Job DescriptionJob Title: Psychiatric Nurse Practitioner Job Summary: CASES is a New York City agency working at the forefront of the intersection between behavioral health and involvement in the criminal justice system. The Clinic division consists of a continuum of services for justice-involved individuals with serious mental illness; The Nathaniel Clinic is an OMH-licensed mental health outpatient treatment and recovery services clinic that provides outpatient mental health services to adolescents and adults, with a focus on providing care to those involved in the criminal legal system. The PNP/PMHNP/NPP will work as part of a multidisciplinary team providing coordinated clinical assessments and assertive treatment in all stages of illness. The team nurse practitioner engages the consumer in shared decision making about medication and the next steps in medication treatment. Medication management will be guided by a medication algorithm that provides information about evolving best practices. Salary: The salary range for this role is $160,000 - $170,000 per year. Shift Hours: The Psychiatric Nurse Practitioner is full-time position working Monday – Friday from 10:00am – 6:00pm, or 11:00am – 7:00pm. We're open to discussing part-time hours with applicants! Location Address: The position may be based either fully at 2090 Adam Clayton Powell Jr Boulevard, 4th Floor, New York, NY 10027, or split between offices at 424 E. 147th Street, Bronx, NY 10455. Our office is easily accessible by public transportation. Workplace Flexibility: Hybrid schedule with one virtual day available upon completion of the 90-day probation period and contingent upon acquiring a sufficient number of telehealth clients. What You Will Be Doing: • Provide compassionate clinical treatment in accordance with the scope of practice and regulations governing Certified Community Behavioral Health Clinics, • Conduct up to 12-14 follow ups or 4-6 IPEs per day, • Conduct psychiatric assessments, • Assist clients, family members, and significant others with concern and empathy, • Work in collaboration with clients, family, and other collaterals to support recovery goals, • Respond to walk-ins and unscheduled client needs and provide expedited services to clients based on risk assessment status, • Prescribe medications indicated by diagnosis and lab results, teach clients about side effects, and monitor response to prescribed medications, • Complete buprenorphine training and obtain the waiver to prescribe, • Work collaboratively with therapists and registered nurses providing specific goals and measurable objectives for inclusion in person/family-centered treatment plans, • Ensure services are person/family-centered, recovery-oriented, and trauma-informed, • Promote integration of primary care working collaboratively with the onsite Federally Qualified Health Center (FQHC) medical providers and the clinic’s nurses to ensure the effective delivery of health monitoring services, • Promote and deliver integrated treatment to consumers with co-occurring substance use disorders, • Utilize best and evidence-based approaches to treatment consistent with CASES’ organizational culture; health/mental health integration, rehabilitation and recovery, and understanding incarceration on treatment engagement, • Provide culturally competent services that are individualized to client needs and reflect the integration of race, ethnicity, culture, primary language, immigration status, developmental status, criminal justice status, sexuality, age, and gender, • Helps maintain a safe and clean environment, ensuring proper use and disposal of medical supplies/equipment as required, adheres to infection control protocols, and notifies their supervisor immediately of any potential safety hazard identified in the office/client-care environment. What We Are Looking For: • A Nurse Practitioner degree with specialized training or experience in Psychiatry. A current license to practice as a nurse practitioner in the State of New York. Board certified preferable., • A current DEA registration for the prescribing of controlled substances. ANCC certification or pending application is required. NYS OPRA status approved or application pending and a Health Commerce Account in place by the onset of employment., • Extensive experience working with people with mental illness, serious mental illness and knowledgeable about risk assessment and risk management, • To be eligible for appointment and to maintain employment, you cannot be listed as an excluded individual or entity on any of the Federal and/or State Medicaid and Medicare exclusion lists. By providing alternatives to incarceration, community-based sentencing options, links to meaningful services, and community-led justice interventions, our programming keeps individuals out of jail, restores community relationships, builds local leadership, and helps participants avoid further criminal justice involvement. Reporting to the Clinical Director, the Clinical Supervisor will be responsible for supporting the clinical practice of the Alternatives to Incarceration (ATI) program. The Clinical Supervisor will be responsible for reviewing assessments and coordinating treatment planning meetings with stakeholders and providers. The Clinical Supervisor will also provide direct clinical support and coverage when needed, provide support with overseeing the day-to-day operations of the clinic, assist with trouble shooting and provide crisis intervention strategies for more challenging cases. Additionally, the Clinical Supervisor may also be responsible for supervising members of the clinical team. Responsibilities include but are not limited to: • Provide clinical operational support to the ATI clinical staff and manage same-day programming;, • Partner with the Intake Coordinator to provide clinical support to staff when needed;, • Support the intake team with triaging of participants admitted and administer an assessment tool, as needed;, • Schedule and facilitate team meetings and clinical group supervisions for clinical and non-clinical staff;, • Provide clinical services to participants including individual counseling and case management sessions;, • Assist clinical staff and other departments with crisis intervention and de-escalation, as needed;, • Support the clinic team in clinical and task supervision;, • Support the implementation of clinical interventions, case management and audit process for ATI staff;, • Support case assignment and monitor compliance through ongoing caseload audits;, • Work closely with senior clinical staff to ensure policies and procedures are consistent across the program;, • Support with data management and tracking and monitoring program performance for both intake and clinical staff oversee evaluation activities;, • Assist the Clinical Director with communication between court operations, intake and clinical staff as needed;, • Provide support with facilitating, scheduling, and monitoring on-site psychoeducational groups;, • Provide support with case management duties and individual counseling to high need participants and additional comprehensive, clinically-informed case management services to individuals with mental illness to help address such issues as substance use, mental health, trauma, domestic violence, unstable housing and vocational needs;, • Assist the Clinical Director and Deputy Project Director in creating new community partnerships;, • Complete all necessary documentation and input data into a computer database;, • Monitor and report on compliance to court stakeholders and work with defense attorneys to bring program participants into compliance, as needed;, • Assist the Clinical Director with individual and team clinical supervisions, as needed;, • Provide coverage for the Clinical Director when needed by reviewing assessment recommendations, providing support to team members, assist with crisis intervention, and other tasks as needed;, • Assist the Clinical Director with facilitating trainings;, • Participate in trainings and attend staff meetings;, • Collaborate closely with managers across other departments and teams within Bronx Community Solutions;, • Assist with data collection and reporting, quarterly reports, and grant writing, as needed;, • Attend Bronx Community Solutions events after hours, as needed; and, • An LMSW with at least 2 years of supervisory experience and 3-4 years of relevant experience. LCSW with at least 2 years of supervisory experience preferred;, • Bilingual (English/Spanish) strongly preferred;, • Excellent written skills are a must;, • Candidate must have experience working with individuals who have experienced trauma, substance abuse, mental health, domestic violence and/or sexual assault;, • Familiarity with trauma-informed care practices;, • Candidate must be skilled communicator able to work in multi-disciplinary setting and maintain strong relationships with multiple agencies and organizations both onsite and in the community;, • Extensive group facilitation experience required and co-facilitation experience preferred;, • Experience with drug/alcohol treatment, mental health services, co-occurring disorders, trauma-informed and anti-oppressive practice strongly preferred;, • Experience facilitating conversations related to identity, oppression, and privilege and must excel at client engagement;, • Highly organized and excellent time management skills; and Position Location: Bronx, New York. Compensation: The compensation range for this position is $78,000 - $98,500 and is commensurate with experience.
