Job Description The Transitions Family Treatment/Rehabilitation (FT/R) program is a specialized, home-based, borough-wide prevention initiative utilizing the Solution-Based Casework (SBC) model. It serves families facing child welfare-related challenges due to substance abuse and/or mental health concerns, with the goal of repairing relationships and ensuring child safety so families can remain together. The Program Coordinator plays a key leadership role, managing and implementing essential program operations. This position works closely with the Program Director to ensure the program exceeds contractual commitments set by the NYC Administration for Children’s Services (ACS). PURPOSE: The purpose of a Trauma Systems Therapy (TST) Therapist in Preventive Service Department is to provide the TST model for families with child(ren) (ages 5 and over) who are impacted by trauma and there may be risk for foster care placement. The Preventive Services Department provides child welfare services using Evidence Based models to ensure that families are able to provide safe environments for their children, keep children safe in their homes and reduce the need for foster care. Prevention is an essential part of the child welfare continuum and requires a commitment to the wellbeing of all members of a client family. All services are provided in a family’s home and community. POSITION OVERVIEW: The Therapist works with families to assess their needs, develop intervention plans, and deliver therapy sessions that are tailored to the family's unique circumstances. They collaborate with other service providers to ensure a comprehensive approach to the family's care. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. PROGRAM BACKGROUND: MercyFirst’s A Safe Way Forward Program (SWF) provides child welfare involved families affected by intimate partner violence (IPV) with comprehensive prevention services including therapeutic case management and evidence-based intervention modalities to address IPV. This joint prevention-clinical services model offers comprehensive trauma-informed services to the whole family, including the survivor of IPV, the person causing harm (PCH), and to children exposed to the IPV in the home. Range of services includes in-home visitation, crisis intervention, case management, advocacy, individual, group and family counseling, psycho-education and support, and referral to other appropriate services. The SWF program will serve a total of 65 families residing in Brooklyn. POSITION SUMMARY: The Therapeutic Case Manager is responsible for screenings and assessment, engagement, safety planning and coordination of services and treatment for members of the family system experiencing IPV. Approach to service is family-centered, trauma-informed, strengths-based and emphasizes both short-term and long-term goals to restore and maintain Safety for all family members, Protect children from future exposure to IPV, and promote Healing and Improved Family Relationships. The qualified candidate will be responsible for adherence to reporting requirements and timelines and meeting programmatic outcome measures. Job Description Astor’s Prevention Services/Family Connections is a time-limited, evidence-based model that utilizes a home-based approach to address both the concrete and clinical needs of the family. Prevention services utilize comprehensive family assessment tools to identify behaviors related to risk and to develop Core Outcomes that address the identified child welfare concerns within a family. The Case Planner provides in-home counseling services to families in need of support in developing skill and competencies that will enable them to keep their children in the home and out of placement. The Case Planner will maintain a caseload of 12-14 families and provide change focused interventions with families to improve parental functioning, address life stressors, and improve overall familial functioning. This is happen over a course of 6 to 12 month length of service and a minimum of 2 face to face contacts with each family per month. The Case Planner will have to have at least 1 late night per week to meet the needs of working families. You must have: · Must have a Bachelor’s Degree. · Case Planner experience required.
