Job Description The Role We are seeking a Community Engagement Specialist based in the Hudson Valley, NY region to join our growing team and play a vital role in expanding access to aptihealth’s virtual behavioral health services. In this position, you will serve as the face of aptihealth in the field, building relationships with healthcare providers and connecting patients directly to the care they need. You will call on physicians, hospitals, skilled nursing facilities, discharge planners, and case managers across your assigned territory to promote aptihealth’s proprietary telehealth platform, which connects patients to therapy, psychiatry, and case management under one digital roof. You will also engage directly with patients across multiple care settings to enroll them in services and ensure a seamless transition into care.
