Each member of the Camp Kehilla Staff team will create a safe and enjoyable environment for our campers and staff while maintaining the high quality of programming associated with Camp Kehilla. Division Heads act as a liaison between the Director and the divisions for which they are responsible. The Division Head oversees an age division consisting of 2 groups of campers and their group leaders. They are responsible for ensuring all activities and special events run within assigned groups smoothly, oversight of support and behavior management of campers, supervision and support of staff, ongoing communication with families, and oversight of day to day operations of assigned groups. Job Description The Assistant Director of Wellness uses his/her specialized knowledge to ensure the Resident’s care and management needs are being adequately and correctly met and assists the Director of Wellness as an additional liaison between Residents, the Residents Legal Representatives, their families, administrative staff, and community-based service providers. Responsibilities include the direct supervision of the Wellness and Reflections Unit; motivation, supervision, and in-service training/education of the Resident Service Aides. Job Description BCBA/LBA Job Description As part of NESS Behavior Consulting (“NESS”), it is the responsibility of the hybrid BCBA to conduct various descriptive and systematic behavioral assessments (e.g., functional analyses) and skill-based assessments (e.g., ABLLS-R, VB-MAPP, AFLS, etc.) for clients across Nassau County, Suffolk County, Queens, Brooklyn, and the Bronx. The BCBA then provides behavior analytic interpretations of the results and designs comprehensive ABA treatment plans based off of evidence-based practices and behavior analytic research literature. The BCBA provides training and supervision to Board Certified Assistant Behavior Analysts, Registered Behavior Technicians, and other responsible parties tasked with the implementation of behavior analytic interventions. Essential Functions cont’d • Assist in the development and monitoring of staffing plans to ensure each person supported health and safety including the possibility of providing direct care when necessary., • Implement and document the delivery of all Residential Habilitation services which may include but is not limited to: person supported skill development training in a variety of areas including: medical concerns, vocational concerns and recreational services., • Assist with the completion and submission of all paperwork as outlined in the Manager’s Monthly Checklist within the assigned timeframes., • Implementation of person supported behavioral support plans/strategies and documentation of person supported behaviors., • Assist in the hiring process and recruitment process., • Assist in the monitoring of staff training requirements., • Assist in developing and managing income and expense budgets to ensure the fiscal integrity for the program., • Monitor the person supported personal allowance accounts to ensure the fiscal integrity of these accounts., • Assist the person supported with transportation., • Participate in the admission and discharges from the program., • Meet with the person supported and family members to share information, solicit feedback, and discuss issues germane to improving the quality of services., • Maintain collaborative relationships with other ACLD programs and departments to maximize service coordination and integration., • All other duties, as assigned. Job Title & Role Description: The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes. Skills Required: • Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.)., • High school diploma or GED required., • A valid driver's license, auto liability insurance, and reliable transportation to travel within the assigned territory., • Experience in chronic care management or working with chronically ill/elderly patients., • Technologically proficient with basic computer skills (typing, using EMR systems)., • Experience with motivational interviewing, trauma-informed care, and care coordination., • Strong interpersonal communication skills with the ability to engage patients and team members effectively., • Ability to prioritize tasks, manage schedules, and work independently in an unstructured environment., • Multi-lingual skills are a plus but not required., • Prior home care experience is beneficial.
