Job Description POSITION: Patient Services Clerk DEPARTMENT: Health Centers LOCATION: Levittown Health Center STATUS: Full-Time (35 hours a week), Non- Exempt, Monday through Friday hours based on health center hours or operations, some evenings, occasional weekends. Hourly Pay: $20 per hour General Description: The position of Patient Services Clerk is a full-time appointment requiring a minimum of 35 hours of service per week to the College. Function: • Reports to the Director of Clinical Operations., • Support the Mission of the College., • Collaborate with the Dean of Clinical Education, Assistant Dean of Clinical Education, Billing Manager, Assistant Billing Manager, health center staff, and other members of the College faculty, staff, and administration. Specific Duties and Responsibilities: • Greet and sign-in patients when they arrive for their appointment. Ensure patients complete the necessary paperwork upon arrival., • Record and track referrals as necessary. Ensure appropriate insurance paperwork is filled out and submitted as required., • Verify patient coverage and co-payments via telephone or carrier website., • Have an understanding of medical insurance, CPT, and ICD-10 coding, verifying insurance benefits., • Actively supports clinicians and students as an enthusiastic participant in departmental meetings and actively identify ways to improve processes and service to patients., • Perform all duties within HIPAA regulations., • Assist with the hardship Account program for patients – execute proper paperwork and maintain timely documentation per episode of care. Communicate with clinicians and patients regarding status., • Assist with annual execution of the Patient Financial Policy with patients., • Identify any issues related to OIG requirements and follow reporting as required., • Optimize provider schedules and patient satisfaction with efficient scheduling., • Answer telephones, schedule patient appointments, take and relay messages as needed., • Collect payment from patients for services provided or any supplies that are purchased., • Enter information from the Patient Encounter Sheets into Electronic Health Records system (EHR)., • Assist with inventory management of nutritional supplements and other retail items., • Assist with the washing and drying of health center laundry., • Make copies and duplicate files for clerks and clinicians. Maintain an adequate supply of clinic forms., • Promote and enforce the policies and procedures of the health center., • When opening the health center, retrieve messages, and ensure the desk is ready to open., • Secure all rooms in the health center at the end of the day., • May be responsible for closing out the cash drawer at the end of the day if working nights or weekends., • Assist in distribution of materials during orientation., • Any other duties as assigned. Service Role: • Participate in College committees as requested. Methods of Accountability: • Verbal and written communications with the Dean of Clinical Education, Assistant Dean of Clinical Education, and Billing Manager., • Compliance with OIG and HIPAA protocols., • Written feedback from student clerks and other College consumers., • Annual performance evaluation from the Director of Clinical Operations. Education, Training, and Experience: • High school diploma or equivalent required, and previous experience working in a medical office recommended., • Customer Service skills and experience preferred. SCHEDULE Monday – Friday, 8:30AM – 4:30PM SUMMARY Family Service League is seeking a full-time Case Manager for the Health Homes program in Amityville. The Case Manager will meet the intensive needs of the population while meeting visit requirements as stipulated by the DOH for the Health Homes program. Clients include individuals with behavioral health, medical, and/or substance abuse diagnoses. RESPONSIBILITIES • Conduct home visits, community outreach, and engagement using a culturally sensitive, trauma-informed approach to build trust with adults living with chronic medical, mental health, and/or substance use conditions., • Complete comprehensive, consumer-driven assessments to identify strengths, challenges, and service needs; develop and regularly update individualized care plans reflecting client goals, preferences, and wellness objectives., • Coordinate and monitor behavioral health, physical health, and social service supports in alignment with each client’s needs and service plan., • Build and maintain linkages with both natural supports (family, friends, community) and formal resources, including hospitals, outpatient clinics, rehabilitation programs, self-help groups, housing agencies, and social service providers., • Manage a caseload of approximately 40–50 clients, providing consistent contact through home visits, phone calls, and field-based engagement in accordance with client acuity and program standards., • Advocate for and coordinate care across multiple systems: medical, behavioral health, and social service, to ensure access to comprehensive, continuous, and integrated care., • Monitor client progress toward goals, supporting self-management and helping to reduce preventable hospitalizations and emergency room utilization., • Collaborate closely with multidisciplinary providers and participate in joint case reviews, ensuring coordinated, goal-directed care., • Maintain detailed, accurate, and timely documentation in the electronic health record (EHR), ensuring all required clinical and eligibility documents meet standards set by the Department of Health, Office of Mental Health, Health Home program, Suffolk County Division of Mental Hygiene, and Family Service League., • Participate actively in supervision, team meetings, and case conferences, including ACM staff meetings and agency-wide meetings, to share resources, address client needs, and contribute to program improvement., • Maintain a positive, professional rapport with the Suffolk County Adult SPOA and other referral sources to facilitate client enrollment and smooth transitions of care., • Respond promptly to crises or urgent client needs, implementing safety plans and coordinating follow-up support., • Serve as a role model of professionalism and FSL’s mission, promoting empathy, empowerment, and recovery-focused practice., • Complete all required reports and documentation accurately and within established timelines to support billing, compliance, and program outcomes., • Follow all agency and program policies and mandates, including confidentiality, safety, and quality assurance standards., • Perform other related duties as assigned to support program operations and the overall mission of Family Service League., • All other duties as assigned. QUALIFICATIONS Bachelor’s Degree in a health-related field is required. Minimum of 2 years of experience providing direct services to people with serious mental illness, medical conditions, developmental disabilities, and/or substance abuse preferred. Proficiency in Microsoft Office required; experience with an EHR is preferred. Strong assessment, engagement, organizational and time management skills. Excellent verbal and written communication skills are required. Valid and clean New York State Driver’s License required. PHYSICAL REQUIREMENTS This position often requires sitting and working at a computer for extended periods of time, traveling to different FSL offices, community meetings, home visits, etc.
