24. Involves the patient and caregiver actively in goal setting., 25. Establishes and maintains a cooperative, effective working relationship with the staff, physician and nonphysician providers, Case Managers and other hospital personnel, outside vendors, agencies and the community at large. 26. Attends weekly to bi-monthly team meetings and participates in case conferences and case load reviews on a weekly to bi-weekly basis. 27. When needed completes Physician’s Orders in a timely manner., 28. Completes admission paperwork, including OASIS, Physician and Nursing POCs and progress notes within 48 hours of visit encounter. 29. Job Description PURPOSE OF POSITION Under the direction of the supervising Assistant Director of Nursing in Community Care, provides community-based professional nursing care to patients in group and private homes, as well as those without housing across New York City. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and the provision of care supportive to and restorative of life and well-being.
